Este articulo afronta los desafios de una estrategia brasilena en el sistema de salud para mejorar la calidad de su respuesta asistencial y gerencial: la Politica Nacional de Humanizacion. Independientemente de analisis politica, conceptual o de resultados, discute la humanizacion del cuerpo en la salud por medio del confronto con sus reificaciones en la clinica, tal como estan presentes en la soberania de los sistemas profesionales sobre las practicas, en la accion disciplinante que legitima las conductas del tipo diagnostico-prescripcion o en la diseminacion de mecanismos que, en nombre de la longevidad de los individuos, estancan la produccion de singularidad de la vida. Las nociones de cuerpo de organos, cuerpo ex-organos y cuerpo sin organos se usan para los problemas de la clinica (atencion) y la nocion de lazos y perspectivas para discutir la transformacion de realidades (gestion). La educacion y la participacion se consideran indisociables de atencion y gestion.

Una acción micro-política y pedagógica intensa: la humanización entre lazos y perspectivas

The inclusion of mental health care actions in the context of the Brazilian Public Health System (SUS; Sistema Unico de Saude) contributes to the consolidation of the Brazilian Psychiatric reform and demands redirecting the practices of family health teams with users with mental health needs. The objective of this study is to identify and analyze the scientific production and actions developed by family health team professionals in mental health care. Systematic analysis originated the following themes: home visits to mentally ill patients and their relatives; attachment and welcoming; referrals; therapeutic workshops. In conclusion, the mental health actions developed in primary care are not performed consistently and depend on the professional or on the political decision of the administrator, which shows that professionals should use new practices to develop comprehensive care, and, therefore, there is a need to invest in improving the qualification of the professionals.

Conversation, Language, and Possibilities: A Postmodern Approach to Therapy

Resumo Este artigo discute sentidos produzidos por agentes comunitarios de saude (ACS) de uma cidade pequena de Sao Paulo sobre sua (des)valorizacao, considerando seu trabalho com redes sociais na Estrategia Saude da Familia (ESF). Foram realizados grupos de discussao com 28 agentes de seis unidades de saude, os quais foram audiogravados e transcritos. A analise qualitativa do material permite identificar nas praticas discursivas dos ACS uma tensao em torno des/valorizacao de seu trabalho. Sentidos de valorizacao surgem quando destacam sua atuacao proxima a comunidade e seu potencial para construcao de vinculos, mas sao tensionados por sentidos de desvalorizacao referidos a aspectos macroestruturais, como baixos salarios e pouco reconhecimento de sua funcao comparativamente a profissionais de nivel superior. Concluimos que a presenca de uma visao ainda pautada na fragmentacao dos processos de trabalho e em uma expectativa de resolutividade imediata das demandas da populacao atraves de acoes individuais dos profissionais de saude pode dificultar a apropriacao, pelos ACS, de uma concepcao mais abrangente de Atencao Primaria, como articuladora da Rede de Atencao a Saude e organizadora do Sistema Unico de Saude.

A (des)valorização do agente comunitário de saúde na Estratégia Saúde da Família

Family Health Strategy (FHS) is one of the foundations of Primary Health Care. FHS offers a decentralized assistance in its referred territories, and attends different types of demands, including issues of mental health. This research aimed to make meanings with FHS professionals about their resources for mental health care. Thus, groups of discussion were conducted with these professionals in a town located in the state of Sao Paulo. Qualitative procedures of thematic analysis were conducted with the material, based on social constructionist contributions. This analysis highlights important technical and relational resources of the team regarding their practices with mental health, even though these resources were apparently little legitimized in their daily work. The need to create contexts for reflection among healthcare professionals where they can make new meanings about their actions is discussed. This may contribute to enhance the quality of mental health assistance in Primary Health Care.

Functions of the community health workers in the work with social networks. �怀 is qualitative study aims to discuss how community health workers (CHW) use the social networks of the population they assist in order to develop actions of health promotion in the Family Health Program. Speci�耀 cally, it aims to present how CHWs make meaning about their practice with social networks, giving visibility to the functions they perform in their daily work. In order to do so, we conducted and recorded twelve group discussions with CHWs of all the family health units of a city in the state of Sao Paulo. �怀 e transcripts were analyzed via thematic analysis procedures, based on the theoretical and methodological perspective of social constructionism. Our results show the CHWs perform two main functions: articulator and mediator of social networks. From that, we discuss that the development of these functions is fundamental in a broader perspective of health, though it is still little valued when compared to the other activities performed by CHWs in the Family Health Program.

RESUMO A Politica Nacional de Humanizacao tem como objetivo promover os principios doutrinarios preconizados pelo Sistema Unico de Saude, sendo a cogestao uma de suas diretrizes. Este artigo tem como objetivo descrever e compreender como aspectos contextuais favorecem ou limitam a construcao da cogestao no âmbito da Atencao Basica. Trata-se de um estudo empirico e qualitativo desenvolvido em duas unidades de saude, sendo uma tradicional e uma com Estrategia Saude da Familia (ESF). A construcao do corpus se deu por meio de imersao no campo e de diario para anotacao de observacoes e reflexoes, analisados por agrupamento tematico. Como resultados, descrevemos os aspectos contextuais da ESF, i.e., horario de funcionamento, equipe multiprofissional e necessidade de reunioes, formacao do medico, agentes comunitarios, e organizacao do espaco fisico como facilitadores para o funcionamento em cogestao. Ja os elementos contextuais da unidade tradicional, foram a estrutura organizacional, modelo medico e ausencia de reunioes, horario de funcionamento, e organizacao do espaco fisico como dificultadores para essa construcao. Esperamos que esta analise promova iniciativas que considerem a importância dos aspectos estruturais, e nao so o protagonismo dos sujeitos, como imprescindiveis para o processo de mudanca.

Aspectos contextuais na construção da cogestão em Unidades Básicas de Saúde

The participation of the families in health care has been emphasized in the construction of policies in Brazil. In this study, we aim to understand meanings constructed with health professionals about families and the implications of these meanings for the production of health care practices in the Family Health Strategy. We conducted a qualitative study, whose corpus was composed by the transcription of 16 meetings of "Family discussion groups", helded as part of the routine of two Family Health Program´s teams. The analysis of the transcriptions was based on the theoretical perspective of the discursive practices and production of meanings in everyday life. As a result, four meanings were constructed: Family as people who live together; as responsible for the care; as a problem; and as a network of relationships. We conclude that there are efforts of professionals to consider the family in their practice, however, actions centered on the individual, and still not guided by the analysis of the context and living conditions, are more common in their discursive practices.
