Background: No reports have been published about age-related sperm malformations in varicocele patients. Aim: To investigate the distribution of abnormal sperm characteristics in adolescents, adults and older men with varicocele. Setting and Design: Records of semen analysis of 143 men aged 14 to 53 years who had evident left-sided varicocele detected by physical examination and confirmed by doppler sonography were selected. Materials and Methods: Sperm concentration, vitality, motility, morphology, hypoosmotic swelling test (HOST) and morphology were measured in adolescent males aged 14 to 20 years (n=31), men 21 to 30 years (n=48), 31 to 40 years (n=40) and older men over 40 (n=24) and compared with a control group of fertile men with no varicocele (n=27) and with a group of infertile men with varicocele (n=26). Statistical Analysis: One-way analysis of variance and the Kruskal-Wallis test were used to compare varicocele groups. Comparisons with the control group and infertile group were performed using the unpaired t-test and the Mann-Whitney test. The discriminating ability of significant sperm characteristics in evaluating the sperm quality of varicocele men was also analyzed using receiver operating characteristics curve to select the cut-off level providing the best combination of sensitivity and specificity. Results: Varicocele men displayed similar impairment of vitality, motility and HOST. Sperm morphology analysis revealed a prevalence of small head, slightly and severely amorphous head and particularly combined anomalies in the study groups. Sperm concentration fell within the normal range of the World Health Organization manual. Differences were not significant between the study groups and when compared with infertile group ( P >0.005). However, a comparative study of the varicocele groups and the infertile group with the control group revealed significant differences in sperm vitality, motility, HOST, morphologically normal sperm, pin-headed, tapered and combined anomalies. Morphologically normal sperm and combined anomalies showed higher accuracy in identifying poor sperm quality in varicocele men (83.7% and 77.9%, at cut-off levels of 9% and 38%, respectively). Conclusions: Varicocele harms equally the sperm characteristics of adolescents, adults and older men. Apparently, it affects sperm quality more adversely than it does sperm production.

Significance of sperm characteristics in the evaluation of adolescents, adults and older men with varicocele.

Im Rahmen eines Studienprojektes wurde vom August 1998 bis Mai 2003 eine prospektive Langsschnittuntersuchung zur Progression der Varikozele testis im Kindes- und Jugendalter durchgefuhrt. Ziel der Untersuchung war, die Entwicklung der Erkrankung im Hinblick auf den Schweregrad der Varikozele und das Wachstum des betroffenen Hodens darzustellen. Neben der klinischen Untersuchung wurde die CW-Dopplersonografie nach der Klassifikation von Hirsh als diagnostisches Instrument zur Beurteilung des Schweregrades eingesetzt. 164 Teilnehmer im Pubertatsstadium I und II nach Tanner mit strikt einseitiger Varikozele konnten uber einen Zeitraum von mindestens drei Jahren regelmasig nachuntersucht werden. 
Im Ergebnis fand sich nur in 11% der Teilnehmer eine vollstandige Ruckbildung der Varikozele. Teilnehmer mit persistierender Varikozele zeigten uberwiegend eine Progredienz der Auspragungsgrade. Der Schweregrad des venosen Refluxes wies keine nennenswerte Anderung auf. Bei ca. 12% der Teilnehmer entwickelte sich zusatzlich eine kontralaterale, meist jedoch nur gering ausgepragte Varikozele. Teilnehmer mit persistierender, unilateraler Varikozele wiesen am ipsilateralen Hoden eine Zunahme des prozentualen Volumendefizites auf. Ebenso fand sich tendenziell eine Zunahme der Rate von volumengeminderten Hoden. Der initiale Varikozelengrad liess keine Aussage uber die weitere Entwicklung des Hodens zu, allerdings bestand ein signifikanter Zusammenhang zwischen dem Varikozelengrad und der Rate sowie dem Ausmas der Hodenhypotrophie zum Zeitpunkt der Abschlussuntersuchung. 
Diese Ergebnisse zeigen, dass die Varikozele in Hinblick auf den zunehmenden Auspragungsgrad sowie das zunehmende Volumendefizit des varikozelentragenden Hodens ein progrediente Erkrankung darstellt. Es wurden Volumendefizite von 15% und mehr beobachtet, die sich in dieser Grosenordnung im Allgemeinen nicht zuruckbilden. Trotz der hohen Pravalenz der Varikozele im Kindesalter (18%)erscheint aufgrund der hier vorgestellten, inkoharenten Ergebnisse und der eingeschrankten Beurteilbarkeit des Hodenvolumens bei prapubertaren Jungen eine fruhe Genitaluntersuchung und Verlaufsbeobachtung nicht sinnvoll. Vielmehr scheint eine Untersuchung erst mit Beginn der Pubertat im Alter von 12-13 Jahren sinnvoll, da erst in dieser Entwicklungsphase ein signifikanter Zusammenhang zwischen dem Varikozelengrad und der Hodenentwicklung erkennbar wird.

Ist die prä- und frühpubertär auftretende Varikozele testis im weiteren Verlauf mit dem Risiko einer Wachstumsstörung des Hodens assoziiert?

The interest for varicocele in the determination of male infertility has increased during the last decades. Most researchers consider varicocele as the primary cause of male infertility, but recently another group of authors give it a secondary role in the alteration of spermatogenesis. We think that the major part of the controversy depends on an absence of a systematic approach to the problem. We give a primary importance to an accurate epidemiological evaluation which consists in a transversal and longitudinal survey of male subjects in puberal age. Our data show that left varicocele is practically inexsistent before the onset of puberty; the percentage incidence of this alteration increases progressively with puberal maturity and the tends to decrease slightly when maturity is complete. By correlating this pathology with puberty we can obtain more precise informations than when it is correlated to the regestrated age. The young patients who result suffering from varicocele, must be controlled carefully and periodically for the evaluation of the period and the opportunity of a therapeutic treatment.
