Abstract: An increasing evidence base suggests that low bone mineral density (BMD) and fractures are associated with cardiovascular disease (CVD). We conducted a systematic review and meta-analysis summarizing the evidence of low BMD and fractures as risk factors for future CVD. Two independent authors searched major databases from inception to 1st August 2016 for longitudinal studies reporting data on CVD incidence (overall and specific CVD) and BMD status and fractures. The association between low BMD, fractures and CVD across longitudinal studies was explored by calculating pooled adjusted hazard ratios (HRs)?±?95% confidence intervals (CIs) with a random-effects meta-analysis. Twenty-eight studies (18 regarding BMD and 10 fractures) followed-up a total of 1,107,885 participants for a median of 5 years. Taking those with higher BMD as the reference, people with low BMD were at increased risk of developing CVD during follow-up (11 studies; HR?=?1.33; 95%CI: 1.27-1.38; I2?=?53%), after adjusting for a median of 8 confounders. This finding was confirmed using a decrease in one standard deviation of baseline BMD (9 studies; HR?=?1.16; 95%CI: 1.09-1.24; I2?=?69%). The presence of fractures at baseline was associated with an increased risk of developing CVD (HR?=?1.20; 95%CI: 1.06-1.37; I2?=?91%). Regarding specific CVD, low BMD was associated with an increased risk of developing coronary artery disease, cerebrovascular conditions, and CVD associated death. Fractures at baseline was associated with an increased risk of cerebrovascular conditions and death due to CVD. In conclusion, low BMD and fractures are associated with a small, but significant increased risk of CVD risk and possibly death.

Abstract: OBJECTIVE: To investigate the risk factors for meniscal damage, an important determinant of knee osteoarthritis. METHODS: We studied 243 men and women aged 20-59 years in whom the diagnosis of a meniscal tear was confirmed for the first time at arthroscopy, over a 25 month period, in 2 British hospitals. Each case was compared with one or 2 community controls, matched by age and sex, who were registered with the same general practitioner. Information on exposure to risk factors was obtained by a structured questionnaire and physical examination. RESULTS: Meniscal tear was strongly associated with participation in sports during the 12 months preceding the onset of symptoms; the risk was particularly high for soccer (OR 3.7; 95% CI 2.1-6.6). Higher body mass index and occupational kneeling (OR 3.8; 95% CI 1.3-11.0) and squatting (OR 2.9; 95% CI 1.0-8.0) were associated with an increased risk of degenerative meniscal lesions, after adjustment for social class, joint laxity, and sports participation. Joint laxity was associated with degenerative meniscal lesions independently of occupational physical activity, sports, and obesity. CONCLUSION: Our results confirm the importance of sporting activities entailing knee torsion in acute meniscal tear. They also point to a role for occupational activity, adiposity, and joint laxity in the pathogenesis of degenerative meniscal lesions. Modifying these mechanical risk factors may serve to reduce the risk of meniscal injury and may also help to prevent later knee osteoarthritis.

Abstract: Patients with multiple sclerosis (MS) may be at an increased risk of fracture owing to a greater risk of falling and decreased bone mineral density when compared with the general population. This study was designed to estimate the relative and absolute risk of fracture in patients with MS. We conducted a population-based cohort study using data from the UK General Practice Research Database linked to the National Hospital Registry (1997–2008). Incident MS patients (n = 5565) were matched 1:6 by year of birth, sex, and practice with patients without MS (controls). Cox proportional-hazards models were used to derive adjusted hazard ratios (HRs) for fracture associated with MS. Time-dependent adjustments were made for age, comorbidity, and drug use. Absolute 5- and 10-year risks of fracture were estimated for MS patients as a function of age. Compared with controls, MS patients had an almost threefold increased risk of hip fracture [HR = 2.79, 95% confidence interval (CI) 1.83–4.26] and a risk of osteoporotic fracture that was increased 1.4-fold (HR = 1.35, 95% CI 1.13–1.62). Risk was greater in patients who had been prescribed oral/intravenous glucocorticoids (GCs; HR = 1.85, 95% CI 1.14–2.98) or antidepressants (HR = 1.79, 95% CI 1.37–2.35) in the previous 6 months. Absolute fracture risks were low in younger MS patients but became substantial when patients were older than 60 years of age. It is concluded that MS is associated with an increased risk of fracture. Fracture risk assessment may be indicated in patients with MS, especially those prescribed GCs or antidepressants. © 2011 American Society for Bone and Mineral Research
