Studies on the local treatment of wounds for the prevention of rabies.

Abstract: This paper reports on laboratory studies made in guinea-pigs on puncture wounds infected with fixed rabies virus and treated one hour later with various substances, the purpose being to review the experience of previous workers and to explore new approaches to the problem of local treatment of wounds inflicted by rabid animals.AMONG THE MEASURES AFFORDING GREATER OR LESSER PROTECTION WERE: nitric acid cauterization; direct application of benzalkonium chloride to the wound or its infiltration, as well as that of methylbenzethonium chloride, proximal to the wound; repeated swabbing and flushing with 20% soap solution or benzalkonium chloride; local inoculation of procaine anaesthetics; infiltration of the leg wound, or inoculation of the opposite leg, with antirabies gamma-globulin; and infiltration of the wound with interferon prepared in guinea-pig tissue cultures.Protection was not afforded by flushing with 20% soap solution alone; topical application of aqueous or tincture preparations of iodine or thiomersal; inoculation of phenoxybenzamine, physostigmine or diphenhydramine hydrochloride; or infiltration of the wound with interferon prepared on monkey kidney tissue cultures.The authors conclude that, at the present time, very vigorous cleansing of the wound with 20% soap solution or 2% benzalkonium chloride, local infiltration of the wound with antirabies serum and-to minimize pain-the use of procaine in saline are indicated in the local treatment of wounds for the prevention of rabies.

Psychiatric and Cognitive Phenotype of Childhood Myotonic Dystrophy Type 1.

Abstract: Aim To investigate the psychiatric and cognitive phenotype in young individuals with the childhood form of myotonic dystrophy type 1 (DM1). Method Twenty-eight individuals (15 females, 13 males) with childhood DM1 (mean age 17y, SD 4.6, range 7–24y) were assessed using standardized instruments and cognitive testing of general intelligence, visual attention, and visual–spatial construction abilities. Results Nineteen patients had repeated a school grade. The mean (SD) Full-scale IQ was 73.6 (17.5) and mean Verbal IQ was significantly higher than the mean Performance IQ: 80.2 (19.22) versus 72.95 (15.58), p=0.01. Fifteen patients had one or more diagnoses on the DSM-IV axis 1, including internalizing disorders (phobia, n=7; mood disorder, n=6; other anxiety disorders, n=5) and attention-deficit–hyperactivity disorder, inattentive subtype (n=8). Twelve out of 22 patients had alexithymia (inability to express feelings with words and to recognize and share emotional states). Cognitive testing found severe impairments in visual attention and visual–spatial construction abilities in four out of 18, and 14 out of 24 patients respectively. No diagnosis was correlated with the transmitting parent’s sex or with cytosine–thymine–guanine (CTG) repeat numbers. Patients with severe visual–spatial construction disabilities had a significantly longer CTG expansion size than those with normal visual–spatial abilities (p=0.04). Interpretation Children and adolescents with childhood DM1 have frequent diagnoses on DSM-IV axis 1, with internalizing disorders being the most common type of disorder. They also have borderline low intelligence and frequent impairments in attention and visual–spatial construction abilities.

Abstract: It is a commonplace of contemporary classical scholarship that in the classical period the political and social status of Athenian women was deplorably low. Relegated to the ranks of slaves and children, scholars suggest that they were even much worse off than the women of earlier and later periods of Greek history. This paper proposes not to challenge this global judgement as a whole, but simply to focus upon one aspect of women's lives which has played an important role in such scholarly discussions.

The Dilemmas of Educational Reform

Abstract: The environment of U.S. schools has changed dramatically over a quarter century as standards tied to test-based accountability and market competition became commonplace. We examine the issues that school systems face in this changing environment, to identify considerations for researchers interested in reform as educational system building. We highlight a central dilemma: Systems manage environmental pressures to become more coherent enterprises that focus on tested outcomes while managing the inherited differentiated organizations and environmental pressures which support these enterprises. We identify four activity domains that are defined by these competing pressure: consensus on outcomes; infrastructure to connect outcomes with instruction; recruitment that is aligned with outcomes; and competing environmental pressures.
