Abstract: Este estudo focaliza questoes da gerencia nos servicos publicos de saude e tem como objetivo identificar elementos do trabalho gerencial do enfermeiro na Rede Basica de Saude de Goiânia-GO. Pesquisa exploratoria descritiva, de base quantitativa, realizada entre 2006/2007. Para a coleta de dados foi utilizada a entrevista estruturada com 44 enfermeiros gerentes de servicos. Foi construido um banco de dados, utilizando o Software Epi-Info versao 3.3.2. Os dados foram submetidos a um processo de codificacao e analisados por medida de frequencia simples e nas variaveis que o enfermeiro pode discorrer utilizamos a categorizacao por semelhanca. Os resultados apontam aspectos que fortalecem e tornam mais visiveis as acoes gerenciais dos enfermeiros, ao mesmo tempo que mostram as fragilidades do trabalho gerencial na Rede Basica de Saude. A contribuicao dos enfermeiros gerentes foi de grande significado para que a formacao seja repensada pelas universidades e pelos gestores.

Abstract: Estudio cualitativo realizado en un hospital universitario con 15 profesionales de enfermeria. Objetivo analizar las razones, actitudes y creencias de los trabajadores de enfermeria referentes a la adhesion a los equipamientos de proteccion individual. Los datos fueron recolectados por medio de grupo focal, analizados por el metodo de interpretacion de sentidos, considerando el referencial del modelo de creencias sobre salud de Rosenstock. De los datos surgieron dos categorias tematicas, Seguridad en el trabajo y Relaciones Interpersonales. Identificamos varias barreras que interfieren en las cuestiones de seguridad y proteccion individual como comunicacion, sobrecarga de trabajo, estructura fisica, accesibilidad a los equipamientos de proteccion y aspectos organizacionales y administrativos. La adhesion a los equipamientos de proteccion es determinada tanto por el contexto experimentado en el ambiente de trabajo, como por valores y creencias individuales; sin embargo, la decision del uso de los equipamientos de proteccion es individual.

Abstract: Ao retornar a comunidade, no periodo pos-parto, a mulher necessita de atendimento de enfermagem sistematizado. Objetivou-se nesta pesquisa analisar a ocorrencia de 22 diagnosticos de enfermagem de interesse no puerperio imediato e tardio. Trata-se de estudo descritivo, do qual participaram 40 puerperas. Realizou-se a coleta de dados no periodo de fevereiro a maio de 2008, no domicilio das participantes, mediante um encontro com entrevista, exame fisico e observacao. O julgamento clinico dos diagnosticos foi baseado na Taxonomia II da NANDA. Entre os diagnosticos identificados, destacaram-se: conhecimento deficiente; risco para infeccao; integridade tissular prejudicada; amamentacao eficaz; ansiedade; nutricao desequilibrada, menos do que as necessidades corporais; disposicao para processos familiares melhorados; risco de integridade da pele prejudicada; e insonia. Foram encontrados indicios de novos diagnosticos: risco de amamentacao interrompida, risco de amamentacao ineficaz. Os resultados indicam areas para o direcionamento das acoes de enfermagem no pre-natal e puerperio e a necessidade de acompanhamento mais proximo da puerpera do que o previsto nas politicas de saude.

Abstract: Promotoras have worked in various areas of public health; however, there is a lack of understanding about their work in mental health. This article reviews the literature in 3 different cultures/languages (English, Spanish, and Portuguese) and aims to describe promotoras' roles, training, and interventions and their outcomes related to mental health activities. Results demonstrate that in different cultures/languages, promotoras empower community members to promote mental health and prevent exacerbation of individuals' mental illness. Promotoras, when trained carefully, have the ability to increase awareness and to promote mental health in populations that would otherwise have limited or no access to care.

Abstract: La toma de decisiones en la practica diaria del enfermero necesita ser basada en conocimiento cientifico. La practica basada en evidencias es una aproximacion que establece la utilizacion de resultados de investigaciones en la practica clinica, con la revision integradora como uno de sus recursos. La finalidad de esta revision integradora es buscar y evaluar las evidencias disponibles sobre las intervenciones eficaces de enfermeria para la prevencion de lesiones de piel en el paciente quirurgico, en el periodo transoperatorio, o como resultado del mismo. Utilizamos dos bases de datos, Cinahl y Medline, para la seleccion de los articulos, resultando en una muestra de 14 articulos. Tras el analisis de los articulos incluidos en la revision, los resultados de los estudios indicaron que los dispositivos considerados los mas eficaces en la prevencion de lesiones de piel fueron, sucesivamente, el sistema de colchon dinamico pulsante multi-celula, la almohada de polimero seco visco-elastico y almohadas con gel.

Abstract: To synthesize current understanding of how community-based health worker (CHW) programs can best be designed and operated in health systems. We searched 11 databases for review articles published between 1 January 2005 and 15 June 2017. Review articles on CHWs, defined as non-professional paid or volunteer health workers based in communities, with less than 2 years of training, were included. We assessed the methodological quality of the reviews according to AMSTAR criteria, and we report our findings based on PRISMA standards. We identified 122 reviews (75 systematic reviews, of which 34 are meta-analyses, and 47 non-systematic reviews). Eighty-three of the included reviews were from low- and middle-income countries, 29 were from high-income countries, and 10 were global. CHW programs included in these reviews are diverse in interventions provided, selection and training of CHWs, supervision, remuneration, and integration into the health system. Features that enable positive CHW program outcomes include community embeddedness (whereby community members have a sense of ownership of the program and positive relationships with the CHW), supportive supervision, continuous education, and adequate logistical support and supplies. Effective integration of CHW programs into health systems can bolster program sustainability and credibility, clarify CHW roles, and foster collaboration between CHWs and higher-level health system actors. We found gaps in the review evidence, including on the rights and needs of CHWs, on effective approaches to training and supervision, on CHWs as community change agents, and on the influence of health system decentralization, social accountability, and governance. Evidence concerning CHW program effectiveness can help policymakers identify a range of options to consider. However, this evidence needs to be contextualized and adapted in different contexts to inform policy and practice. Advancing the evidence base with context-specific elements will be vital to helping these programs achieve their full potential.

Abstract: This systematic review evaluates efforts to date to involve community health workers (CHWs) in delivering evidence-based mental health interventions to underserved communities in the United States and in low- and middle-income countries. Forty-three articles (39 trials) were reviewed to characterize the background characteristics of CHW, their role in intervention delivery, the types of interventions they delivered, and the implementation supports they received. The majority of trials found that CHW-delivered interventions led to symptom reduction. Training CHWs to support the delivery of evidence-based practices may help to address mental health disparities. Areas for future research as well as clinical and policy implications are discussed.

Abstract: The work of Kurt Lewin dominated the theory and practice of change management for over 40 years. However, in the past 20 years, Lewin's approach to change, particularly the 3-Step model, has attracted major criticisms. The key ones are that his work: assumed organizations operate in a stable state; was only suitable for small-scale change projects; ignored organizational power and politics; and was top-down and management-driven. This article seeks to re-appraise Lewin's work and challenge the validity of these views. It begins by describing Lewin's background and beliefs, especially his commitment to resolving social conflict. The article then moves on to examine the main elements of his Planned approach to change: Field Theory; Group Dynamics; Action Research; and the 3-Step model. This is followed by a brief summary of the major developments in the field of organizational change since Lewin's death which, in turn, leads to an examination of the main criticisms levelled at Lewin's work. The article concludes by arguing that rather than being outdated or redundant, Lewin's approach is still relevant to the modern world.
