Abstract: Recientemente han aparecido las resinas compuestas bulk-fill (RBFs), las cuales, de acuerdo a los fabricantes, se podrian utilizar para restaurar cavidades en incrementos de 4-5 mm. Esto significaria probablemente una simplificacion importante de la tecnica, con respecto a la tecnica incremental. Sin embargo, las propiedades y modos de empleo de las RBFs varian significativamente entre ellas. Ademas, debido a su corta data de aparicion en el mercado, todavia existe una escasez de estudios clinicos que avalen el desempeno clinico de estos materiales. Este articulo tiene como objetivo realizar una revision del estado del arte de la literatura disponible en la base de datos Medline/Pubmed y ScIELO, publicados entre los anos 2000 al 2014, detallandose la composicion, clasificacion y los resultados de las investigaciones publicadas actualmente. Abstract: Aim: To compare the number of colony forming units (CFU) and agar adherence of S. mutans on amalgam (AM) and resin composite (RC) occlusal restorations. Sixty-five healthy patients older than 18 years with high caries risk who had at least one occlusal AM and RC restorations (n=130) were selected. Methods: The restorations were evaluated according to the alpha Ryge criteria (Cohen-Kappa 0.8). For each patient, a biofilm sample was taken using an impression tray technique with previously loaded with solid trypticase yeast extract cysteine sucrose with bacitracin agar placed over the AM restorations and RC restorations in the same patients. The tray was placed inside an oven at 37 °C for 48 h, and the S. mutans count was then performed. Data were analyzed with the test Wilcoxon with a 95% confidence level. Results: RC restorations had statistically significant higher number of CFU of S. mutans than AM restorations (p<0.05). Conclusions: In adult patients with high caries risk, RC occlusal surfaces showed greater agar adherence of S. mutans count than AM restorations.

Fracture of the ceramic eneers as a result of oral function or trauma is not an uncommon problem in clinical practice. Although fractures of such restorations do not necessarily mean the failure of the restoration, the renewal process is both costly and time consuming and therefore remains a clinical problem. Fractures in the anterior region pose an aesthetic problem but when they are in the posterior, chewing function could also be affected. The published literature reveals that reasons for failures cover a wide spectrum from iatrogenic factors to laboratory mistakes or because of factors related to the inherent structure of the ceramics or simply to trauma.

Moreover, they pose an aesthetic and functional dilemma both for the patient and the dentist. This problem has created demand for the development of practical repair options which do not necessitate the removal and remake of the entire restoration. Published literatureon repair techniques for fractured fixed partial dentures, concentrating on the data obtained both from in vitro and in vivo studies, reveals that the repair techniques based on sandblasting and silanization are the most durable in terms of adhesive and cohesive failures compared with those using different etching agents.

Abstract: The objective of this systematic review and meta-analysis was to determine the clinical effectiveness of bulk-fill and conventional resin in composite restorations. A bibliographic search was carried out until May 2020, in the biomedical databases Pubmed/MEDLINE, EMBASE, Scopus, CENTRAL and Web of Science. The study selection criteria were: randomized clinical trials, in English, with no time limit, with a follow-up greater than or equal to 6 months and that reported the clinical effects (absence of fractures, absence of discoloration or marginal staining, adequate adaptation marginal, absence of post-operative sensitivity, absence of secondary caries, adequate color stability and translucency, proper surface texture, proper anatomical form, adequate tooth integrity without wear, adequate restoration integrity, proper occlusion, absence of inflammation and adequate point of contact) of restorations made with conventional and bulk resins. The risk of bias of the study was analyzed using the Cochrane Manual of Systematic Reviews of Interventions. Sixteen articles were eligible and included in the study. The results indicated that there is no difference between restorations with conventional and bulk resins for the type of restoration, type of tooth restored and restoration technique used. However, further properly designed clinical studies are required in order to reach a better conclusion.

Abstract: Purpose The aim of this study was to determine whether there are differences in the distribution of various indicators of oral health among elderly people with and without multimorbidity (ie, two or more chronic diseases). Subjects and methods A cross-sectional, comparative study was conducted using a sample of Mexican elderly individuals aged ≥60 years. The average age of the cohort was 79.06±9.78 years, and 69.1% were women. The variables indicating oral health were as follows: functional dentition, edentulism, hyposalivation, xerostomia, root caries and periodontitis. The multimorbidity variable was operationally categorized as follows: 0= subjects with no chronic disease or one chronic disease and 1= subjects with two or more chronic diseases. Questionnaires were used to collect information on various variables regarding general health. Likewise, the participants underwent a clinical oral examination. The analysis was performed using Stata 11.0. Results The overall prevalence of multimorbidity was 27.3%. The prevalences of various oral health indicators were as follows: without functional dentition 89.9%; hyposalivation 59.7%; edentulism 38.9% and self-reported xerostomia 25.2%. Dental caries were observed in 95.3% of the subjects, and the prevalence of severe periodontitis was 80%. We found a significant difference only in edentulism; its prevalence was higher among subjects with multimorbidity (55.3% vs 32.7%, P=0.015) than among those without multimorbidity. Conclusion The presence of edentulism in this sample of Mexican older adults was higher in subjects with multimorbidity. Multimorbidity and oral diseases constitute a true challenge in elderly people, because they affect quality of life and are associated with high health care costs.
