This paper analyses some aspects of the trajectory of the Argentinian physician and sociologist Juan Cesar Garcia (1932-1984) in the field of Latin American Social Medicine. Three dimensions constituting his basic orientations are highlighted: the elaboration of systematic and reflective social thought; a critical attitude in questioning teaching and professional practices; a commitment to the institutionalization and dissemination of health knowledge.

Resumo A escala geografica tem sido amplamente discutida na tentativa de formar um conceito que de conta de explicar as diferentes realidades do espaco. Nos diversos seguimentos de planejamento em saude, faz-se uma adocao das categorias e conceitos geograficos sem entender os problemas inerentes a escala geografica, o que afeta diretamente nas politicas publicas implantadas e, consequentemente, na vida das pessoas. O objetivo deste artigo foi discutir o conceito de escala a partir dos seus diferentes niveis, e debater a dificuldade de articula-los por meio da atuacao dos orgaos de vigilância em saude publica brasileira que lidam com a leishmaniose visceral. Realizamos uma revisao de literatura para demonstrar como o debate do conceito de escala geografica com a saude e ainda incipiente. Utilizamos conceitos geograficos, integrando as politicas municipais, estaduais e nacional sob a otica da escala geografica. Dessa forma, acreditamos que seja possivel uma articulacao dos orgaos dos sistemas de saude com a sociedade na transposicao de escalas, num acontecer solidario, produzindo um sistema de saude mais eficaz. A discussao nao sera aqui esgotada, mas permitira uma reflexao nas opcoes teorico-metodologicas em pesquisas e em saude publica.

O uso da escala geográfica na saúde pública: as escalas da leishmaniose visceral.

A history of historiography about the birth of public health, already known in Brazil, is redone to point out reductionisms. It seeks to clarify Rosen's position by explaining the leap in his approach concerning that of Sigerist. Foucault's genealogical reception of Rosen is presented to reveal a transformation of public law. It shows how, boosting Rosen's history of medicine and the Frankfurt School's history of punishment, Foucault's genealogy exposes the death power of the State’s reason covered by the invention of the politics of life. The aim is to reactivate the understanding of such genealogy, a decisive factor in the Brazilian birth of collective health, and to undo the coverings that erase the revolutionary instigation.

Do nascimento da saúde pública à saúde coletiva: reativar a contra-história

The scope of this paper is to reflect onthe theoretical construction in the constitutionof the sociology of health, still called medical soci-ology in some countries. Two main ideas consti-tute the basis for this: interdisciplinarity and thedegree of articulation in the fields of medicineand sociology. We sought to establish a dialoguewith some dimensions – macro/micro, structure/action – that constitute the basis for understand-ing medicine/health in relation to the social/so-ciological dimension. The main aspects of thesedimensions are initially presented. Straus’ twomedical sociologies and the theory/applicationimpasses are then addressed, as well as the dilem-mas of the sociology of medicine in the 1960s and1970s. From these analyses the theoretical pro-duction before 1970 is placed as a counterpoint.Lastly, the sociology of health is seen in the gener-al context of sociology, which underwent a frag-mentation process from 1970 with effects in allsubfields of the social sciences. This process in-volves a rethinking of the theoretical issues in abroadened spectrum of possibilities. The 1980s arehighlighted when theoretical issues in the sociol-ogy of health are reinvigorated and the issue ofinterdisciplinarity is once again addressed.
