In 1985, the National Institute for Occupational Safety and Health (NIOSH) convened an ad hoc committee of experts who reviewed the current literature on lifting, recommend criteria for defining lifting capacity, and in 1991 developed a revised lifting equation. Subsequently, NIOSH developed the documentation for the equation and played a prominent role in recommending methods for interpreting the results of the equation. The 1991 equation reflects new findings and provides methods for evaluating asymmetrical lifting tasks, lifts of objects with less than optimal hand-container couplings, and also provides guidelines for a larger range of work durations and lifting frequencies than the 1981 equation. This paper provides the basis for selecting the three criteria (biomechanical, physiological, and psychophysical) that were used to define the 1991 equation, and describes the derivation of the individual components (Putz-Anderson and Waters 1991). The paper also describes the lifting index (LI), an index of relative physical stress, that can be used to identify hazardous lifting tasks. Although the 1991 equation has not been fully validated, the recommended weight limits derived from the revised equation are consistent with or lower than those generally reported in the literature. NIOSH believes that the revised 1991 lifting equation is more likely than the 1981 equation to protect most workers.

Revised NIOSH equation for the design and evaluation of manual lifting tasks

Four new manual handling experiments are reviewed. The experiment used male and female subjects to study lifting, lowering, pushing, pulling, and carrying tasks. Each experiment used a psychophysical methodology with measurements of oxygen consumption, heart rate, and anthropometric characteristics. Independent variables included task frequency, distance, height and duration; object size and handles; extended horizontal reach; and combination tasks. The results of the four experiments were integrated with the results of seven similar experiments published previously by this laboratory. The integrated data were used to revise maximum acceptable weights and forces originally published in 1978. The revised tables are presented and compared with the original tables.

The content of this manuscript is intended to assist the reader in collecting valid and reliable data for quantifying muscular strength and power. Various drawbacks and pitfalls of specific tests, as well as recommendations for the practitioner are also provided. The content is divided into sections covering isometric, isotonic, field tests, and isokinetic modes of exercise. Inherent in these modes are both concentric and eccentric muscle actions as well as both open and closed kinetic chain activities. For Isometric testing, contractions should occur over a four to five seconds duration with a one second transition period at the start of the contraction. At least one minute of rest should be provided between contractions. For each muscle tested at each position, at least three contractions should be performed although more may be performed if deemed necessary by the tester. For isotonic testing, the 1-RM test should be performed. After the general warm-up, the subject should perform a specific warm-up set of 8 repetitions at approximately 50% of the estimated 1-RM followed by another set of 3 repetitions at 70% of the estimated 1RM. Subsequent lifts are single repetitions of progressively heavier weights until failure. Repeat until the 1-RM is determined to the desired level of precision. The rest interval between sets should be not less than one and not more than five minutes. The optimal number of single repetitions ranges from three to five. Data and guidelines of the following field tests are also provided ; vertical jump, bench press, Wingate anaerobic cycle test (WAT), and the Margaria stair-run test. For isokinetic testing, details are provided for testing peak torque, work, power, endurance, and estimation of fiber type percentages.

Low Back Disorders : Evidence-Based Prevention and Rehabilitation

A continuing challenge for ergonomists has been to determine quantitatively the types of trunk motion and how much trunk motion contributes to the risk of occupationally-related low back disorder (LBD). It has been difficult to include this motion information in workplace assessments since the speed at which trunk motion becomes dangerous has not been determined. An in vivo study was performed to assess the contribution of three-dimensional dynamic trunk motions to the risk of LBD during occupational lifting in industry. Over 400 industrial lifting jobs were studied in 48 varied industries. The medical records in these industries were examined so that specific jobs historically categorized as either low, medium, or high risk for occupationally-related LBD could be identified. A tri-axial electrogoniometer was worn by workers and documented the three-dimensional angular position, velocity, and acceleration characteristics of the lumbar spine while workers worked at these low, medium, or high risk jobs. Workplace and individual characteristics were also documented for each of the repetitive lifting tasks. A multiple logistic regression model indicated that a combination of five trunk motion and workplace factors predicted well both medium risk and high risk occupational-related LBD. These factors included lifting frequency, load moment, trunk lateral velocity, trunk twisting velocity, and trunk sagittal angle. Increases in the magnitude of these factors significantly increased the risk of LBD. The analyses have enabled us to determine the LBD risk associated with combined changes in the magnitudes of the five factors. The results indicate that by suitably varying these five factors observed during the lift collectively, the odds of high risk group membership may decrease by over ten times. These results were related to the biomechanical, ergonomic, and epidemiologic literature. The five trunk motion and workplace factors could be used as quantitative, objective measures to redesign the workplace so that the risk of occupationally-related LBD is minimized.

A new approach for estimating metabolic rates for manual materials handling jobs is presented. This approach was applied to 48 different jobs. The model validation showed a correlation coefficient of 0.95 between the measured and predicted metabolic rates. The coefficient of variation (standard error/sample mean) was 10.2 percent.

Several physical stressors, including repetitive, sustained, and forceful exertions, awkward postures, localized mechanical stress, highly dynamic movements, exposures to low temperatures, and vibration have been linked to increased risk of work-related musculoskeletal disorders. Repetitive exertions have been among the most widely studied of these stressors, but there is no single metric for assessing exposure to repetitive work. A new methodology enables repetitive hand activity to be rated based on observable characteristics of manual work. This method uses a series of 10-cm visual-analog scales with verbal anchors and benchmark examples. Ratings for repetition reflect both the dynamic aspect of hand movements and the amount of recovery or idle hand time. Trained job analysis experts rate the jobs individually and then agree on ratings. Development and Evaluation of an Observational Method for Assessing Repetition in Hand Tasks

Preemployment strength testing: an updated position.

This investigation was performed to determine if isometric strength tests can be used to select workers for strenuous jobs and to reduce occupational injuries which are caused by a mismatch between worker strength and job strength requirements. Twenty jobs in a tire and rubber plant were studied biomechanically to identify critical strength-demanding tasks. Four strength tests were designed to simulate these tasks, and performance criteria were established for passing the tests. New applicants were administered the tests during their preplacement examinations to determine if they possessed sufficient strength to qualify for the jobs. The medical incidence rate of employees who were selected using the strength tests was approximately one-third that of employees selected using traditional medical criteria. It was concluded that isometric strength tests can be used to reduce occupational injuries and should be considered for implementation in industries with strenuous jobs.

This paper summarizes a three year epidemiological study conducted in five large industrial plants in order to evaluate the validity of two alternative modeling approaches to overexertion injury prediction. Detailed biomechanical and psychophysical job evaluations were performed for 55 industrial jobs comprised of 2934 potentially stressful manual materials handling tasks. The medical experiences of 6912 incumbent workers were monitored retrospectively for two years and prospectively for one year to establish a data base for comparison of the different models. The results show that each of the models can be used to predict both the incidence and severity of certain overexertion types of injuries such as contact, musculoskeletal and back injuries. The application of these models to identify or design administrative and engineering controls, however, may be limited as a result of the inherent correlation between the available indices and multifaceted jobs.
