Abstract: Current standards involving technical specification of hearing aids provide limited possibilities for assessing the influence of the spectral and temporal characteristics of the input signal, and these characteristics have a significant effect on the output signal of many recent types of hearing aids. This is particularly true of digital hearing instruments, which typically include non-linear amplification in multiple channels. Furthermore, these instruments often incorporate additional non-linear functions such as "noise reduction" and "feedback cancellation". The output signal produced by a non-linear hearing instrument relates to the characteristics of the input signal in a complex manner. Therefore, the choice of input signal significantly influences the outcome of any acoustic or psychophysical assessment of a non-linear hearing instrument. For this reason, the International Collegium for Rehabilitative Audiology (ICRA) has introduced a collection of noise signals that can be used for hearing aid testing (including real-ear measurements) and psychophysical evaluation. This paper describes the design criteria, the realisation process, and the final selection of nine test signals on a CD. Also, the spectral and temporal characteristics of these signals are documented. The ICRA noises provide a well-specified set of speech-like noises with spectra shaped according to gender and vocal effort, and with different amounts of speech modulation simulating one or more speakers. These noises can be applied as well-specified background noise in psychophysical experiments. They can also serve as test signals for the evaluation of digital hearing aids with noise reduction. It is demonstrated that the ICRA noises show the effectiveness of the noise reduction schemes. Based on these initial measurements, some initial steps are proposed to develop a standard method of technical specification of noise reduction based on the modulation characteristics. For this purpose, the sensitivity of different noise reduction schemes is compared by measurements with ICRA noises with a varying ratio between unmodulated and modulated test signals: a modulated-unmodulated ratio. It can be anticipated that this information is important to understand the differences between the different implementations of noise reduction schemes in different hearing aid models and makes.

Abstract: OBJECTIVE. The goal was to assess risky and protective listening behaviors of adolescent users of MP3 players and the association of these behaviors with demographic characteristics and frequency of use. METHODS. In 2007, 1687 adolescents (12-19 years of age) in 68 classes in 15 Dutch secondary schools were invited to complete questionnaires about their music-listening behaviors. RESULTS. Ninety percent of participants reported listening to music through earphones on MP3 players; 32.8% were frequent users, 48.0% used high volume settings, and only 6.8% always or nearly always used a noise-limiter. Frequent users were >4 times more likely to listen to high-volume music than were infrequent users, and adolescents in practical prevocational schools were more than twice as likely to listen to high-volume music as were those attending preuniversity education. CONCLUSIONS. When using MP3 players, adolescents are very likely to engage in risky listening behaviors and are unlikely to seek protection. Frequent MP3 player use is an indicator of other risky listening behaviors, such as listening at high volumes and failing to use noise-limiters. Abstract: Objectives. We estimated whether and to what extent a group of adolescents were at risk of developing permanent hearing loss as a result of voluntary exposure to high-volume music, and we assessed whether such exposure was associated with hearing-related symptoms. Methods. In 2007, 1512 adolescents (aged 12-19 years) in Dutch secondary schools completed questionnaires about their music-listening behavior and whether they experienced hearing-related symptoms after listening to highvolume music. We used their self-reported data in conjunction with published average sound levels of music players, discotheques, and pop concerts to estimate their noise exposure, and we compared that exposure to our own "loosened" (i.e., less strict) version of current European safety standards for occupational noise exposure. Results. About half of the adolescents exceeded safety standards for occupational noise exposure. About one third of the respondents exceeded safety standards solely as a result of listening to MP3 players. Hearing symptoms that occurred after using an MP3 player or going to a discotheque were associated with exposure to high-volume music. Conclusions. Adolescents often exceeded current occupational safety standards for noise exposure, hiqhliqhtinq the need for specific safety standards for leisure-time noise exposure.

Abstract: The Generation R Study is a population-based prospective cohort study from fetal life until young adulthood. The study is designed to identify early environmental and genetic causes of normal and abnormal growth, development and health during fetal life, childhood and adulthood. The study focuses on four primary areas of research: (1) growth and physical development; (2) behavioural and cognitive development; (3) diseases in childhood; and (4) health and healthcare for pregnant women and children. In total, 9,778 mothers with a delivery date from April 2002 until January 2006 were enrolled in the study. General follow-up rates until the age of 4 years exceed 75%. Data collection in mothers, fathers and preschool children included questionnaires, detailed physical and ultrasound examinations, behavioural observations, and biological samples. A genome wide association screen is available in the participating children. Regular detailed hands on assessment are performed from the age of 5 years onwards. Eventually, results forthcoming from the Generation R Study have to contribute to the development of strategies for optimizing health and healthcare for pregnant women and children.

Abstract: This paper summarizes twenty studies, published since 1989, that have measured experimentally the relationship between speech recognition in noise and some aspect of cognition, using statistical techniques such as correlation or factor analysis. The results demonstrate that there is a link, but it is secondary to the predictive effects of hearing loss, and it is somewhat mixed across study. No one cognitive test always gave a significant result, but measures of working memory (especially reading span) were mostly effective, whereas measures of general ability, such as IQ, were mostly ineffective. Some of the studies included aided listening, and two reported the benefits from aided listening: again mixed results were found, and in some circumstances cognition was a useful predictor of hearing-aid benefit.

Abstract: Intelligibility listening tests are necessary during development and evaluation of speech processing algorithms, despite the fact that they are expensive and time consuming. In this paper, we propose a monaural intelligibility prediction algorithm, which has the potential of replacing some of these listening tests. The proposed algorithm shows similarities to the short-time objective intelligibility STOI algorithm, but works for a larger range of input signals. In contrast to STOI, extended STOI ESTOI does not assume mutual independence between frequency bands. ESTOI also incorporates spectral correlation by comparing complete 400ms length spectrograms of the noisy/processed speech and the clean speech signals. As a consequence, ESTOI is also able to accurately predict the intelligibility of speech contaminated by temporally highly modulated noise sources in addition to noisy signals processed with time-frequency weighting. We show that ESTOI can be interpreted in terms of an orthogonal decomposition of short-time spectrograms into intelligibility subspaces, i.e., a ranking of spectrogram features according to their importance to intelligibility. Abstract: Background: Among injury victims relatively high prevalence rates of posttraumatic stress disorder (PTSD) have been found. PTSD is associated with functional impairments and decreased health-related quality of life (HRQoL). Previous studies that addressed the latter were restricted to injuries at the higher end of the severity spectrum. This study examined the association between PTSD symptoms and health-related quality of life (HRQoL) in a comprehensive population of injury patients of all severity levels and external causes. Methods: We conducted a self-assessment survey which included items regarding demographics of the patient, accident type, sustained injuries, EuroQol health classification system (EQ-5D) and Health Utilities Index (HUI) to measure functional outcome and HRQoL, and the Impact of Event Scale (IES) to measure PTSD symptoms. An IES-score of 35 or higher was used as indication for the presence of PTSD. The survey was completed by 1,781 injury patients two years after they were treated at the Emergency Department (ED), followed by either hospital admission or direct discharge to the home environment. Results: Symptoms indicative of PTSD were associated with more problems on all EQ-5D and HUI3 domains of functional outcome and a considerable utility loss in both hospitalized (0.23-0.24) and non-hospitalized (0.32-0.33) patients. Differences in reported problems between patients with IES scores higher or lower than 35 were largest for EQ-5D health domains pain/discomfort (82% versus 28%) and anxiety/depression (53% versus 11%) and HUI domains emotion (92% versus 33%) and pain (84% versus 38%). After adjusting for potential confounders, PTSD remained strongly associated with adverse HRQoL. Conclusions: Among patients treated at an ED posttraumatic stress symptoms indicative of PTSD were associated with a considerable decrease in HRQoL in both hospitalized and non-hospitalized patients. PTSD symptoms may therefore raise a major barrier for full recovery of injury patients of even minor levels of severity.

Abstract: Objectives: This descriptive summary of the literature provides an overview of the available studies (published between January 1980 and January 2009) on correlates of help-seeking behavior for hearing loss, hearing-aid uptake, hearing-aid use, and satisfaction with the device. Methods: Publications were identified by structured searches in Pubmed and Cinahl and by inspecting the reference lists of relevant articles. The articles covered different stages that a person with hearing impairment may go through: prior to hearing aid fitting, the period covering the fitting and the period post hearing aid fitting. Inclusion of articles occurred according to strict inclusion and exclusion criteria. Data were extracted by two independent researchers. Thirty-nine papers were included that identified 31 factors examined in relation to the four outcome measures. These covered personal factors (e.g., source of motivation, expectation, attitude), demographic factors (e.g., age, gender) and external factors (e.g., cost, counseling). Only two studies covered the actual fitting process. There was only one factor positively affecting all four outcome variables. This was self-reported hearing disability. The vast majority of studies showed no relationship of age and gender with any of the outcome domains. Discussion and conclusion: Whereas research of the last 28 years yielded valuable information regarding relevant and irrelevant factors in hearing aid health care, there are still many relevant issues that have never been investigated in controlled studies. These are discussed.
