Abstract: Aim. The aims of this paper are to review the literature to determine what factors are contributing to the nursing shortage in the United States of America (USA) and discuss possible solutions to this current and future nursing shortage. Background. The need for nurses is often depicted as cyclical in nature. Throughout history, the USA has experienced a series of nursing surpluses and shortages. However, the current shortage has been characterized as being unlike those experienced in the past. Trends of an ageing Registered Nurse (RN) workforce and limited supply to fill the impending vacancies are some of the unique aspects that bring a new dimension to an old problem. Today's nursing shortage will not be resolved by simply returning to the solutions of yesteryear, and strategies to reduce its impact will have to be more creative and focus on the long-term. Methods. Integrative literature review of published literature on the current nursing shortage in the USA from 1999 to 2001. Discussion. Four main areas were identified as the major contributors to the nursing shortage in the USA: the ageing RN workforce; declining enrolment; the changing work climate; and the poor image of nursing. Solutions to the shortage followed similar themes to the contributing factors and encompassed four main areas: exploring recruitment efforts; exploring retainment efforts; improving the image of nursing; and supporting legislation that helps to rectify the shortage. Conclusion. There is firm evidence that the USA amidst a nursing shortage. Much is known about the many contributing factors but now nurses need to become proactive to help secure the future of their workforce. By forming partnerships within the profession and with other influential parties, nurses can be in the forefront of resolving their workforce issues.

Abstract: Experiential learning in nursing programs includes role-play, simulation, and live clinical experiences. Anxiety levels can heighten during experiential learning as students attempt to gain psychomotor skills and transfer knowledge into critical thinking. Nursing students may experience anxiety that can interfere with learning and critical thinking. However, the presence of student anxiety can be used to initiate a purposeful caring transaction between nursing faculty and student. The caring transaction is a way for faculty to model both caring and presence, create experiential learning of caring by students, and lead students to initiate self-care interventions to manage anxiety through the nursing program and beyond. Multiple learning outcomes can be achieved as the students integrate faculty-modeled concepts of caring and presence into simulated or real clinical situations, reduce or manage their anxiety, and improve their clinical judgment and critical thinking skills.

Abstract: Test anxiety is a phenomenon that can affect as many as 40% of students. Many nursing students are under great stress from long hours of study, a rigorous curriculum, and balancing work and family life. These stressors can lead to anxiety in many areas of the student's life, most notably in situations where he or she is being evaluated. This article will aim to discuss how the use of guided reflection can help the student actualize his or her feelings about test anxiety by using Johns's Model for Structured Reflection. By using cues from the model and structure provided by a guide, the student will partake in a journey to gain insight about oneself and discover ways to decrease test anxiety that can be incorporated into the student's holistic self-care plan.

Abstract: The discussion method is a teaching strategy commonly used by nurse educators in a variety of educational settings. However, relatively unknown to nursing education is a unique discussion teaching strategy: the deliberative discussion method. The deliberative discussion method was developed by the National Issues Forums Institute for the sole purpose of creating a means to engage people and communities to dialogue with one another. In essence, a deliberative discussion is a shared inquiry that asks participants to talk through and weigh the costs and consequences of a variety of options of solutions to a public problem. At the heart of deliberation is the group's willingness to work through the conflicts, to accept the consequences of their choices, and to establish grounds for action. Deliberative discussion offers an innovative approach to health care or other nursing issues in the classroom.

Abstract: Deliberative discussion is a teaching method that invites nursing students to engage in a shared inquiry regarding public issues. The purpose of this pretest-posttest control group experimental study was to investigate the effects of using the deliberative discussion method to enhance the critical thinking abilities of first year nursing students. Forty four students were randomly assigned to attend three deliberative discussion sessions over a 13 week period or to the control group. Using the California Critical Thinking Skills Test, no statistically significant differences were found in critical thinking scores between the two groups. The level and depth of students’ critical thinking abilities during the discussions did not increase from session one to session three. The authors of this study suggest that participation in deliberative discussions may not produce an immediate effect on critical thinking and possible long term benefits are not known. Further considerations include having more exposures to the deliberative method and having opportunities to critically reflect after the discussions.

Abstract: Background—Cardiovascular disease (CVD) is the leading cause of death in the United States and is responsible for 17% of national health expenditures. As the population ages, these costs are expected to increase substantially. Methods and Results—To prepare for future cardiovascular care needs, the American Heart Association developed methodology to project future costs of care for hypertension, coronary heart disease, heart failure, stroke, and all other CVD from 2010 to 2030. This methodology avoided double counting of costs for patients with multiple cardiovascular conditions. By 2030, 40.5% of the US population is projected to have some form of CVD. Between 2010 and 2030, real (2008$) total direct medical costs of CVD are projected to triple, from $273 billion to $818 billion. Real indirect costs (due to lost productivity) for all CVD are estimated to increase from $172 billion in 2010 to $276 billion in 2030, an increase of 61%. (2010) Journal of Nursing Management 18, 993–1003 Authentic leadership of preceptors: predictor of new graduate nurses' work engagement and job satisfaction Aim To examine the relationships between new graduate nurses’ perceptions of preceptor authentic leadership, work engagement and job satisfaction. Background During a time when the retention of new graduate nurses is of the upmost importance, the reliance on preceptors to facilitate the transition of new graduate nurses is paramount. Methods A predictive non-experimental survey design was used to examine the relationships between study variables. The final sample consisted of 170 randomly selected Registered Nurses (RNs) with <3 years experience and who worked in an acute care setting. Results Hierarchical multiple regression demonstrated that 20% of the variance in job satisfaction was explained by authentic leadership and work engagement. Furthermore, work engagement was found to partially mediate the relationship between authentic leadership of preceptors and engagement of new graduate nurses. Conclusions New graduate nurses paired with preceptors who demonstrate high levels of authentic leadership feel more engaged and are more satisfied. Engagement is an important mechanism by which authentic leadership affects job satisfaction. Implications for nursing management Managers must be aware of the role preceptors’ authentic leadership plays in promoting work engagement and job satisfaction of new nurses.

Abstract: Aims and objectives. This paper provides a context for this special edition. It highlights the scale of the challenge of nursing shortages, but also makes the point that there is a policy agenda that provides workable solutions. Results. An overview of nurse:population ratios in different countries and regions of the world, highlighting considerable variations, with Africa and South East Asia having the lowest average ratios. The paper argues that the ‘shortage’ of nurses is not necessarily a shortage of individuals with nursing qualifications, it is a shortage of nurses willing to work in the present conditions. The causes of shortages are multi-faceted, and there is no single global measure of their extent and nature, there is growing evidence of the impact of relatively low staffing levels on health care delivery and outcomes. The main causes of nursing shortages are highlighted: inadequate workforce planning and allocation mechanisms, resource constrained undersupply of new staff, poor recruitment, retention and ‘return’ policies, and ineffective use of available nursing resources through inappropriate skill mix and utilisation, poor incentive structures and inadequate career support. Conclusions. What now faces policy makers in Japan, Europe and other developed countries is a policy agenda with a core of common themes. First, themes related to addressing supply side issues: getting, keeping and keeping in touch with relatively scarce nurses. Second, themes related to dealing with demand side challenges. The paper concludes that the main challenge for policy makers is to develop a co-ordinated package of policies that provide a long term and sustainable solution. Relevance to clinical practice. This paper highlights the impact that nursing shortages has on clinical practice and in health service delivery. It outlines scope for addressing shortage problems and therefore for providing a more positive staffing environment in which clinical practice can be delivered.
