Abstract: To update the European System for Cardiac Operative Risk Evaluation (EuroSCORE) risk model. A dedicated website collected prospective risk and outcome data on 22 381 consecutive patients undergoing major cardiac surgery in 154 hospitals in 43 countries over a 12-week period (May-July 2010). Completeness and accuracy were validated during data collection using mandatory field entry, error and range checks and after data collection using summary feedback confirmation by responsible officers and multiple logic checks. Information was obtained on existing EuroSCORE risk factors and additional factors proven to influence risk from research conducted since the original model. The primary outcome was mortality at the base hospital. Secondary outcomes were mortality at 30 and 90 days. The data set was divided into a developmental subset for logistic regression modelling and a validation subset for model testing. A logistic risk model (EuroSCORE II) was then constructed and tested. Compared with the original 1995 EuroSCORE database (in brackets), the mean age was up at 64.7 (62.5) with 31% females (28%). More patients had New York Heart Association class IV, extracardiac arteriopathy, renal and pulmonary dysfunction. Overall mortality was 3.9% (4.6%). When applied to the current data, the old risk models overpredicted mortality (actual: 3.9%; additive predicted: 5.8%; logistic predicted: 7.57%). EuroSCORE II was well calibrated on testing in the validation data subset of 5553 patients (actual mortality: 4.18%; predicted: 3.95%). Very good discrimination was maintained with an area under the receiver operating characteristic curve of 0.8095. Cardiac surgical mortality has significantly reduced in the last 15 years despite older and sicker patients. EuroSCORE II is better calibrated than the original model yet preserves powerful discrimination. It is proposed for the future assessment of cardiac surgical risk. (Less)

Abstract: Control loops that are closed over a communication network get more and more common. A problem with such systems is that the transfer delays will be varying with different characteristics depending on the network hardware and software. The network delays are typically varying due to varying network load, scheduling policies in the network and the nodes, and due to network failures. Two network models of different complexity are studied: Random delays that are independent from transfer to transfer, Random delays with probability distribution functions governed by an underlying Markov chain. The delay models are verified by experimental measurements of network delays. In the thesis it is shown how to analyze stability and expected performance of linear controllers where the network delays are described by one of the two network models above. Methods to evaluate quadratic cost functions are developed. Through the same analysis we find criteria for mean square stability of the closed loop for the different network models. The Linear Quadratic Gaussian (LQG) optimal controller is developed for the two delay models. The derived controller uses knowledge of old time delays. These can be calculated using ``timestamping'' of messages in the network. ``Timestamping'' means that every transfered signal is marked with the time of generation. The receiving node can then calculate how long the transfer delay was by comparing the timestamp with the node's internal clock. (Less)

Abstract: Horizontal temperature gradients are small in the tropical atmosphere, as a consequence of the smallness of the Coriolis parameter near the equator. This provides a strong constraint on both large-scale fluid dynamics and diabatic processes. This work is a step toward the construction of a balanced dynamical theory for the tropical circulation that is based on this constraint, and in which the diabatic processes are explicit and interactive. The authors first derive the basic fluid-dynamical scaling under the weak temperature gradient (WTG) approximation in a shallow water system with a fixed mass source representing an externally imposed heating. This derivation follows an earlier similar one by Held and Hoskins, but extends the analysis to the nonlinear case (though on an f plane), examines the resulting system in more detail, and presents a solution for an axisymmetric ‘‘top-hat’’ forcing. The system is truly balanced, having no gravity waves, but is different from other balance models in that the heating is included a priori in the scaling. The WTG scaling is then applied to a linear moist model in which the convective heating is controlled by a moisture variable that is advected by the flow. This moist model is derived from the Quasi-equilibrium Tropical Circulation Model (QTCM) equations of Neelin and Zeng but can be viewed as somewhat more general. A number of additional approximations are made in order to consider balanced dynamical modes, apparently not studied previously, which owe their existence to interactions of the moisture and flow fields. A particularly interesting mode arises on an f plane with a constant background moisture gradient. In the limit of low frequency and zero meridional wavenumber this mode has a dispersion relation mathematically identical to that of a barotropic Rossby wave, though the phase speed is eastward (for moisture decreasing poleward in the background state) and the propagation mechanism is quite different. This mode also has significant positive growth rate for low wavenumbers. The addition of the b effect complicates matters. For typical parameters, when b is included the direction of phase propagation is ambiguous, and the growth rate reduced, as the effects of the background gradients in moisture and planetary vorticity appear to cancel to a large degree. Possible relevance to intraseasonal variability and easterly wave dynamics is briefly discussed.
