Zika virus (ZIKV) has recently caused a pandemic disease, and many cases of ZIKV infection in pregnant women resulted in abortion, stillbirth, deaths and congenital defects including microcephaly, which now has been proposed as ZIKV congenital syndrome. This study aimed to investigate the in situ immune response profile and mechanisms of neuronal cell damage in fatal Zika microcephaly cases. Brain tissue samples were collected from 15 cases, including 10 microcephalic ZIKV-positive neonates with fatal outcome and five neonatal control flavivirus-negative neonates that died due to other causes, but with preserved central nervous system (CNS) architecture. In microcephaly cases, the histopathological features of the tissue samples were characterized in three CNS areas (meninges, perivascular space, and parenchyma). The changes found were mainly calcification, necrosis, neuronophagy, gliosis, microglial nodules, and inflammatory infiltration of mononuclear cells. The in situ immune response against ZIKV in the CNS of newborns is complex. Despite the predominant expression of Th2 cytokines, other cytokines such as Th1, Th17, Treg, Th9, and Th22 are involved to a lesser extent, but are still likely to participate in the immunopathogenic mechanisms of neural disease in fatal cases of microcephaly caused by ZIKV.

BackgroundZika virus (ZIKV) has been linked to central nervous system malformations in fetuses. To characterize the spectrum of ZIKV disease in pregnant women and infants, we followed patients in Rio de Janeiro to describe clinical manifestations in mothers and repercussions of acute ZIKV infection in infants. MethodsWe enrolled pregnant women in whom a rash had developed within the previous 5 days and tested blood and urine specimens for ZIKV by reverse-transcriptase–polymerase-chain-reaction assays. We followed women prospectively to obtain data on pregnancy and infant outcomes. ResultsA total of 345 women were enrolled from September 2015 through May 2016; of these, 182 women (53%) tested positive for ZIKV in blood, urine, or both. The timing of acute ZIKV infection ranged from 6 to 39 weeks of gestation. Zika Virus Infection in Pregnant Women in Rio de Janeiro - Preliminary Report.

Summary The Zika virus has spread rapidly in the Americas since its first identification in Brazil in early 2015. Prenatal Zika virus infection has been linked to adverse pregnancy and birth outcomes, most notably microcephaly and other serious brain anomalies. To determine whether Zika virus infection during pregnancy causes these adverse outcomes, we evaluated available data using criteria that have been proposed for the assessment of potential teratogens. On the basis of this review, we conclude that a causal relationship exists between prenatal Zika virus infection and microcephaly and other serious brain anomalies. Evidence that was used to support this causal relationship included Zika virus infection at times during prenatal development that were consistent with the defects observed; a specific, rare phenotype involving microcephaly and associated brain anomalies in fetuses or infants with presumed or confirmed congenital Zika virus infection; and data that strongly support biologic plausibility, including the identification of Zika virus in the brain tissue of affected fetuses and infants. Given the recognition of this causal relationship, we need to intensify our efforts toward the prevention of adverse outcomes caused by congenital Zika virus infection. However, many questions that are critical to our prevention efforts remain, including the spectrum of defects caused by prenatal Zika virus infection, the degree of relative and absolute risks of adverse outcomes among fetuses whose mothers were infected at different times during pregnancy, and factors that might affect a woman’s risk of adverse pregnancy or birth outcomes. Addressing these questions will improve our ability to reduce the burden of the effects of Zika virus infection during pregnancy.

Summary A widespread epidemic of Zika virus (ZIKV) infection was reported in 2015 in South and Central America and the Caribbean. A major concern associated with this infection is the apparent increased incidence of microcephaly in fetuses born to mothers infected with ZIKV. In this report, we describe the case of an expectant mother who had a febrile illness with rash at the end of the first trimester of pregnancy while she was living in Brazil. Ultrasonography performed at 29 weeks of gestation revealed microcephaly with calcifications in the fetal brain and placenta. After the mother requested termination of the pregnancy, a fetal autopsy was performed. Micrencephaly (an abnormally small brain) was observed, with almost complete agyria, hydrocephalus, and multifocal dystrophic calcifications in the cortex and subcortical white matter, with associated cortical displacement and mild focal inflammation. ZIKV was found in the fetal brain tissue on reversetranscriptase–polymerase-chain-reaction (RT-PCR) assay, with consistent findings on electron microscopy. The complete genome of ZIKV was recovered from the fetal brain.

In parallel with the observed greater frequency of natural disasters worldwide, there has been an ever-increasing interest in urban resilience and its assessment. Experience obtained in recent extreme events (in particular, earthquakes and floods) has revealed that both the level of preparedness and the response of affected cities were insufficiently high, whereas the recovery process was long and expensive. For this reason improved pre-disaster mitigation actions, as well as smart and strategic urban planning in threatened areas (e.g. in earthquake-prone regions), is essential. For this purpose, a comprehensive review of the existing literature has been performed in relation to the holistic assessment of urban system resilience to natural disasters, with an emphasis on the effect of earthquakes. The main goal of the review was to try to determine how to best assess the resilience of urban systems as a whole, taking into account all of their components, i.e. both the physical components (i.e. of buildings, infrastructure, and open spaces) and the social components (i.e. of the community), as well as the dynamic interactions between them. Besides considering the commonly measured indicators (e.g. determination of the scope of actual structural damage caused by an earthquake), the paper tries to extend the discussion to some indicators which are not so commonly taken into account, by applying a quantitative resilience assessment approach. Based on the results of the described new literature review, a preliminary concept which could be used to assess the seismic resilience of complex urban systems, taking into account all urban components which have been identified as having an important impact on the latter, is presented. This concept consists of three different parts: (i) a probabilistic fragility analysis for each individual physical element (i.e. a building or an infrastructure element), (ii) a composite index methodology for the measurement of community disaster resilience, and (iii) a complex network approach (graph theory) for the assessment of the resilience of urban systems as a whole. Since, in the existing literature, there is a lack of consideration of urban open space, which can have a significant role in the recovery process, it is suggested that, in future research of seismic resilience assessment, such open space should be taken into account, and that an in-depth study of possible recovery strategies be performed.

Resilience assessment of complex urban systems to natural disasters: A new literature review

Avian-specific toll like receptor 15 (TLR15) is functionally equivalent to a group of TLR2 family proteins that the mammalian innate immune system utilizes to recognize a broad spectrum of microbe-associated molecular patterns, including bacterial lipoproteins. In this study we examined the role of chicken TLR2 family members in the innate immune response to the avian pathogenic bacterium, Mycoplasma synoviae. We found that Mycoplasma synoviae, and specifically the N-terminal diacylated lipopeptide (MDLP) representing the amino-terminal portion of its mature haemagglutinin protein, significantly induces the expression of TLR15, but not TLR1 and TLR2 in chicken macrophages and chondrocytes. TLR15 activation is specific and depends on diacylation of the lipopeptide. Activation of TLR15 after stimulation with Mycoplasma synoviae and MDLP triggers an increase in the expression of transcription factor nuclear factor kappa B and nitric oxide production. Moreover, transfection of avian macrophage cells with small interfering RNA reduces the expression of TLR15 after stimulation with MDLP. This leads to decreased activation of the innate immune response, as measured by nitric oxide production. Additionally, pretreatment of cells with neutralizing anti-TLR15 antibody results in a notable attenuation of MDLP-driven release of nitric oxide. This positive correlation may constitute a mechanism for stimulating the innate immune response against avian mycoplasmas in chicken cells via TLR15.

For a city to perform successfully, and its citizens to feel safe and comfortable, the health of basic urban components and the overall resilience of the urban system is crucial. As the importance of the resilient urban system has been recognized in the scientific literature, many studies have been done on this topic. Therefore, to find out the gaps in the existing literature and the opportunities for further research, a new systematic literature review has been performed in three stages. Different bibliographic techniques (co-occurrence and co-citation analysis) have been applied and, in the final stage of the analysis, an in-depth study of the content of the selected papers addressing open space in relation to urban seismic resilience has been carried out. The obtained results and trends have shown a lack of research on the potential of open space for enhancing urban seismic resilience, as well as a challenge for its quantitative assessment. The ability of the affected resilient system is to achieve at least a pre-disaster performance level in an acceptable time, which can be gained, among others, by using the restorative potential of open space. Based on the synthesis of these findings the authors’ draft model of an urban system integrating the potential of open space is presented in terms of a complex network theory.
