Abstract: BACKGROUND: Determining and restoring physiological vertical dimension of occlusion (VDO) is the critical step during complete mouth rehabilitation. The improper VDO compromises the aesthetics, phonetics and functional efficiency of the prosthesis. Various methods are suggested to determine the accurate VDO, including the facial measurements in the clinical situations with no pre-extraction records. The generalisation of correlation between the facial measurements to VDO is criticised due to gender dimorphism and racial differences. Hence, it is prudent to verify the hypothesis of facial proportion and correlation of lower third of the face to remaining craniofacial measurements in different ethnic groups. The objective of the study was to evaluate the correlation of craniofacial measurements and OVD in the Saudi-Arabian ethnic group.METHODOLOGY: Total of 228 participants from Saudi-Arabian Ethnic group were randomly recruited in this cross-sectional study. Fifteen craniofacial measurements were recorded with modified digital Vernier callipers, and OVD was recorded at centric occlusion. The obtained data were analysed by using the Spearman’s correlation and linear regression analysis.RESULTS: The Mean OVD in male participants was higher (69.25 ± 5.54) in comparison to female participants (57.41 ± 5.32). The craniofacial measurement of Exocanthion-right labial commissure and the Mesial wall of the right external auditory canal-orbitale lateral had a strong positive correlation with VDO. The strong correlation was recorded with a trichion-upper border of right eyebrow line and trichion-Nasion only in males. Meanwhile, the length of an auricle recorded the positive correlation in female participants.CONCLUSIONS: Being simple and non-invasive technique, craniofacial measurements and linear equations could be routinely utilised to determine VDO.

Abstract: Introduction. The endodontically treated teeth (ETT) with thin remaining radicular dentin thickness are predisposed to fracture; hence it requires the diligent selection and the execution of endodontic post treatment. The objective of the study was to evaluate the reinforcing effect of both multiple fiber reinforced composite (FRC) and Ni-Cr cast metal posts at anterior and posterior regions. Material and Methods. Forty recently extracted root canal treated canine and single rooted premolar teeth were used for the study. They were randomly divided into four groups (n=10) as: Group 1, single FRC post; Group 2, multiple FRC posts; Group 3, single Ni-Cr metal post, Group 4, multiple Ni-Cr posts. The posts were cemented with self-adhesive resin cement and subsequently restored with full veneer metal crown. The compressive static load at 1300 for canine and 450 for premolar was applied with the cross-head speed of 0.5mm/minute until the fracture. The obtained data was analyzed using the Kruskal–Wallis and Pairwise comparison tests with SPSS. Results. The results indicate that multiple FRC post restored canine had the maximum fracture load (1843.80±7.13 N), followed by cast multiple posts (1648.99±26.84 N), single fiber post (1623±40.31 N), and cast metal single post (1493±27.33 N). A similar trend was observed in premolar with higher max fracture load with multiple FRC posts at 1920.86±20.61 N and multiple cast metal posts at 1735.43±6.05 N. Conclusion. The restoration of ETT with larger canals by multiple FRC and metal posts provides substantially higher fracture resistance in comparison to wider single post.

Abstract: European-American norms are still used in the orthodontic treatment of Saudi patients, despite the different ethnic backgrounds of Saudis. The aims of this study were to evaluate the cephalometric features of a Saudi population and to establish cephalometric norms for Saudis living in the western region of Saudi Arabia. Seventy lateral cephalometric radiographs of Saudis (32 females and 38 males; aged 18–28 years) with acceptable profiles and Class I dental relationships were traced and analyzed. The mean value, standard deviation, and range of 16 angular and linear variables were calculated. The resulting norms for Saudis were compared with European- American norms using an independent t-test. Male and female groups were also compared using the t-test. Saudis tend to have an increased ANB angle because of retrognathic mandibles and bimaxillary protrusion as compared with European-Americans. Males tend to have more prognathic mandibles than females as indicated by the statistically significant increase in facial angle (P , .05) and SNB angle (P , .05). Although the anterior lower face height was similar in males and females, males tend to have a steeper mandibular plane angle when related to the anterior cranial base than females (P , .05). Saudis have distinct cephalometric features, which should be used as a reference in treating Saudi orthodontic patients.

Abstract: PEEK has been used in many dental applications except intra-radicular post. The aim of this study is to test polyetheretherketone (PEEK) as a dental post material through tensile bond strength (TBS) and surface roughness (SR), and to compare it with glass-fiber and cast-metal posts. Thus, 60 human maxillary central incisors with a single root were endodontically treated and divided into three groups (n = 20) according to the type of post (Group P: PEEK, Group F: Glass-fiber, Group M: Cast-metal). Appropriate surface treatment was employed for each group and SR was determined by a three-dimensional non-contact profilometer before cementation. All posts were luted to the canal dentin using self-etch resin cement (Panavia F2.0). Pull-out test was performed on a universal testing machine at a speed of 0.5 mm/min crosshead speed until failure, and TBS were calculated. One-way ANOVA, Tukey's HSD, and Pearson chi-squared tests were performed for statistical analyses (α = 0.05). According to the results, group F demonstrated the highest SR (2.93 ± 0.18 µm) and lowest TBS values (10.05 ± 0.53 MPa), while group P exhibited lowest SR (1.37 ± 0.11 µm) and highest TBS values (14.33 ± 0.58 MPa) (p < 0.001). No significant differences in failure modes were identified among groups, mostly adhesive (p = 0.243). As conclusion, PEEK may be a reliable and contemporary option for dental post systems when used with appropriate surface treatment and luting agent. This high-performance polymer may be a novel candidate as a contemporary dental post system due to its superior mechanical, chemical, thermal, and esthetical properties with low risk of fracture.

Abstract: Objective To perform a systematic review of the literature focused to evaluate in vitro function of prefabricated fiber posts with and without customization by additional auxillary fiber posts and composite resin on the fracture strength of wide or enlarged canals and the failure pattern. Methods Six databases were used as primary search sources (PubMed, Scopus, LILACS, SciELO, Science Direct, and Web of Science) and three databases (Open Grey, Open Thesis, and OATD) were used to partially capture the “grey literature”. The research included laboratory studies that used human upper anterior teeth aiming to assess the fracture strength and failure pattern of different glass fiber post customizations by additional auxiliary fiber posts or composite resin. The search had no restriction of year, language, and publication status. The risk of bias of the studies was assessed from the criteria established in systematic reviews of laboratory studies. Standardized mean differences were calculated by comparing the mean fracture strengths of customized and non-customized posts. Pooled estimates were calculated by Glass' delta method using the random-effects model. Subtotal estimates were presented according to each type of relining procedure and an overall estimate was described considering all studies combined. Results The search provided 2291 results, from which six met the eligibility criteria and were included in the qualitative assessment of the review. Only three studies presented a moderate risk of bias. The meta-analysis results showed that the use of auxiliary posts produced higher mean fracture strengths than non-customized posts (SMD = 2.21; 95%CI: 0.74; 3.68), and it was more effective than the use of composite resin to reline the posts. Conclusion Based on laboratories studies, even though has not been observed any difference to a statistically significant level on fracture strength and failure pattern of the customized and non-customized post, future studies should follow a standardized approach to implementation and reporting of data.

Abstract: AIM To evaluate the fracture resistance under static loading of endodontically treated teeth (maxillary central incisors) restored with carbon fiber posts, glass fiber posts, and everStick posts. The objectives of this study were to check the fracture resistance of the everStick post and compare it with glass fiber posts and carbon fiber posts and evaluated their modes of failure. MATERIALS AND METHODS An estimated 80 freshly extracted human maxillary central incisors were collected for this study. The coronal portions of 60 teeth were sectioned 4 mm incisal to the cemento-enamel junction (CEJ) and 20 teeth were left intact to be used as controls. All the samples were embedded vertically in acrylic resin blocks and the groups were divided as follows. I-control group, II-carbon fiber group, III-glass fiber group, and IV-everStick (E-glass) group. Root canal treatment was completed in all the 80 teeth of 4 groups. Control group teeth were restored with a composite. In all other teeth, post and core placement was carried out. All specimens were mounted on a test block and subjected to static loading until fracture and the mode of fracture was recorded. Statistical analysis was carried out using ANOVA to calculate the mean values of all groups. An intergroup comparison was carried out using Tukey's post hoc test. RESULTS The study showed that group I showed the highest fracture resistance followed by group IV, which is everStick post group under static loading. There was a significant difference among test groups. The main mode of fracture was repairable as there was debonding of the core in all the fiber posts. CONCLUSION Teeth of the control group showed highest fracture resistance and teeth restored with everStick posts showed higher fracture resistance than those restored with glass fiber or carbon fiber posts under static loading (p < 0.05). The principal mode of fracture was debonding of core and remaining root. Teeth restored without application of a post showed the highest fracture resistance than all other groups. CLINICAL SIGNIFICANCE EverStick/E-glass fiber posts showed significantly higher fracture resistance than the other fiber posts and can be suggested for clinical use.
