Evidence is mounting to suggest a causal relationship between the built environment and people’s physical activity behaviours, particularly active transport. The evidence base has been hindered to date by restricted consideration of cost and economic factors associated with built environment interventions, investigation of socioeconomic or ethnic differences in intervention effects, and an inability to isolate the effect of the built environment from other intervention types. The aims of this systematic review were to identify which environmental interventions increase physical activity in residents at the local level, and to build on the evidence base by considering intervention cost, and the differential effects of interventions by ethnicity and socioeconomic status. A systematic database search was conducted in June 2015. Articles were eligible if they reported a quantitative empirical study (natural experiment or a prospective, retrospective, experimental, or longitudinal research) investigating the relationship between objectively measured built environment feature(s) and physical activity and/or travel behaviours in children or adults. Quality assessment was conducted and data on intervention cost and whether the effect of the built environment differed by ethnicity or socioeconomic status were extracted. Twenty-eight studies were included in the review. Findings showed a positive effect of walkability components, provision of quality parks and playgrounds, and installation of or improvements in active transport infrastructure on active transport, physical activity, and visits or use of settings. There was some indication that infrastructure improvements may predominantly benefit socioeconomically advantaged groups. Studies were commonly limited by selection bias and insufficient controlling for confounders. Heterogeneity in study design and reporting limited comparability across studies or any clear conclusions to be made regarding intervention cost. Improving neighbourhood walkability, quality of parks and playgrounds, and providing adequate active transport infrastructure is likely to generate positive impacts on activity in children and adults. The possibility that the benefits of infrastructure improvements may be inequitably distributed requires further investigation. Opportunities to improve the quality of evidence exist, including strategies to improve response rates and representativeness, use of valid and reliable measurement tools, cost-benefit analyses, and adequate controlling for confounders.

Does active commuting improve psychological wellbeing? Longitudinal evidence from eighteen waves of the British Household Panel Survey

Background Increasing global urbanisation has resulted in a greater proportion of the world9s population becoming exposed to risk factors unique to urban areas, and understanding these effects on public health is essential. The aim of this study was to examine the association between access to green space and mental health among adult twin pairs. Methods We used a multilevel random intercept model of same-sex twin pairs (4338 individuals) from the community-based University of Washington Twin Registry to analyse the association between access to green space, as measured by the Normalised Difference Vegetation Index and self-reported depression, stress, and anxiety. The main parameter of interest was the within-pair effect for identical (monozygotic, MZ) twins because it was not subject to confounding by genetic or shared childhood environment factors. Models were adjusted for income, physical activity, neighbourhood deprivation and population density. Results When treating twins as individuals and not as members of a twin pair, green space was significantly inversely associated with each mental health outcome. The association with depression remained significant in the within-pair MZ univariate and adjusted models; however, there was no within-pair MZ effect for stress or anxiety among the models adjusted for income and physical activity. Conclusions These results suggest that greater access to green space is associated with less depression, but provide less evidence for effects on stress or anxiety. Understanding the mechanisms linking neighbourhood characteristics to mental health has important public health implications. Future studies should combine twin designs and longitudinal data to strengthen causal inference.

Access to green space, physical activity and mental health: a twin study

To maximise the impact of public health research, research interventions found to be effective in improving health need to be scaled up and delivered on a population-wide basis. Theoretical frameworks and approaches are useful for describing and understanding how effective interventions are scaled up from small trials into broader policy and practice and can be used as a tool to facilitate effective scale-up. The purpose of this literature review was to synthesise evidence on scaling up public health interventions into population-wide policy and practice, with a focus on the defining and describing frameworks, processes and methods of scaling up public health initiatives. The review involved keyword searches of electronic databases including MEDLINE, CINAHL, PsycINFO, EBM Reviews and Google Scholar between August and December 2013. Keywords included ‘scaling up’ and ‘scalability’, while the search terms ‘intervention research’, ‘translational research’, ‘research dissemination’, ‘health promotion’ and ‘public health’ were used to focus the search on public health approaches. Studies included in the review were published in English from January 1990 to December 2013 and described processes, theories or frameworks associated with scaling up public health and health promotion interventions. There is a growing body of literature describing frameworks for scaling health interventions, with the review identifying eight frameworks, the majority of which have an explicit focus on scaling up health action in low and middle income country contexts. Key success factors for scaling up included the importance of establishing monitoring and evaluation systems, costing and economic modelling of intervention approaches, active engagement of a range of implementers and the target community, tailoring the scaled-up approach to the local context, the use of participatory approaches, the systematic use of evidence, infrastructure to support implementation, strong leadership and champions, political will, well defined scale-up strategy and strong advocacy. Effective scaling up requires the systematic use of evidence, and it is essential that data from implementation monitoring is linked to decision making throughout the scaling up process. Conceptual frameworks can assist both policy makers and researchers to determine the type of research that is most useful at different stages of scaling up processes.

Narrative review of models and success factors for scaling up public health interventions.

Policies and changes to the built environment are promising targets for obesity prevention efforts and can be evaluated as 'natural'- or 'quasi'-experiments. This systematic review examined the use of natural- or quasi-experiments to evaluate the efficacy of policy and built environment changes on obesity-related outcomes (body mass index, diet or physical activity). PubMed (Medline) was searched for studies published 2005-2013; 1,175 abstracts and 115 papers were reviewed. Of the 37 studies included, 18 studies evaluated impacts on nutrition/diet, 17 on physical activity and 3 on body mass index. Nutrition-related studies found greater effects because of bans/restrictions on unhealthy foods, mandates offering healthier foods, and altering purchase/payment rules on foods purchased using low-income food vouchers compared with other interventions (menu labelling, new supermarkets). Physical activity-related studies generally found stronger impacts when the intervention involved improvements to active transportation infrastructure, longer follow-up time or measured process outcomes (e.g., cycling rather than total physical activity), compared with other studies. Only three studies directly assessed body mass index or weight, and only one (installing light-rail system) observed a significant effect. Studies varied widely in the strength of their design and studies with weaker designs were more likely to report associations in the positive direction.

Impact of policy and built environment changes on obesity-related outcomes: a systematic review of naturally occurring experiments.

Active travel, particularly walking and cycling, has been recommended because of the health benefits associated with increased physical activity. Use of public transport generally involves some walking to bus stops or train stations. This paper is a systematic review of how much time is spent in physical activity among adults using public transport. It also explores the potential effect on the population level of physical activity if inactive adults in NSW, Australia, increased their walking through increased use of public transport. Of 1,733 articles, 27 met the search criteria, and nine reported on absolute measures of physical activity associated with public transport. A further 18 papers reported on factors associated with physical activity as part of public transport use. A range of 8-33 additional minutes of walking was identified from this systematic search as being attributable to public transport use. Using "bootstrapping" statistical modelling, if 20% of all inactive adults increased their walking by only 16 minutes a day for five days a week, we predict there would be a substantial 6.97% increase in the proportion of the adult population considered "sufficiently active". More minutes walked per day, or a greater uptake of public transport by inactive adults would likely lead to significantly greater increases in the adult population considered sufficiently active.

Physical Activity Associated with Public Transport Use—A Review and Modelling of Potential Benefits

More intervention research is needed, particularly 'real world' intervention replication and dissemination studies, to optimize improvements in health. This study assessed the proportion and type of published public health intervention research papers over time in physical activity and falls prevention, both important contributors to preventable morbidity and mortality.

Public health research outputs from efficacy to dissemination: a bibliometric analysis.

Issue addressed: Despite recognition that urban infrastructure influences physical activity, there have been no comparisons between Australian city-level patterns of physical activity. This study ranked Australian cities in terms of adults' participation in leisure-time physical activity and examined city-level variations in activity trends between 2001 and 2009. Methods: Data on participation in leisure-time physical activity in adults (>/-15 years) between 2001 and 2009 were obtained from the Exercise Recreation and Sport Survey (ERASS), a computer-assisted telephone interview conducted to collect population-level sport participation information by the Australian Sports Commission. Data were analysed for respondents residing in the eight capital cities of Australia. The prevalence of meeting recommended 'health-enhancing physical activity' (HEPA) and levels of walking were calculated by age, gender and survey year. Multiple linear logistic regression analyses were used to compare cities. Results: Pooled data from 174,323 adults across years showed that Melbourne, Brisbane, Perth and Canberra residents were significantly more active than Sydney, Adelaide and Hobart residents in terms of HEPA. Hobart, Perth and Melbourne residents were significantly more likely to walk >/-5 sessions a week compared with their counterparts in other cities. HEPA and walking increased across most cities between 2001 and 2009. Conclusion: There are significant differences between Australian cities in physical activity and walking levels, over and above differences attributable to age, gender or educational levels. While this may be due to infrastructure differences, comparative information on indicators of the built environment and transport infrastructure are not available.
