Abstract: Introduction: Ankylosing spondylitis (AS) can lead to an increased risk of cervical fractures. Sources of data: A systematic review was undertaken using the keywords ‘ankylosing spondylitis’, ‘spine fractures’, ‘cervical fractures’, ‘surgery’ and ‘postoperative outcomes’ on Medline, Pubmed, Google Scholar, Ovid and Embase, and the quality of the studies included was evaluated according to the Coleman Methodology Score. Areas of agreement: Surgery ameliorates neurological function in patients with unstable AS-related cervical fractures. The combined anterior/posterior and the posterior approaches are more effective than the anterior approach. Areas of controversy: The optimal approach, anterior, posterior or combined anterior/posterior, for the management of AS related cervical fractures has not been defined.

Abstract: Chronic Achilles tendon rupture posed significant disability to the patients. Flexor hallucis longus (FHL) transfer can be used to restore the plantarflexion power of the ankle. Traditionally, a long medial incision with extensive soft tissue dissection is needed for FHL transfer. With the advance in flexor hallucis tendoscopy, endoscopic FHL tendon transfer can be performed without extensive soft tissue dissection.

Abstract: Purpose
Iliotibial tract friction syndrome (ITBFS) might be caused by repetitive friction and abrasion of the iliotibial (ITB) tract across the lateral femoral epicondyle. Thickening in the ITB adjacent to the lateral femoral epicondyle has been reported. The values of ITB thickness at the lateral femoral epicondyle in healthy adult volunteers were determined, and the association between the thickness of the ITB and the subjects’ age, weight, height, dominant limb and the volume of training or sporting activity was assessed.

Abstract: Partial-thickness rotator cuff tears present partial disruption of tendon fibers with no communication between the subacromial bursa and the glenohumeral joint. The clinical presentation is surprisingly variable, ranging from mild discomfort to decreased throwing speed, chronic pain, and shoulder inability. The first approach to partial-thickness rotator cuff tears is usually conservative, but the hypovascularity of the critical zone and mechanical factors often result in poor spontaneous tendon healing. Surgical options include arthroscopic cuff "debridement" or "repair," performed arthroscopically or by open surgery, and subacromial decompression or debridement if necessary. No agreement has been reached on the best surgical management. However, repair is usually indicated if bursal-sided and articular tears involve more than 50% of tendon thickness; debridement is generally undertaken if <50% of the rotator cuff is torn. There is a need for randomized clinical trails formulating and testing guidelines of management and for further studies on imaging or intraoperative measures and methods to assess the thickness of the rotator cuff to inform management.

STRATEGIES FOR IMPROVING DIABETES CARE D iabetes is a chronic illness that requires continuing medical care and patient self-management education to prevent acute complications and to reduce the risk of long-term complications. Diabetes care is complex and requires that many issues, beyond glycemic control, be addressed. A large body of evidence exists that supports a range of interventions to improve diabetes outcomes. These standards of care are intended to provide clinicians, patients, researchers, payors, and other interested individuals with the components of diabetes care, treatment goals, and tools to evaluate the quality of care. While individual preferences, comorbidities, and other patient factors may require modification of goals, targets that are desirable for most patients with diabetes are provided. These standards are not intended to preclude more extensive evaluation and management of the patient by other specialists as needed. For more detailed information, refer to Bode (Ed.): Medical Management of Type 1 Diabetes (1), Burant (Ed): Medical Management of Type 2 Diabetes (2), and Klingensmith (Ed): Intensive Diabetes Management (3). The recommendations included are diagnostic and therapeutic actions that are known or believed to favorably affect health outcomes of patients with diabetes. A grading system (Table 1), developed by the American Diabetes Association (ADA) and modeled after existing methods, was utilized to clarify and codify the evidence that forms the basis for the recommendations. The level of evidence that supports each recommendation is listed after each recommendation using the letters A, B, C, or E.
