Frailty is a geriatric syndrome of increased vulnerability to stressors that has been implicated as a causative and prognostic factor in patients with cardiovascular disease (CVD). The American Heart Association and the Society of Geriatric Cardiology have called for a better understanding of frailty as it pertains to cardiac care in the elderly. The aim of this study was to systematically review studies of frailty in patients with CVD. A search was conducted of Ovid MEDLINE, EMBASE, the Cochrane Database, and unpublished sources. Inclusion criteria were an assessment of frailty using systematically defined criteria and a study population with prevalent or incident CVD. Nine studies were included, encompassing 54,250 elderly patients with a mean weighted follow-up of 6.2 years. In community-dwelling elders, CVD was associated with an odds ratio (OR) of 2.7 to 4.1 for prevalent frailty and an OR of 1.5 for incident frailty in those who were not frail at baseline. Gait velocity (a measure of frailty) was associated with an OR of 1.6 for incident CVD. In elderly patients with documented severe coronary artery disease or heart failure, the prevalence of frailty was 50% to 54%, and this was associated with an OR of 1.6 to 4.0 for all-cause mortality after adjusting for potential confounders. In conclusion, there exists a relation between frailty and CVD; frailty may lead to CVD, just as CVD may lead to frailty. The presence of frailty confers an incremental increase in mortality. The role of frailty assessment in clinical practice may be to refine estimates of cardiovascular risk, which tend to be less accurate in the heterogenous elderly patient population.

Role of frailty in patients with cardiovascular disease.

Background. Care for elderly persons with disabilities is usually characterized by fragmentation, often leading to more intrusive and expensive forms of care such as hospitalization and institutionalization. There has been increasing interest in the ability of integrated models to improve health, satisfaction, and service utilization outcomes. Methods. A program of integrated care for vulnerable community-dwelling elderly persons (SIPA [French acronym for System of Integrated Care for Older Persons]) was compared to usual care with a randomized control trial. SIPA offered community-based care with local agencies responsible for the full range and coordination of community and institutional (acute and long-term) health and social services. Primary outcomes were utilization and public costs of institutional and community care. Secondary outcomes included health status, satisfaction with care, caregiver burden, and out-of-pocket expenses. Results. Accessibility was increased for health and social home care with increased intensification of home health care. There was a 50% reduction in hospital alternate level inpatient stays (‘‘bed blockers’’) but no significant differences in utilization and costs of emergency department, hospital acute inpatient, and nursing home stays. For all study participants, average community costs per person were C$3390 higher in the SIPA group but institutional costs were C$3770 lower with, as hypothesized, no difference in total overall costs per person in the two groups. Satisfaction was increased for SIPA caregivers with no increase in caregiver burden or out-of-pocket costs. As expected, there was no difference in health outcomes. Conclusions. Integrated systems appear to be feasible and have the potential to reduce hospital and nursing home utilization without increasing costs.

A System of Integrated Care for Older Persons With Disabilities in Canada: Results From a Randomized Controlled Trial

The prevalence of chronic diseases today calls for new ways of working with patients to manage their care. Although patient-centered approaches have contributed to significant advances in care and to treatments that more fully respect patients' preferences, values, and personal experiences, the reality is that health care professionals still hold a monopoly on the role of healer. Patients live with their conditions every day and are experts when it comes to their own experiences of illness; this expertise should be welcomed, valued, and fostered by other members of the care team. The patient-as-partner approach embodies the ideal of making the patient a bona fide member of the health care team, a true partner in his or her care. Since 2010, the University of Montreal, through the Direction of Collaboration and Patient Partnership, has embraced this approach. Patients are not only active members of their own health care team but also are involved in research and provide valuable training to health sciences students. Including patients as full partners in the health care team entails a significant shift in both the medical practice and medical education cultures. In this perspective, the authors describe this innovative approach to patient care, including the conceptual framework used in its development and the main achievements of patient partners in education, health care, and research.

Dans le souci d’ameliorer la sante de la population et d’ameliorer la qualite des soins et des services delivres par le systeme de sante, une des voies prometteuses est l’engagement des patients a tous les niveaux du systeme de sante ainsi que dans la formation des professionnels de la sante. Depuis 2010, un nouveau modele relationnel, base sur le partenariat entre les patients et les professionnels de la sante, a ete developpe a la Faculte de medecine de l’Universite de Montreal. Ce modele de partenariat patient s’appuie sur la reconnaissance des savoirs experientiels du patient, issus de la vie avec la maladie, et complementaires des savoirs scientifiques des professionnels de la sante. Il s’inscrit dans un continuum d’engagement des patients et peut s’appliquer dans les milieux de soins, de la formation des professionnels, de l’enseignement et de la recherche. Nous exposerons dans cet article, les fondements theoriques du partenariat patient, puis comment ce nouveau modele peut etre mis en œuvre aux niveaux clinique, organisationnel et systemique et quels en sont les facteurs de reussite tant du cote des patients que des professionnels de la sante. Le « Montreal Model », en rendant le patient un partenaire incontournable pour toutes les decisions qui le concernent et comme expert de l’organisation des soins, offre aujourd’hui des perspectives pertinentes pour la gestion des maladies chroniques. Cette approche devrait avoir un impact important sur la sante des populations en ameliorant la sante physique, psychologique et le bien-etre des personnes. Dans ce cadre, plusieurs programmes de recherches sont actuellement en cours pour en evaluer l’impact.

Le « Montreal model » : enjeux du partenariat relationnel entre patients et professionnels de la santé

Budget constraints, technological advances and a growing elderly population have resulted in major reforms in health care systems across Canada. This has led to fewer and smaller acute care hospitals and increasing pressure on the primary care and continuing care networks. The present system of care for the frail elderly, who are particularly vulnerable, is characterized by fragmentation of services, negative incentives and the absence of accountability. This is turn leads to the inappropriate and costly use of health and social services, particularly in acute care hospitals and long-term care institutions. Canada needs to develop a publicly managed community-based system of primary care to provide integrated care for the frail elderly. The authors describe such a model, which would have clinical and financial responsibility for the full range of health and social services required by this population. This model would represent a major challenge and change for the existing system. Demonstration projects are needed to evaluate its cost-effectiveness and address issues raised by its introduction.

Care for Canada's frail elderly population: Fragmentation or integration?

Objectives: To assess outcomes and satisfaction among frail elderly day care clients and their informal caregivers and the impact of adult day care on the cost of health services. Methods:One-hundred eight elderly participants were randomly assigned to the experimental group (immediate admission to an adult day care center) and 104 participants to the control group (3 months on a waiting list). Results:Participants’ and caregivers’ subjective perceptions of the day center’s effects were positive. However, using standard research instruments, there was no evidence of an effect of day center attendance on the client’s anxiety, depression, or functional status; on caregiver burden; or on the cost of health services. Discussion:It is difficult to demonstrate objectively the benefits of programs and interventions that are perceived by clients, caregivers, and staff to have positive effects. In future studies, maintenance of high levels of participation should be incorporated as an explicit program goal.
