Abstract: Recently in a series of articles ([1], [2], [3], [6], [7], [8]) the estimation of the standard deviation of the normal distribution based on the sample standard deviation, s, has been thoroughly explored-both for unbiased estimation ([1], [2], [3], [6]) and minimum mean square error estimation ([7], [8]). If one is willing to retain the assumption of normality then it is well known that many simple highly efficient linear estimates of the standard deviation also exist (for example in [4] Chapter 9). The usual drawbacks of these linear estimates are that they are appropriate either to small samples (e.g., the sample range) or large samples (e.g., estimates using a few sample quantiles) or else they do apply to all sample sizes (e.g., the BLUE estimates) but involve elaborate weighting schemes for the ordered observations-thus rendering them chiefly only of academic interest. In the following we examine one linear estimate first proposed by Downton [5]. It does not suffer the above mentioned drawbacks and further it compares extremely well with the estimates based on s. Abstract: Efficacy trials, designed to gain regulatory marketing approval, evaluate drugs in optimally selected patients under advantageous conditions for relatively short time periods. Effectiveness trials, designed to evaluate use in usual practice, assess treatments among more typical patients in real-world conditions with longer follow-up periods. In "efficacy-to-effectiveness (E2E) trials," if the initial efficacy trial component is positive, the trial seamlessly transitions to an effectiveness trial component to efficiently yield both types of evidence. Yet more time could be saved by simultaneously addressing efficacy and effectiveness in an "efficacy and effectiveness too (EE2) trial." Additionally, hybrids of the E2E and EE2 approaches with differing degrees of overlap of the two components could allow flexibility for specific drug development needs. In planning EE2 trials, each stakeholder's current and future needs, incentives, and perspective must be considered. Although challenging, the ultimate benefits to stakeholders, the health system, and the public should justify this effort.

Abstract: Metabolomic studies of increased fat oxidation showed increase in circulating acylcarnitines C2, C8, C10, and C12 and decrease in C3, C4, and C5. We hypothesize that urinary F2-isoprostanes reflect intensity of fatty acid oxidation and are associated with circulating C2, C8, C10, and C12 directly and with C3, C4, and C5 inversely. Four urinary F2-isoprostane isomers and serum acylcarnitines are quantified using LC-MS/MS within the Insulin Resistance Atherosclerosis Study nondiabetic cohort (n = 682). Cross-sectional associations between fasting urinary F2-isoprostanes (summarized as a composite index) and the selected acylcarnitines are examined using generalized linear models. F2-isoprostane index is associated with C2 and C12 directly and with C5 inversely: the adjusted beta coefficients are 0.109, 0.072, and −0.094, respectively (P < 0.05). For these acylcarnitines and for F2-isoprostanes, the adjusted odds ratios (ORs) of incident diabetes are calculated from logistic regression models: the ORs (95% CI) are 0.77 (0.60–0.97), 0.79 (0.62–1.01), 1.18 (0.92–1.53), and 0.51 (0.35–0.76) for C2, C12, C5, and F2-isoprostanes, respectively. The direction of the associations between urinary F2-isoprostanes and three acylcarnitines (C2, C5, and C12) supports our hypothesis. The inverse associations of C2 and C12 and with incident diabetes are consistent with the suggested protective role of efficient fat oxidation.

Abstract: The Centers for Medicare & Medicaid Services (CMS) recently approved lung cancer screening with computed tomography (CT); smoking is a risk factor for lung cancer and atherosclerotic cardiovascular disease (ASCVD).1 Data are needed regarding the cardiovascular risk of patients who are eligible for lung cancer screening, including implications of coronary artery calcification (CAC) visible on lung cancer screening CT. In a longitudinal primary prevention cohort study, we determined lung cancer screening eligibility in the community and its association with statin eligibility, CAC, and incident ASCVD events. A total of 3000 asymptomatic FHS (Framingham Heart Study Offspring Cohort) participants aged 55 to 77 years and free of prevalent cardiovascular disease or lung cancer were divided based on eligibility for lung cancer screening CT by CMS criteria: (1) aged 55 to 77 years (all by definition), (2) current or recent (≤15 years) former smoker, and (3) ≥30 pack-year cigarette smoking history.1 Participants were enrolled at enrolled at examinations 3 (1984-1987) and 7 (1998-2001) and followed for the primary outcome of incident ASCVD (myocardial infarction, death due to coronary heart disease, or ischemic stroke) and the secondary outcome of lung cancer. Participants were contacted annually with suspected ASCVD or lung cancer events adjudicated by a panel of 3 physicians based on review of medical records, histopathology reports, and death certificates. A subgroup of 980 from examination 7 had electrocardiography-gated calcium score multidetector CT. The institutional review boards of Boston University Medical Center and Massachusetts General Hospital approved the study. …

Abstract: G*Power (Erdfelder, Faul, & Buchner, 1996) was designed as a general stand-alone power analysis program for statistical tests commonly used in social and behavioral research. G*Power 3 is a major extension of, and improvement over, the previous versions. It runs on widely used computer platforms (i.e., Windows XP, Windows Vista, and Mac OS X 10.4) and covers many different statistical tests of thet, F, and χ2 test families. In addition, it includes power analyses forz tests and some exact tests. G*Power 3 provides improved effect size calculators and graphic options, supports both distribution-based and design-based input modes, and offers all types of power analyses in which users might be interested. Abstract: "The Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure" provides a new guideline for hypertension prevention and management. The following are the key messages(1) In persons older than 50 years, systolic blood pressure (BP) of more than 140 mm Hg is a much more important cardiovascular disease (CVD) risk factor than diastolic BP; (2) The risk of CVD, beginning at 115/75 mm Hg, doubles with each increment of 20/10 mm Hg; individuals who are normotensive at 55 years of age have a 90% lifetime risk for developing hypertension; (3) Individuals with a systolic BP of 120 to 139 mm Hg or a diastolic BP of 80 to 89 mm Hg should be considered as prehypertensive and require health-promoting lifestyle modifications to prevent CVD; (4) Thiazide-type diuretics should be used in drug treatment for most patients with uncomplicated hypertension, either alone or combined with drugs from other classes. Certain high-risk conditions are compelling indications for the initial use of other antihypertensive drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, β-blockers, calcium channel blockers); (5) Most patients with hypertension will require 2 or more antihypertensive medications to achieve goal BP (<140/90 mm Hg, or <130/80 mm Hg for patients with diabetes or chronic kidney disease); (6) If BP is more than 20/10 mm Hg above goal BP, consideration should be given to initiating therapy with 2 agents, 1 of which usually should be a thiazide-type diuretic; and (7) The most effective therapy prescribed by the most careful clinician will control hypertension only if patients are motivated. Motivation improves when patients have positive experiences with and trust in the clinician. Empathy builds trust and is a potent motivator. Finally, in presenting these guidelines, the committee recognizes that the responsible physician's judgment remains paramount.

Abstract: The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension.
