Abstract: Einleitung Psychopharmaka sind gut bekannte Ausloser klinisch bedeutsamer Blutdruckanderungen. Unser Ziel war es, Rangreihen fur das Risiko einer Psychopharmaka-assoziierten arteriellen Hyper- bzw. Hypotonie zu erstellen. Methoden Es wurden fur 105 in Deutschland in der Erwachsenenpsychiatrie zugelassene Psychopharmaka samtliche in der Roten oder Gelben Liste online verfugbaren 784 Fachinformationen (FI) ausgewertet und die dort standardisiert genannten Risiken einer Blutdrucksteigerung oder -senkung extrahiert. Ergebnisse Fur blutdrucksteigernde Effekte hatte Atomoxetin laut FI das hochste Risiko („sehr haufig“, > 10 %) und 15 weitere Psychopharmaka waren in der Kategorie „haufig“ (> 1 %): Duloxetin, Milnacipran, Venlafaxin, Bupropion, Citalopram, Tranylcypromin (v. a. bei Diatfehlern), Reboxetin, Methylphenidat, Clozapin, Paliperidon, Risperidon, Buprenorphin+Naloxon, Memantin, Galantamin und Rivastigmin. Umgekehrt waren mit Amitriptylin, Tranylcypromin, Chlorprothixen, Flupentixol, Levomepromazin, Olanzapin und Trimipramin 7 Stoffe „sehr haufig“ als mit blutdrucksenkenden Effekten assoziiert beschrieben, weitere 25 „haufig“. Fur viele andere Psychopharmaka war kein Hypertonie- bzw. Hypotonie-Risiko dokumentiert. Nebenbefundlich fanden sich in den FIs verschiedener Hersteller zu einer Substanz teils erhebliche Unterschiede (z. B. bei Citalopram), sodass fur viele Stoffe keine eindeutige Risikozuordnung bestand. Diskussion Gemas den FIs weisen viele Psychopharmaka ein Hypertonie- und starker noch ein Hypotonie-Risiko auf. Nur vereinzelt zeigten sich Gruppeneffekte, z. B. Blutdruckerhohung durch noradrenerge Antidepressiva. Gangige Tabellen zu medikamentosen Ursachen der arteriellen Hypertonie sind im Bereich Psychopharmaka revisionswurdig. Unsere Rangreihen dienen der Auswahl geeigneter Psychopharmaka bei Patienten mit bekanntem Hypertonus bzw. Synkopenneigung und bei Blutdruckentgleisungen unter Psychopharmaka. Fur eine definitive Risikobewertung sind allerdings kontrollierte Studien notig.

Abstract: Background. The aim of this study was to investigate the impact of short‐term treatment with the angiotensin II receptor blocker (ARB) valsartan on retinal endothelial function in elderly patients with mild to moderate essential hypertension. Methods. In an open‐labeled study, 20 elderly, male patients with arterial hypertension (WHO I–II) were treated with the ARB valsartan (80–160 mg once daily) over 8 days. Central retinal artery perfusion at rest and during flicker light stimulation was measured before and after treatment using pulsed wave Doppler sonography. Retinal capillary flow was assessed with scanning laser Doppler flowmetry at rest and following systemic infusion of the nitric oxide synthase (NOS) inhibitor NG‐monomethyl‐l‐arginine (L‐NMMA). Results. While valsartan significantly lowered blood pressure, central retinal artery perfusion at rest as well as after flicker light stimulation was similar before and after treatment. Abstract: The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension. The key messages of this report are these: in those older than age 50, systolic blood pressure (BP) of greater than 140 mm Hg is a more important cardiovascular disease (CVD) risk factor than diastolic BP; beginning at 115/75 mm Hg, CVD risk doubles for each increment of 20/10 mm Hg; those who are normotensive at 55 years of age will have a 90% lifetime risk of developing hypertension; prehypertensive individuals (systolic BP 120-139 mm Hg or diastolic BP 80-89 mm Hg) require health-promoting lifestyle modifications to prevent the progressive rise in blood pressure and CVD; for uncomplicated hypertension, thiazide diuretic should be used in drug treatment for most, either alone or combined with drugs from other classes; this report delineates specific high-risk conditions that are compelling indications for the use of other antihypertensive drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, beta-blockers, calcium channel blockers); two or more antihypertensive medications will be required to achieve goal BP (<140/90 mm Hg, or <130/80 mm Hg) for patients with diabetes and chronic kidney disease; for patients whose BP is more than 20 mm Hg above the systolic BP goal or more than 10 mm Hg above the diastolic BP goal, initiation of therapy using two agents, one of which usually will be a thiazide diuretic, should be considered; regardless of therapy or care, hypertension will be controlled only if patients are motivated to stay on their treatment plan. Positive experiences, trust in the clinician, and empathy improve patient motivation and satisfaction. This report serves as a guide, and the committee continues to recognize that the responsible physician's judgment remains paramount.
