Abstract: Background: The financial burden for EU health systems associated with cardiovascular disease has been estimated to be nearly € 110 billion in 2006, corresponding to 10% of total healthcare expenditure across EU or a mean € 223 annual cost per capita. The main purpose of this study is to estimate the costs related to hypertension and the economic impact of increasing adherence to anti-hypertensive therapy in 5 European countries (Italy, Germany, France, Spain and England).Methods: A probabilistic prevalence-based decision tree model was developed to estimate the direct costs of cardiovascular disease related to hypertension (CV defined as: stroke, heart attack, kidney disease, heart failure) in 5 European countries. Our model considered adherence to hypertension treatment as a main driver of blood pressure control (BP Results: The model estimated a total of 8.6 million (1.4 in Italy, 3.3 in Germany, 1.2 in Spain, 1.8 in France and 0.9 in England) CV events related to hypertension over the 10 year time horizon. Increasing the adherence rate to anti-hypertensive therapy to 70% (baseline value is different for each country) would lead to 82,235 fewer CV events (24,058 in Italy, 7,870 in Germany, 18,870 in Spain, 24,855 in France and 6,553 in England). From the NHS perspective, the direct cost associated with hypertension was estimated to be € 51.1 billion (8.1 in Italy, 17.8 in Germany, 12.2 in Spain, 8.8 in France and 4.1 in England). Increasing adherence to anti-hypertensive therapy to 70% would save a total of € 323.7 million (CI 95%: € 304.2 - € 342.2 million) from the NHSs perspective.Conclusion: This study is the first attempt to estimate the economic impact of nonadherence amongst patients with diagnosed hypertension in Europe, using data from five European countries (Italy, France, Germany, Spain and England).

Abstract: Der Blutdruck ist einem zirkadianen Rhythmus unterworfen, der zu einer nachtlichen Absenkung fuhrt. Bei Diabetes, Niereninsuffizienz, linksventrikularer Hypertrophie, Schlafapnoe, Schwangerschaftshypertonie und verschiedenen sekundaren Hypertonieformen kann die nachtliche Absenkung aufgehoben oder in ihr Gegenteil verkehrt sein. Die Diagnostik erfolgt mit Hilfe der 24-h-Blutdruckmessung, die aber in der Praxis noch zu selten fur eine klinische Einschatzung des Patienten und die Therapiesteuerung herangezogen wird. Eine Anpassung der Auswahl und des Einnahmezeitpunkts der antihypertensiven Pharmakotherapie im Hinblick auf den zirkadianen Rhythmus wirkt sich gunstig auf die Blutdruckkontrolle und die kardiovaskulare Morbiditat aus. Das gilt vor allem fur Patienten mit fehlendem nachtlichem Blutdruckabfall oder einem Anstieg, bei denen die abendliche Medikation zu einer Normalisierung des Blutdrucks und der Wiederherstellung der physiologischen zirkadianen Rhythmik beitragen kann. Die vorliegende Arbeit gibt einen aktuellen Uberblick zur Chronopharmakotherapie der arteriellen Hypertonie und gibt praktische Empfehlungen zur Umsetzung dieser Erkenntnisse im klinischen Alltag.

Abstract: AIMS Congestive heart failure (CHF) and impaired renal function are two often co-existing medical conditions and associated with adverse cardiovascular outcome The aim of the current study was to assess renal and intraglomerular haemodynamics by constant infusion input clearance technique in subjects with CHF METHODS AND RESULTS The group of subjects with CHF consisted of 27 individuals with HFpEF and 27 individuals with HFrEF and were compared with 31 healthy controls Subjects underwent renal clearance examination to measure glomerular filtration rate (GFR) and renal blood and plasma flow (RBF and RPF) and to calculate intraglomerular haemodynamics such as resistances of the afferent (RA ) and efferent arterioles (RE ) as well as intraglomerular pressure (Pglom ) Measured GFR was lower in CHF subjects (681 ± 101 mL/min/173 m2 ) compared with controls (836 ± 134 mL/min/173 m2 , Padj < 0001) as was Pglom (Padj < 0001) Total renal vascular resistance (RVR) was higher in CHF subjects (873 ± 201 vs 738 ± 171 dyn × s/cm5 , Padj < 0001) mediated by an increased resistance at the afferent site (3201 ± 1084 vs 2181 ± 796 dyn × s/cm5 , Padj < 0001) Comparing HFpEF and HFrEF subjects, RA was higher in HFrEF subjects The severity of CHF assessed by NT-proBNP revealed an inverse association with renal perfusion (RPF r = -0421, P = 0002, RBF r = -0414, P = 0002) and a positive relation with RVR (r = 0346, P = 0012) at the post-glomerular site (RE : r = 0318, P = 0022) CONCLUSIONS Renal function assessed by measured GFR is reduced and renal vascular resistance at the preglomerular, afferent site is increased in HFpEF and, to greater extent, in HFrEF Our data indicate a close cardiorenal interaction in CHF

Abstract: In patients with type 2 diabetes (T2D) sodium-glucose cotransporter 2 (SGLT-2) inhibitors improve glycaemic control as well as cardiovascular and renal outcomes. Their effects on L-arginine (Arg) related risk markers asymmetric and symmetric dimethylarginine (ADMA and SDMA) and the protective biomarker L-homoarginine (hArg) linking T2D to cardiovascular and renal disease have not yet been reported.Plasma and 24-h urine samples taken before and after 6 weeks of treatment were available from two prospective, randomized, double-blind, placebo-controlled, cross-over trials with empagliflozin (71 patients analyzed, NCT02471963) and dapagliflozin (59 patients analyzed, NCT02383238). In these samples, concentrations of hArg, Arg, ADMA, SDMA, and creatinine were determined by liquid-chromatography coupled to tandem mass-spectrometry. Additionally, intraindividual changes of the biomarkers in plasma were correlated with intraindividual changes of clinical parameters.Treatment with empagliflozin and dapagliflozin was associated with a reduction of plasma hArg by 17.5% and 13.7% (both p < 0.001), respectively, and increase in plasma SDMA concentration of 6.7% and 3.6%, respectively (p < 0.001 and p < 0.05), while plasma Arg and ADMA concentrations were not significantly altered. 24-h urinary excretion of ADMA was reduced by 15.2% after treatment with empagliflozin (p < 0.001) but not after dapagliflozin treatment, while excretion of the other markers was not significantly altered. Renal clearance of SDMA was reduced by 9.1% and 3.9% for both drugs (both p < 0.05). A reduction in ADMA clearance was observable after empagliflozin treatment only (- 15.5%, p < 0.001), but not after dapagliflozin. Renal clearance of hArg and Arg was not significantly altered. Treatment effects on L-arginine related biomarkers were not constantly correlated with effects on glycated hemoglobin, fasting plasma glucose, body mass index, and systolic blood pressure.Treatment with SGLT-2 inhibitors has divergent effects on Arg-related biomarkers and could affect risk estimates associated with these markers. The observed effects are unlikely to explain the known cardiovascular and renal benefits of treatment with empagliflozin or dapagliflozin but still may indicate new therapeutic approaches in patients treated with SGLT-2 inhibitors. Abstract: The National High Blood Pressure Education Program presents the complete Seventh Report of the Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure. Like its predecessors, the purpose is to provide an evidence-based approach to the prevention and management of hypertension. The key messages of this report are these: in those older than age 50, systolic blood pressure (BP) of greater than 140 mm Hg is a more important cardiovascular disease (CVD) risk factor than diastolic BP; beginning at 115/75 mm Hg, CVD risk doubles for each increment of 20/10 mm Hg; those who are normotensive at 55 years of age will have a 90% lifetime risk of developing hypertension; prehypertensive individuals (systolic BP 120-139 mm Hg or diastolic BP 80-89 mm Hg) require health-promoting lifestyle modifications to prevent the progressive rise in blood pressure and CVD; for uncomplicated hypertension, thiazide diuretic should be used in drug treatment for most, either alone or combined with drugs from other classes; this report delineates specific high-risk conditions that are compelling indications for the use of other antihypertensive drug classes (angiotensin-converting enzyme inhibitors, angiotensin-receptor blockers, beta-blockers, calcium channel blockers); two or more antihypertensive medications will be required to achieve goal BP (<140/90 mm Hg, or <130/80 mm Hg) for patients with diabetes and chronic kidney disease; for patients whose BP is more than 20 mm Hg above the systolic BP goal or more than 10 mm Hg above the diastolic BP goal, initiation of therapy using two agents, one of which usually will be a thiazide diuretic, should be considered; regardless of therapy or care, hypertension will be controlled only if patients are motivated to stay on their treatment plan. Positive experiences, trust in the clinician, and empathy improve patient motivation and satisfaction. This report serves as a guide, and the committee continues to recognize that the responsible physician's judgment remains paramount.
