Abstract: A typical telemedicine system involves a small set of hospitals providing remote healthcare services to a small section of the society using dedicated nodal centers However, in developing nations like India where majority live in rural areas that lack specialist care, we envision the need for much larger Internet-based telemedicine systems that would enable a large pool of doctors and hospitals to collectively provide healthcare services to entire populations We propose a scalable, Internet-based P2P architecture for telemedicine integrating multiple hospitals, mobile medical specialists, and rural mobile units This system, based on the store and forward model, features a distributed context-aware scheduler for providing timely and location-aware telemedicine services Other features like zone-based overlay structure and persistent object space abstraction make the system efficient and easy to use Lastly, the system uses the existing internet infrastructure and supports mobility at doctor and patient ends

Abstract: A desalination system using a spray tower and Vapour Compression Refrigeration (VCR) system is proposed for obtaining fresh water from brackish water. In the spray tower, simultaneous heat and mass transfer take place between the brackish water and air, which results in the evaporation of the brackish water and humidification of the air. Fresh water is obtained from the humidified air by condensing the water vapour using a VCR system. Parametric studies were carried out to study the effect of various operational parameters on the fresh water production rate.

Abstract: Cloud resources like Amazon EC2 and Google Compute Engine have become an integral part of our life with their pay-per-use pricing models. On the other hand, attempts have also been made to create virtual clouds on the fly using available devices in the locality like smartphones and tablets. Such devices being equipped with powerful processors, this is a novel method of utilizing their normally unused computational capabilities. We attempt to design and develop a pricing model for such resources created for opportunistic computing. Since the values of the resources in these virtual clouds cannot be calculated, the pay-per-resource model has to give way to a pay-per-effort model. We model the scenario as a principal-agent problem with moral hazards and propose an incentive-driven Quality of Service (QoS) to ensure that the agents act appropriately. In view of the short contractual duration, we consider only deterministic contracts which are easy to understand and implement. We show that the mobile devices can be modeled as economic actors in a contractual arrangement and discuss how the optimal decisions are identified.

Abstract: An obfuscated policy data encryption system and method for re-encrypting data to maintain the confidentiality and integrity of data about a user when the data is stored in a public cloud computing environment. The system and method allow a user to specify in a data-sharing policy who can obtain the data and how much of the data is available to them. This policy is obfuscated such that it is unintelligible to the cloud operator and others processing and storing the data. In some embodiments, a patient species with whom his health care data should be shared with and the encrypted health care data is stored in the cloud in an electronic medical records system. The obfuscated policy allows the electronic medial records system to dispense the health care data of the patient to those requesting the data without disclosing the details of the policy itself.

Abstract: An emerging breakthrough paradigm shift in health industry and wearable devices, large scale and distributed mobile cloud computing technologies have led to new opportunities for medical healthcare systems. Telemedicine service selection and management of Medical-Quality of Service (m-QoS) in large-scale and distributed medical health system (e.g. medical data centers, hospitals, medical servers and medical clouds, etc.) is a key challenge for both industry and academia. The aim of this chapter is to improve and manage m-QoS by prioritizing Telemedicine service by using decisive and intelligent tool called Analytic Hierarchy Process (AHP). This service will be provided on urgency basis from the pool of medical services with the help of AHP. In this connection, four telemedicine services are considered i.e. Tele-surgery, Tele-Consultation, Tele-Education and Tele-Monitoring. In this research, three m-QoS parameters are considered i.e. throughput, jitter and delay. These services are evaluated by potential doctors and patients. We propose an AHP based decision making algorithm for selecting urgent and important service for the fast and cost-effective treatment of the emergency patients at the remote location in the hospital, because AHP is the significantly fast decision making technique used to assess, select and manage the emergency services at various priority levels in large scale and distributed medical health systems. The comprehensive purpose is indicated in the first level of the strategy. The decisive entities are presented in the intermediate level and the target-based alternatives are located at the lowest level. MATLAB is used for experimental results to measure and evaluate goal, decision making parameters and options from both qualitative and quantitative aspect. The proposed AHP algorithm is simulated for three decision parameters and four different Telemedicine services in which highest priority is given to decision parameter, throughput and Telemedicine service, Tele-Surgery for large scale and distributed medical health systems.

Abstract: Over the last two decades, a public rationale for the implementation of telehealth has emerged at the interplay of specialised literature and political orientations. Despite the lack of consistent findings on the magnitude of its benefits, telehealth is nowadays presented as a worthy solution both for patients and healthcare institutions. Far from denying the potential advantages of telehealth, the main objective of this work is to provide a critical assessment on the spread of the remote services as a vector of positive transformation of contemporary health systems. For pursuing this objective, the EU agenda for the promotion of telehealth will be retraced, and the main evidences alleged to sustain the implementation of remote care services will be assessed. Furthermore, it will be evaluated the attempt made by the European Commission to establish an ethical framework for guiding the use of telehealth in daily practice, and a roadmap of the most relevant legal and ethical issues posed by the spread of telehealth will be traced. In the conclusions, it will be argued that the radical transformations induced by this form of technological innovation call on to a new, ad hoc ethics through which critically evaluate benefits and implications of telehealth services, with a view to keep high the standard of healthcare against the economic interests of private stakeholders and ICTs' vendors.

Abstract: The present investigation was focused on the removal of dissolved organics and suspended solids from tannery saline wastewater [TSWW]Raw, known as soak liquor, by sequential oxic-anoxic bio reactor (SOAR) using halophilic bacteria 3and filamentous bacteria (FB) The high concentration of Sodium Chloride (NaCl) in tannery wastewater retards biodegradation of dissolved organics The aim of this study was to cultivate halophilic bacteria and facultative bacteria from tannery soil supported sewage culture using soak liquor nutrient, for the degradation of organics in [TSWW]Raw SOAR was used for the maximum removal of proteins, 474%; carbohydrates, 448% and lipids, 704% from [TSWW]Raw at optimum HRT of 12 h The Chemical Oxygen Demand (COD), Biochemical Oxygen Demand (BOD) and suspended solids were removed by 368%, 312% and 726% respectively from [TSWW]Raw containing total dissolved solids (TDS) concentration of 6–7% (w/v) without the addition of any coagulants The degradation of proteins (albumin and globulin) in [TSWW]Raw was evaluated and confirmed through circular dichroism (CD) studies Further, the removal of organic compounds was characterized by UV–vis spectroscopy and fluorescence spectroscopy, TGA-DSC and FT-IR studies The presence of filamentous bacteria was confirmed through SEM analysis
