Abstract: L’auteur propose une reflexion sur l’elaboration d’affects de haine fraternelle, suite a un traumatisme infantile precoce chez une patiente. Ces affects, lies a la relation avec un frere psychiquement atteint, seront revisites au cours de la cure. Des vecus repetes de meurtre, de rage et de destructivite seront elabores ; la compulsion de repetition a l’œuvre se transformant progressivement en capacite d’autoreflexion et d’elaboration. La haine laissera ainsi progressivement la place a une rivalite plus tolerable et constructrice des auto-erotismes.

Abstract: Objectifs : Cette recherche vise une meilleure comprehension de la construction identitaire des freres et soeurs de personnes autistes. Elle porte sur l'evolution de la representation de soi de ces membres de fratries a l'adolescence, et l'analyse est centree sur l'image du corps, l'investissement narcissico-objectal et le processus de subjectivation. Une revue theorique et clinique des travaux francophones et anglo-saxons met a jour l'etat des connaissances dans ce domaine clinique encore peu etudie. Methode : Nous procedons par approche comparative en constituant un groupe temoin de fratries sans condition de handicap en parallele au groupe d'etude clinique de fratries avec autisme, chacun constitue de 24 sujets âges de 13 a 18 ans. Nous adoptons une methode clinique et projective en utilisant lentretien clinique, deux tests projectifs (Rorschach et TAT), une activite d'ecriture originale creee pour la recherche, deux echelles standardisees (R-CMAS : anxiete ; SEI : estime de soi). Des analyses statistiques sont proposees en complement de l'analyse des donnees cliniques. Resultats : Nous degageons une specificite des caracteristiques de la representation de soi chez les adolescents de fratrie avec autisme qui se traduit a un triple niveau : celui de l'integration psychique du corps en transformation ; celui de l'engagement en direction de l'autre sexue ; celui de l'elaboration de projets d'avenir personnels. Une constellation de traits particuliers apparait plus ou moins fortement selon les sujets et nous amene a proposer des perspectives preventives et therapeutiques tenant compte de la variabilite des fonctionnements psychiques individuels.

Abstract: Resume But de l’etude Les auteurs presentent des donnees de recherche clinique sur la facon dont fonctionne l’identite d’adolescents ayant un frere ou une sœur autiste. Une revue de litterature de travaux francais et nord-americains permet d’eclairer les enjeux psychiques de jeunes typiques se developpant dans un environnement atypique. Cette recherche veut explorer quelles fragilites psychologiques peuvent etre induites par le contexte de vie de ces adolescents. Methode Utilisant deux echelles cliniques thematiques, deux outils cliniques (entretien avec adolescents et avec parents) et deux tests projectifs de personnalite, l’observation consiste en une exploration des fondements de l’identite : l’estime de soi ; l’anxiete ; la representation de soi ; les assises narcissiques ; les images du corps, et methode comparative : groupe d’etude (adolescents de fratries ayant un membre autiste) versus groupe temoin (adolescents typiques apparies) ; analyses statistiques et analyses qualitatives des donnees cliniques. Resultats La representation de soi apparait nettement alteree chez les adolescents du groupe d’etude, ses differentes composantes sont fragilisees et generent des desadaptations sensibles. Le repli relationnel tend a dominer des que le sujet se trouve face a un enjeu affectif. Pistes de recherche et propositions preventives sont enoncees.

Abstract: Summary The present work focuses on the transformations of the psychotherapeutic field through the relationship dynamics which occur within it. The first part of this article starts with a brief outline of the Gestalt psychological understanding of the field concept, also in its application to the psychotherapeutic situation, followed by a brief review of the introduction of the field concept into the psychoanalytic theory formation. After this, the first author first presents the theoretical concept underlying a new approach he has developed for observing the relationship dynamics in psychotherapy. Mirroring a formation of both psychoanalytic and Gestalt theory of the main author, this new approach is based on the combination of psychoanalytic and Gestalt psychological concepts. According to the clinical experience and insights of the author, the phenomenological and relational approach of Gestalt theory fits well with the psychoanalytic approach; on this basis, a criterion for recording the progress of therapy can be developed. This criterion is the phenomenology of the development of the qualities of the relationships of the client, as they become visible in his dream narrations and the subsequent associations in the analysis room and continue to develop during the session and the further course of therapy. The relationship dynamics in the dream narration is thus compared with those which develop in the course of the subsequent associations. This is demonstrated and further elaborated in the second part of this article on the basis of a clinical case. The clinical example shows how the relationship dynamics develop in this sense in the individual therapy sessions and over a longer course of therapy. The associated transformations of the therapeutic field give a good indication of the progress of therapy. The main author gained such insights into the transformations of the therapeutic field and the progression of therapy, which are visible in the course of therapy, from the careful application of the criterion “MDAC of relational dynamics”. In the specific case, there was also a high degree of correspondence between the results of the application of this phenomenological criterion and the empirical evidence of the symptom questionnaire, a self-report measure requested by the patient himself during the course of the therapy.

Abstract: L’articolo affronta da un punto di vista clinico alcune questioni affettive della relazione tra genitori e figli adolescenti. Cogliendo le dinamiche familiari dalla prospettiva della mente del figlio e dei cambiamenti emotivi cui egli va incontro con l’ingresso nell’adolescenza, l’attenzione si sofferma in particolare sul ruolo delicato del genitore nell’espletare determinate funzioni affettive nei confronti del figlio cruciali per il superamento da parte di quest’ultimo degli ostacoli che l’adolescenza pone alla sua crescita. In quest’ottica alcuni riferimenti al contributo di autori e a teorie del campo psicodinamico sono considerati in rapporto ai temi sia della funzione genitoriale, sia dello sviluppo emotivo in adolescenza e delle dinamiche genitori-figli in questa fase del ciclo di vita familiare.
