Abstract: Het (chronisch) gebruik van antipsychotica bij bewoners in Belgische woonzorgcentra (WZC) ligt hoog. In het kader van het PHEBE-project vonden Azermai et al. dat ongeveer een op de drie bewoners (33%), ongeacht de aanwezigheid van dementie, een antipsychoticum toegediend kreeg.1 Bij 93% van hen was dit gebruik chronisch (= langer dan drie maanden). Ook uit cross-sectioneel onderzoek in Brugse woonzorgcentra bleek 29% van de bewoners antipsychotica te gebruiken.2

Abstract: Nitrofurantoine werd in 1953 goedgekeurd door de FDA, maar met de komst van cotrimoxazole en betalactamantibiotica verloor het product aan populariteit. Na 2000 werd het terug meer en meer gepromoot wegens de resistentieproblematiek. Op dit moment is het gebruik van nitrofurantoine in de meeste landen sterk gestegen, met uitzondering van Frankrijk waar het geneesmiddelenagentschap waarschuwde voor mogelijke zeer ernstige bijwerkingen.

Abstract: Klinische vraagWat is het effect van paracetamol en ibuprofen in monotherapie versus alternerende toediening van beide middelen bij kinderen tot drie jaar met koorts?AchtergrondKoorts leidt vaak tot irritabiliteit bij het kind en ongerust-heid bij de ouders. Koortswerende middelen, zoals para-cetamol en ibuprofen, worden bijna systematisch toege-diend. In de praktijk worden beide ook gecombineerd gebruikt. Daarover zijn weinig studies beschikbaar.Bestudeerde populatieDrie Israelische pediatrische eerstelijnsgezondheidscen-tra rekruteerden crechekinderen tussen 6 en 36 maanden oud met koorts >38,5°C (rectaal gemeten). Exclusie-criteria waren: gebruik van koortswerende medicatie of antibiotica tijdens de voorbije tien dagen, afwijkende lever- en niertesten, gastro-intestinale bloeding in de voorgeschiedenis, allergie voor antipyretica en astma. Uiteindelijk werden 480 kinderen met een gemiddelde leeftijd van 19 maanden (SD 9) geincludeerd. De dia-gnose was bij ongeveer de helft ‘bovenste luchtweginfec-tie’ en bij ongeveer 30% ‘virale infectie’. De gemiddelde koorts bij inclusie bedroeg ongeveer 40,65°C.OnderzoeksopzetIn een gecontroleerde, gerandomiseerde en dubbel-blinde studie werden de kinderen verdeeld in drie groepen. Een groep kreeg 12,5 mg/kg/dosis paraceta-mol om de zes uur (n=160). Een tweede groep kreeg 5 mg/kg/dosis ibuprofen om de acht uur (n=160). De derde groep kreeg om de vier uur 12,5 mg/kg/dosis paracetamol afgewisseld met 5 mg/kg/dosis ibuprofen (n=160). In elke studiegroep kreeg de helft van de kin-deren een initiele ladingsdosis van 25 mg/kg parace-tamol. De andere helft kreeg een initiele ladingsdosis van 10 mg/kg ibuprofen. Aan de ouders werd gevraagd om minstens driemaal per dag de rectale temperatuur te meten met een kwikthermometer. Rectale tempera-tuur, medicatiegebruik, aantal dagen werkverlet en NCCPC stressscore werden door de ouders in een dagboek bijgehouden. Verder bestond de follow-up uit een telefonisch interview na 24 en na 48 uur, en ver-volgconsulten op de dagen 3, 5 en 7. Lever- en nier-functieparameters en occult fecaal bloedverlies werden onderzocht bij de start, op de dagen 3 en 5, en nadien tweewekelijks gedurende in totaal twaalf weken.UitkomstmetingDe primaire uitkomstmaat was de hoogst gemeten lichaamstemperatuur op de dagen 1, 2 en 3. Andere uitkomstmaten waren: de NCCPC stressscore, de hoe-veelheid ingenomen koortswerende medicatie, het aantal dagen werkverlet van de ouders, herval van koorts, het aantal consulten op spoedopname binnen de tien dagen na inclusie in de studie, lever- en nierfunctieparameters, en het optreden van gastrointestinaal bloedverlies.ResultatenVan de 480 geincludeerde peuters beeindigden er 466 (96,7%) de studie. Op zowel dag 1, dag 2 als dag 3 was in de groep die beide medicijnen alternerend kreeg, de koorts significant lager dan in de groepen die een van beide kregen (p<0,001). Er was geen verschil in koorts-daling tussen de paracetamol- en de ibuprofengroep. Het verschil in koorts op dag 1 tussen de combinatie-groep versus de monotherapiegroepen bedroeg onge-veer 1°C (39,6° versus 40,6°). De stressscore daalde dag na dag in alle groepen, maar significant sneller en meer uitgesproken in de groep die beide medicijnen nam (p<0,001). Die groep kreeg significant minder dosissen toegediend (p<0,001) en de ouders waren bijna een dag (1,7 versus 2,6) minder lang afwezig op het werk. Er waren geen significante verschillen in aantal spoedop-names, er werden geen ernstige nevenwerkingen gerap-porteerd en tussen de groepen waren er geen verschillen in lever- en nierfunctieparameters. Het medicijn voor de ladingsdosis had geen invloed op de uitkomstmaten.Conclusie van de auteursDe auteurs besluiten dat bij kinderen een alternerend gebruik van paracetamol (12,5 mg/kd/dosis) en ibu-profen (5 mg/kg/dosis) om de vier uur gedurende drie dagen resulteert in een snellere daling van de koorts.FinancieringNiet vermeldBelangenvermengingNiet vermeld
