Abstract: A surgical stapler is provided which is insertable through an endoscopic tube to enable a surgeon to staple a hernia patch to tissue inside a body cavity. The endoscopic surgical stapler includes a staple cartridge pivotally mounted on the distal end of a support tube extending from a handle provided with an actuator mechanism for actuating a staple forming mechanism inside the staple cartridge to fasten staples to the tissue. The staple cartridge is adjustable to different angular orientations relative to the support tube. The staple actuator mechanism is operable to actuate the staple cartridge in any of the angular orientations. The support shaft is rotatable about its longitudinal axis relative to the handle to adjust the rotational orientation of the support shaft and the staple cartridge. The staple cartridge is rotatable about its longitudinal axis relative to the support shaft to adjust the rotational position of the staple cartridge relative to the support shaft. Separate actuator mechanisms are provided on the handle to control the pivotal movement of the staple cartridge, the rotation of the support shaft, and the rotation of the staple cartridge relative to the support shaft.

Abstract: A surgical instrument is provided which is insertable through an endoscopic tube to enable a surgeon to perform a surgical technique inside a body cavity. The surgical instrument includes a shaft with a pivotally mounted tip attached thereto, the shaft extending from a handle for gripping the instrument for insertion to the body cavity. The tip of the shaft is adjustable to different angular orientations relative to the support shaft and is rotatable at any angular orientation of the tip. The support shaft may be rotatable about its longitudinal axis relative to the handle, or it may be fixed with respect to the handle, in which case the handle may be rotated to rotate the shaft inside the body cavity. In one embodiment, the surgical instrument comprises an endoscopic stapling device and the tip of the instrument comprises a staple cartridge which is rotatable 360° about its axes after the cartridge has been articulated with respect to the support shaft. An improved device for articulating the shaft tip is also provided in which a control knob is mounted to the support shaft for providing sweeping articulation of the shaft tip. Separate actuator mechanisms are provided on the handle to control the pivotal movement of the shaft tip, as well as the rotation of the shaft tip and the rotation of the support shaft.

A joy stick mounted to the shaft of the instrument near the handle allows for remote articulation of the staple head assembly. The staple head assembly includes a narrow contour for insertion between adjacent rib pairs without rib spreading. The staple head assembly further includes a reloadable staple cartridge which may be replaced with differently sized staples of a second cartridge. A closure trigger clamps tissue prior to stapling and cooperates with a second trigger that performs the staple firing operation. The triggers include a safety lock mechanism for preventing premature staple firing.

Abstract: An improved surgical stapling instrument is provided for applying surgical staples to human tissue. The instrument has an articulated stapling head assembly mounted by a rotatable and flexible support shaft on an actuator handle assembly with an improved actuator mechanism for closing and firing the stapling head assembly. The stapling head assembly comprises a fixed jaw with a staple cartridge and a movable jaw with an anvil for clamping the tissue therebetween. The actuator mechanism comprises a jaw closure lever for closing the jaws and a staple firing lever for actuating a staple driver to drive the staples from the staple cartridge into the tissue and against the anvil. The actuator mechanism also includes a cam pulley mounted for rotation by the staple firing lever and secured to a firing cable for actuating the staple driver. A set of drive gears rotatably positioned on opposite sides of the cam pulley mesh with gear sectors on the staple firing lever to rotate the cam pulley and apply tension to the firing cable when the staple firing lever is actuated. The cam pulley has a contoured cam lobe for actuating the firing cable with different mechanical advantages as the pulley is rotated. An anti-backup member engages the cam pulley to prevent the pay-out of the firing cable before and after the staple firing lever is actuated. The length of the support shaft can be adjusted to set the tension in the closure cable so that the jaws are completely closed by the jaw closure lever to produce staples of uniform height when the staple firing lever is actuated.

Abstract: A surgical instrument useful for laparoscopic surgery comprising an insulated shaft, a control means and a connection to an electrosurgical radiofrequency power source, its distal end being of general J-hook form having a tissue-engaging ledge, and the shaft supporting a tissue-engaging clamping or cutting member movable toward the ledge in response to the control means so that an artery or other tissue in the pocket of the J-hook can be clamped or cut while radiofrequency current is applied. Instrument constructions disclosed incorporate one or two clamping members and a cutting member arranged such that with actuation, the clamping member(s) engages the ledge before the cutting member, and, by selective actuation of the electrosurgical power source, coagulation current can be applied while clamping an artery or other tissue against the ledge to stop flow of blood, and by further actuation of the control and suitable current to the cutting member, the artery or tissue may be cut. Selector means enable movement of one and prevent movement of one of the members during actuation; bipolar or monopolar energization is employed; the relatively long leg of the J-member has portals. Communicating through a lumen in the shaft for infusion or aspiration of fluid; the distal end surface of the J-hook defines a cutting edge for blunt dissection; and the control means comprises a single actuatable member by actuator rods for sealing clamp and cutting members, while a compression spring associated with the actuating rod of the sealing clamp member compresses when the clamp member is stopped, in clamping action upon the artery or tissue while cutting of tissue thereafter proceeds. A diamond film on tissue-engaging surfaces of a clamp, grasping or cutting surgical instrument improves heat transfer between tissue and instrument.

Abstract: A surgical stapler instrument is provided for applying lateral lines of staples to tissue while cutting the tissue between those staple lines. The instrument includes a handle portion, an implement portion, a reciprocating section, a drive member and a movable actuator. The implement portion includes a staple cartridge and an anvil. The reciprocating section is adapted to move back and forth along an axis of the implement portion. The movable actuator is associated with the handle portion and is engaged with the drive member such that motion of the actuator causes the drive member to move back and forth between first and second drive positions separated by a first distance. A multiplier is further provided and is associated with the reciprocating section and the drive member for causing the reciprocating section to move back and forth between first and second reciprocating positions in response to movement of the drive member. The reciprocating section includes a work portion which, when moved distally, effects the firing of staples in the staple cartridge toward the anvil. The work portion is also provided with a reciprocating knife. The first and second reciprocating positions are separated by a second distance which differs from the first distance.

The stapling device includes an approximation mechanism for moving a cartridge assembly and an anvil assembly between spaced and approximated positions and a firing mechanism for ejecting the array of staples from the cartridge assembly. A single trigger is operable to effect approximation and firing of the device. The device also includes an alignment pin assembly which can be selectively manually or automatically advanced. The anvil assembly includes a stiffener plate which allows the device to have a reduced head portion profile.
