Abstract: The Fifth Eriksholm Workshop on “Hearing Impairment and Cognitive Energy” was convened to develop a consensus among interdisciplinary experts about what is known on the topic, gaps in knowledge, the use of terminology, priorities for future research, and implications for practice. The general term cognitive energy was chosen to facilitate the broadest possible discussion of the topic. It goes back to Titchener (1908) who described the effects of attention on perception; he used the term psychic energy for the notion that limited mental resources can be flexibly allocated among perceptual and mental activities. The workshop focused on three main areas: (1) theories, models, concepts, definitions, and frameworks; (2) methods and measures; and (3) knowledge translation. We defined effort as the deliberate allocation of mental resources to overcome obstacles in goal pursuit when carrying out a task, with listening effort applying more specifically when tasks involve listening. We adapted Kahneman’s seminal (1973) Capacity Model of Attention to listening and proposed a heuristically useful Framework for Understanding Effortful Listening (FUEL). Our FUEL incorporates the well-known relationship between cognitive demand and the supply of cognitive capacity that is the foundation of cognitive theories of attention. Our FUEL also incorporates a motivation dimension based on complementary theories of motivational intensity, adaptive gain control, and optimal performance, fatigue, and pleasure. Using a three-dimensional illustration, we highlight how listening effort depends not only on hearing difficulties and task demands but also on the listener’s motivation to expend mental effort in the challenging situations of everyday life.

Abstract: Working memory is important for online language processing during conversation. We use it to maintain relevant information, to inhibit or ignore irrelevant information, and to attend to conversation selectively. Working memory helps us to keep track of and actively participate in conversation, including taking turns and following the gist. This paper examines the Ease of Language Understanding model (i.e., the ELU model, Ronnberg, 2003; Ronnberg et al., 2008) in light of new behavioral and neural findings concerning the role of working memory capacity (WMC) in uni-modal and bimodal language processing. The new ELU model is a meaning prediction system that depends on phonological and semantic interactions in rapid implicit and slower explicit processing mechanisms that both depend on WMC albeit in different ways. It is based on findings that address the relationship between WMC and (a) early attention processes in listening to speech, (b) signal processing in hearing aids and its effects on short-term memory, (c) inhibition of speech maskers and its effect on episodic long-term memory, (d) the effects of hearing impairment on episodic and semantic long-term memory, and finally, (e) listening effort. New predictions and clinical implications are outlined. Comparisons with other WMC and speech perception models are made.

Abstract: A general working memory system for ease of language understanding (ELU, Ronnberg, 2003a) is presented. The purpose of the system is to describe and predict the dynamic interplay between explicit and implicit cognitive functions, especially in conditions of poorly perceived or poorly specified linguistic signals. In relation to speech understanding, the system based on (1) the quality and precision of phonological representations in long-term memory, (2) phonologically mediated lexical access speed, and (3) explicit, storage, and processing resources. If there is a mismatch between phonological information extracted from the speech signal and the phonological information represented in long-term memory, the system is assumed to produce a mismatch signal that invokes explicit processing resources. In the present paper, we focus on four aspects of the model which have led to the current, updated version: the language generality assumption; the mismatch assumption; chronological age; and the episodic buffer function of rapid, automatic multimodal binding of phonology (RAMBPHO). We evaluate the language generality assumption in relation to sign language and speech, and the mismatch assumption in relation to signal processing in hearing aids. Further, we discuss the effects of chronological age and the implications of RAMBPHO.

Abstract: Two experiments were conducted to investigate possible relationships between cognitive function and hearing aid use. In Experiment 1. 72 first-time hearing aid users were tested for speech recognition in noise (Hagerman sentence test) with and without hearing aids. Cognitive function was assessed by tests of working memory (reading span test) and verbal information-processing speed. The results indicate that. after controlling for age and hearing loss, significant correlations exist between the measures of cognitive performance and speech recognition in noise, both with and without hearing aids. High cognitive performance was associated with high performance in the speech recognition task. In Experiment 2, 17 first-time hearing aid users with either high or low working-memory capacity tested ail experimental hearing aid which processed the sound differently depending on whether or not speech was detected. The results revealed that those with high working-memory capacity were better than those with low capacity at identifying and reporting the specific processing effects of the aid. This may have implications for how reported results should be interpreted in a research context, how a person's rehabilitation needs are formulated, and how hearing aid controls should be supervised. In conclusion, careful attention should be paid to the cognitive status of listeners, as it can have a significant influence on their ability to utilize their hearing aids.

Abstract: This study which included 23 experienced hearing aid users replicated several of the experiments reported in Gatehouse et al (2003, 2006) with new speech test material, language, and test procedure. The performance measure used was SNR required for 80% correct words in a sentence test. Consistent with Gatehouse et al, this study indicated that subjects showing a low score in a cognitive test (visual letter monitoring) performed better in the speech recognition test with slow time constants than with fast time constants, and performed better in unmodulated noise than in modulated noise, while subjects with high scores on the cognitive test showed the opposite pattern. Furthermore, cognitive test scores were significantly correlated with the differential advantage of fast-acting versus slow-acting compression in conditions of modulated noise. The pure tone average threshold explained 30% of the variance in aided speech recognition in noise under relatively simple listening conditions, while cognitive test scores explained about 40% of the variance under more complex, fluctuating listening conditions, where the pure tone average explained less than 5% of the variance. This suggests that speech recognition under steady-state noise conditions may underestimate the role of cognition in real-life listening.

Abstract: In the development process of noise-reduction algorithms, an objective machine-driven intelligibility measure which shows high correlation with speech intelligibility is of great interest. Besides reducing time and costs compared to real listening experiments, an objective intelligibility measure could also help provide answers on how to improve the intelligibility of noisy unprocessed speech. In this paper, a short-time objective intelligibility measure (STOI) is presented, which shows high correlation with the intelligibility of noisy and time-frequency weighted noisy speech (e.g., resulting from noise reduction) of three different listening experiments. In general, STOI showed better correlation with speech intelligibility compared to five other reference objective intelligibility models. In contrast to other conventional intelligibility models which tend to rely on global statistics across entire sentences, STOI is based on shorter time segments (386 ms). Experiments indeed show that it is beneficial to take segment lengths of this order into account. In addition, a free Matlab implementation is provided.
