Abstract: Background One common model utilized to understand clinical staff and patients' technology adoption is the technology acceptance model (TAM). Objective This article reviews published research on TAM use in health information systems development and implementation with regard to application areas and model extensions after its initial introduction. Method An electronic literature search supplemented by citation searching was conducted on February 2017 of the Web of Science, PubMed, and Scopus databases, yielding a total of 492 references. Upon eliminating duplicates and applying inclusion and exclusion criteria, 134 articles were retained. These articles were appraised and divided into three categories according to research topic: studies using the original TAM, studies using an extended TAM, and acceptance model comparisons including the TAM. Results The review identified three main information and communication technology (ICT) application areas for the TAM in health services: telemedicine, electronic health records, and mobile applications. The original TAM was found to have been extended to fit dynamic health service environments by integration of components from theoretical frameworks such as the theory of planned behavior and unified theory of acceptance and use of technology, as well as by adding variables in specific contextual settings. These variables frequently reflected the concepts subjective norm and self-efficacy, but also compatibility, experience, training, anxiety, habit, and facilitators were considered. Conclusion Telemedicine applications were between 1999 and 2017, the ICT application area most frequently studied using the TAM, implying that acceptance of this technology was a major challenge when exploiting ICT to develop health service organizations during this period. A majority of the reviewed articles reported extensions of the original TAM, suggesting that no optimal TAM version for use in health services has been established. Although the review results indicate a continuous progress, there are still areas that can be expanded and improved to increase the predictive performance of the TAM.

Abstract: Beslenme desteği kritik yetişkin hastaların bakımında önemlidir. Fakat kritik hasta için uygun kalori hedefi net değildir. Hedeflenen kaloriye ulaşılması ile protein katabolizması ve malnutrisyonun iyileştirilmesi ve bununla ilişkili morbidite ve mortalitede azalma amaçlanmaktadır. Farklı kalori miktarına sahip enteral beslenme alımını inceleyen gözlemsel çalışmalar çelişkili sonuçlar ortaya koymuştur. Kritik hastalarda rutin enteral beslenme ile yüksek kalorili enteral beslenmeyi karşılaştıran iki randomize kontrollü çalışma (RKÇ) yüksek kalorili enteral beslenme ile mortalitede bir azalma göstermemiştir. Akut akciğer hasarı veya akut solunum yetmezlikli kritik hastalarda yapılan iki RKÇ mimimum ya da trofik enteral beslenme ile (hedeflenen kalorinin %15-25) standart enteral beslenmede (SEB) benzer sonuçlar göstermiştir. Mevcut kanıtlar erken kritik hastalık sırasında eş zamanlı kalori alımına bakılmaksızın belli bir düzeyde protein alımını desteklemesine rağmen protein dışı enerji kısıtlaması yapılan hastalarda protein ihtiyacının tamamının yerine konmasının sonuçlar üzerine etkisi bilinmemektedir. Sıçanlarda yapılan bir çalışma, glukoz verilmeden protein ile beslenmenin malnutrisyon ile azalmış olan mitokondriyal fonksiyonlarda düzelme sağladığını göstermiştir. Bu nedenle diyetle yeterli protein alımı sağlanırsa kalori kısıtlamasının yararlı olabileceği öne sürülmüştür. Bu çalışmanın hipotezi protein alımı korunurken protein dışı kalori kısıtlaması (permisif hipokalorik beslenme) yapılan kritik erişkin hastalarda, standart beslenme alan hastalara göre 90 günlük sağ kalımın daha iyi olduğunun gösterilmesidir.

Abstract: Background and objectives The coronavirus disease 2019 (COVID-19) pandemic is experienced differently across individuals, and older adults' different life experiences lead to a variety of ways of coping. The present study explores older adults' reports of what about the pandemic is stressful, and what brings joy and comfort in the midst of stress. Research design and methods An online survey asked 825 U.S. adults aged 60 and older to complete questionnaires assessing 3 psychological well-being indicators: perceived stress, negative affect, and positive affect. Participants also responded to open-ended questions about what was stressful and what brought joy or comfort at the time of the survey. A mixed-method approach first qualitatively analyzed the open-ended responses, content analysis identified themes most frequently reported, and quantitative analysis examined the associations between various stressors and joys and the psychological well-being indicators. Results Qualitative analysis revealed 20 stress categories and 21 joy/comfort categories. The most commonly reported stressors were confinement/restrictions, concern for others, and isolation/loneliness; the most commonly reported sources of joy/comfort were family/friend relationships, digital social contact, and hobbies. Demographic comparisons revealed variations in experience. Independent t tests revealed stress from concern for others, the unknown future, and contracting the virus to be significantly associated with poorer psychological well-being; faith, exercise/self-care, and nature were associated with more positive psychological well-being. Discussion and implications Results are discussed in the context of stress and coping theory, highlighting the importance of understanding the unique stress experience of each individual for effective distress intervention.
