Community engagement is increasingly recognized as a critical element of medical research, recommended by ethicists, required by research funders and advocated in ethics guidelines. The benefits of community engagement are often stressed in instrumental terms, particularly with regard to promoting recruitment and retention in studies. Less emphasis has been placed on the value of community engagement with regard to ethical good practice, with goals often implied rather than clearly articulated. This article outlines explicitly how community engagement can contribute to ethical global health research by complementing existing established requirements such as informed consent and independent ethics review. The overarching and interlinked areas are (1) respecting individuals, communities and stakeholders; (2) building trust and social relationships; (3) determining appropriate benefits; minimizing risks, burdens and exploitation; (4) supporting the consent process; (5) understanding vulnerabilities and researcher obligations; (6) gaining permissions, approvals and building legitimacy and (7) achieving recruitment and retention targets. read more read less

There is a growing body of literature describing conceptual frameworks for working with participatory visual methods (PVM). Through a global health lens, this paper examines some key themes within these frameworks. We reflect on our experiences of working with with an array of PVM to engage community members in Vietnam, Kenya, the Philippines and South Africa in biomedical research and public health. The participants that we have engaged in these processes live in under-resourced areas with high prevalence of communicable and non-communicable diseases. Our paper describes some of the challenges that we have encountered while using PVM to foster knowledge exchange, build relationships and facilitate change among individuals and families, community members, health workers, biomedical scientists and researchers. We consider multiple ethical situations that have arisen through our work and discuss the ways in which we have navigated and negotiated them. We offer our reflections and learning from facilitating these processes and in doing so we add novel contributions to ethical framework concepts. read more read less

The economy has not been able to provide employment and an income for the vast majority of rural migrants, including the urban poor. Significantly, a large number of urban poor work in the informal sector as they have less skill, less education and less capital. In many developing countries, including India, due to the fast pace of urbanization, many rural areas are gradually becoming part of urban areas, where urban amenities are lacking and open spaces, cultivable lands and water bodies remain, at least in the initial stages of urbanization. Due to growing environmental concerns, there is often a demand to preserve such open spaces to protect the environment. This provides an opportunity for some people to earn a livelihood by following traditional occupations like agriculture and thereby providing fresh vegetables for the urban population. However, these people do not enjoy some of the urban amenities and continue to be connected to the... read more read less
