Abstract: The oxidation of organic hydrocarbons by oxygen atom transfer from metal oxo complexes has been well studied, and numerous methods of epoxidation by oxo transfer are routinely utilized in organic synthesis. 2] In contrast to oxygen atom transfer, methods for transferring nitrogen atoms to organic substrates are not as well-developed. Such methods would be of great utility owing to the value of the anticipated end products (e.g. aziridines) and the potential for using N2 as the nitrogen atom source. [4]

TL;DR: This case study demonstrates an effective incident investigation approach to identify system‐related root causes and helps the team better understand the timing and conditions of the event, better understand why people make the decisions they make at the time the event unfolds, and ensures that systemic root causes are discovered.
Abstract: This case study demonstrates an effective incident investigation approach to identify system-related root causes. The premise is simple: “What people do makes sense to them at the time.” The approach combines human error concepts and human factors analyses. The combined approach helps define and eliminate “hindsight bias” (the investigator's bias that exists because of the known bad consequence). The case study's human error mistake was simple and straightforward; however, it failed to capture the role that previous decisions played in the incident. The team's original recommendation was to “be more careful next time.” However, a deeper process safety management system-related issue was uncovered by continuing the probe using the human error “root cause” as the starting point of the investigation. This is the point where the real systemic issues are found. This case study showed how poor communications between the different people involved with engineering design, contractor fabrication, equipment inspection, and subsequent site installation caused the incident. The final recommendation of the team was to link the separate management of change (MOC), prestartup safety review (PSSR), and mechanical integrity quality assurance-related efforts together, ensuring an inspection step for “replacement-in-kind.” In conclusion, the combined approach helps the team better understand the timing and conditions of the event, better understand why people make the decisions they make at the time the event unfolds, and ensures that systemic root causes are discovered so that more appropriate, system-related preventive measures are chosen and implemented. © 2011 American Institute of Chemical Engineers Process Saf Prog, 2011

TL;DR: In this paper, the authors provide a brief historical overview of major LOC incidents, describes how poor operational discipline increases process safety risk and can contribute to LOC incidents and provides an incident investigation tool to help monitor and improve operational discipline characteristics.
Abstract: Process safety and risk management systems are implemented to reduce injuries and process incidents. The effectiveness of these systems is dependent on the company's implementation and support for operational discipline (OD)-related programs to ensure that the process safety management system requirements are followed day-to-day at all levels of the organization. However, OD-program-related problems continue to contribute to process incidents, including loss of containment (LOC) incidents, especially since they may not be formally addressed during incident investigations. This article provides a brief historical overview of major LOC incidents, describes how poor OD increases process safety risk and can contribute to LOC incidents, and provides an incident investigation tool to help monitor and improve OD characteristics. © 2011 American Institute of Chemical Engineers Process Saf Prog, 2011

TL;DR: In this paper, the authors present a concept that combines these reliability and safety efforts, such that the maintenance, operations, engineering and safety groups speak the same language to minimize the overall business risk.
Abstract: Maintenance ensures the reliability of the equipment to meet operation's demands. Operations ensure the production of sufficient and high quality product. Engineering designs and installs equipment that minimizes risk. Safety ensures that the engineering and administrative controls are in place to minimize that risk. This article presents a concept that combines these reliability and safety efforts, such that the maintenance, operations, engineering and safety groups speak the same language to minimize the overall business risk. This proposed approach combines existing plant reliability efforts with several Process Safety Management elements. The basic steps include identifying safety-critical equipment, ensuring accurate and up-to-date Process Safety Information (PSI), establishing the mechanical integrity (MI) Program, establishing Planned Maintenance (PM) programs, and creating equipment-specific maintenance procedures. The resultant mechanical integrity quality assurance (MI QA) program combines equipment reliability and operational integrity efforts to ensure that the equipment is properly designed (via PSI), fabricated as designed (via PSI and MI), and installed as designed (via Pre-Start-up Safety Reviews). The MI QA program also ensures that the equipment is operated and maintained as designed by trained personnel. Some benefits include being able to: (1) share and document best MI and PM practices between different sites; (2) focus limited PM-related resources on the process safety-critical equipment; and (3) help monitor the essential reliability efforts to ensure continued safe operation. © 2011 American Institute of Chemical Engineers Process Saf Prog, 2011

TL;DR: A Cabot team used the ISA 84-2004 (IEC61511-1) standard to evaluate the safety instrumented system (SIS) for an existing fumed silica burner and made practical design changes to reduce the control-related process safety risks.
Abstract: A Cabot team used the ISA 84-2004 (IEC61511-1) standard to evaluate the safety instrumented system (SIS) for an existing fumed silica burner and made practical design changes to reduce the control-related process safety risks. Prior subjective layers of protection reviews and poor definitions for “tolerable risk” had resulted in assigning unreasonably high and impractical safety integrity levels (SIL) on safety critical interlocks. The Team turned to the ISA 84-approved layers of protection analysis (LOPA) procedure and found it to be a more realistic and a less subjective risk evaluation that resulted in a practical, defensible process safety system design. The Team's procedure focused on the following steps within the SIS life cycle: 1Perform a hazard and operability (HAZOP) process hazards analysis (PHA), 2Use PHA/HAZOP software to generate LOPA spreadsheets, 3Populate the LOPA spreadsheets for each risk scenario and compare the risk levels to the corporate risk matrix, 4Design an SIS/SIL to bridge the risk gaps identified using the LOPA, and 5Implement and document the changes. After completing the PHA/LOPA/tolerable risk matrix analysis (Steps 1–3), two scenarios were identified where the process safety risk was marginal and required design improvements. However, the Team struggled with Step 4, the SIS/SIL design, and progress essentially stopped. The Team's key learning, overcoming this roadblock, focused on locating a Certified Functional Safety Expert to complete and verify the SIS/SIL design integrity. Once Step 4 was completed, the Team implemented both identified SIS design improvements (Step 5), reducing the inherent control-related process safety risks with a practical solution. © 2011 American Institute of Chemical Engineers Process Saf Prog, 2011

TL;DR: Racing and motorcycle tire compounding has been studied extensively in the literature as discussed by the authors, with a focus on reducing the rolling resistance of the tire compounds, which has left a certain void in the science of compounding for racing and motorcycle treads.
Abstract: Racing tires and motorcycle tires present individual segments of the tire market. For instance, while the average life of car and truck tires is 50 000 miles, the average life of race tires is 100 miles. Because tires play a critical role in a race, technical demands to assure safety and performance are growing. Similarly, tires have a large influence on safety, handling/grip, and performance of the rapidly growing world fleet of motorcycles, due to the fact of only two wheels being in contact with the ground. Thus, the common feature of both market segments is that the typical tire compromise of wear, rolling resistance, and traction is strongly weighted toward traction. Most of the recent efforts of rubber scientists have been directed toward lowering rolling resistance of the tread compounds, which left a certain void in the science of compounding for racing and motorcycle treads. Particularly, the industrial assortment of polymers and fillers used for motorcycle treads is commonly different f...
