Abstract: Median nerve compression at the wrist (Carpal Tunnel Syndrome) is commonly associated with local trauma around the flexor retinaculum. Repeated manual activity also exacerbates the disease severity. We undertook a prospective study of the incidence of Carpal Tunnel Syndrome (CTS) in 47 paraplegic patients who have used their hands extensively for daily activity. Since surgical decompression generally provides excellent relief of symptoms, early detection of CTS will be particularly important in these patients. Of the 47 patients studied, 19 had clinical CTS (40%). A total of 91 hands (nerves) were tested with motor and sensory nerve conduction of the median and ulnar nerves. Electrophysiological evidence of CTS was noted in 57 hands (63%). The incidence of CTS appears to be related to the duration of Spinal Cord Injury. Concurrent ulnar neuropathy at the elbow was noted in 19 patients (40%). There was no predisposing factor such as diabetes mellitus in any of these patients, and the compressive neuropathy appears to be purely mechanical.

Abstract: Stress fractures of the lower ribs related to sports are rare. Fractures of the ribs related to rowing sports have not been reported. We have documented seven cases of stress fractures of the lower ribs in female athletes; four were elite rowers and three were engaged in tennis, golf, and gymnastics. Each athlete was initially seen and treated elsewhere for muscular strain. All athletes presented with pain in the posterolateral thorax in and around the scapula. The time from onset of symptoms to diagnosis ranged from 2 to 6 months. Bone scans were used to document the fractures when roentgenograms were equivocal. Each athlete responded to a 4 to 8 week period of rest or training modification. Biomechanical analysis of the forces across the ribs demonstrates that these fractures tend to occur along the posterolateral segment where the bending stresses on the rib are greatest. The predominant muscle forces are generated by the forced couple of scapular retraction and protraction acting through the serratus anterior. We postulate that inadequate strength and resistance training background in women is the cause for the apparent increased susceptibility of women to these skeletal injuries.

Abstract: A gas injector element for use in a mixing apparatus having a plurality of mixing elements, where the injector element is airfoil-shaped and is installed adjacent to the first of the plurality of mixing elements. The airfoil-shaped injector is fabricated from a metal or other suitable material having a porosity within a range of 0.5 to 100 or more microns, such that fluid injected into the mixing conduit through the injector will be released from the injector and will immediately contact material flowing through the conduit without the undesirable result of bubbles of the fluid or gas flowing from the injector coalescing with larger bubbles resulting in ineffective mixing of the gas with the material in the mixer.

Abstract: A fast algorithm for the solution of a Toeplitz system of equations is presented. The algorithm requires order N(\log N)^{2} computations where N is the number of equations. For banded Toeplitz matrices the order of computations is reduced to only N \log N + m(\log m)^{2} where 2m is the maximum number of nonzero principal subdiagonals of the Toeplitz matrix. The algorithm is in essence a fast implementation of the Trench algorithm in reverse. Thus, the algorithm involves imbedding of the given matrix in a cyclic matrix and a fast HD (half-divisor) algorithm to compute the first row of the inverse matrix. The desired solution is then obtained directly from the first row by applying fast Fourier transform techniques in order N \log N computations. Finally, the extension of the algorithm to block Toeplitz matrices is also presented.

Abstract: Cells of a freshwater sediment bacterium produced firmly bound extracellular polymers in laboratory cultures which, at the ultrastructural level, resembled those produced by natural sediment bacterial populations. Production of the exopolymers during subculture was maintained by using as a source of inoculum the population of cells which adhered to each other and to the wall of the glass culture vessel. The exopolymers were selectively released from the cells by blending and centrifugation in the presence of EDTA. Evaluation of glucose-6-phosphate dehydrogenase activity and 2-keto-3-deoxyoctonate concentration indicated that only small amounts of intracellular and cell wall components were released from the cells during exopolymer removal. Chemical analysis of the isolated crude exopolymer material indicated that it contained protein, polysaccharide, and DNA. The treatment promoted the selective isolation of firmly bound polymers from the surface of adherent cells.

Abstract: Between January 1976 and March 1982, 80 patients with histologically proven diagnosis of squamous cell carcinoma of the tonsillar region were treated with definitive radiotherapy. Sixty-five (81%) of these patients had locally advanced tumors (Stage III and IV); 49% of patients had clinically palpable cervical lymphadenopathy. All patients received a combined external megavoltage and interstitial irradiation. The dose of external irradiation was limited to 4500–5000 cGy over 4 1 2 to 5 1 2 weeks. This was followed by interstitial 192iridium implants to doses of 2000–2500 cGy in 50–60 hours for T1, T2 lesions and 3000–4000 cGy in 60–100 hours for T3, T4 lesions. The neck masses were also separately implanted to deliver additional doses of 2000–4000 cGy in 50–80 hours. Overall local tumor control was observed in 84% of patients with a minimum follow-up period of 2 years. An absolute 3-year disease free survival of the entire group was 72%. Treatment related complications such as soft tissue necrosis or osteoradionecrosis occurred in 6% ( 5 80 ) of patients. The salvage of neck failures and local failures was possible in 78 and 38% of patients, respectively, either by surgery or by re-irradiation employing interstitial 192iridium implants. Functional and esthetic integrity was well preserved in most cases.

Abstract: A systematic and quantitative analysis is carried out to investigate the effects of the shape (point, cube, Gaussian) and size of the generation volume, the surface recombination velocity, and the diffusion length on the electron‐beam‐induced current (EBIC) and its derivative (DEBIC). Thick homogeneously doped samples exhibiting diffusion lengths in the low micron and submicron range are considered. The results are presented in computed EBIC curves as a function of scanning distance and of the ratio true diffusion length/effective diffusion length. Using these curves, we show (1) a simple and yet rigorous method for the determination of the true diffusion length, taking into consideration all of the factors cited above, (2) a method for the rapid determination of the surface recombination velocity, (3) the condition under which the source shape becomes insignificant, and (4) a new value for the lower limit of the diffusion length which can be determined by the EBIC technique.

Abstract: For a sample of 109 graduate students beginning their first semester of practice teaching at the elementary-school level the concurrent validity of each of five subscales of the measure of academic self-concept entitled Dimensions of Self-Concept (DOSC)-Level of Aspiration, Anxiety, Academic Interest and Satisfaction, Leadership and Initiative, and Identification versus Alienation-was found relative to each of three criterion scales of the Maslach Burnout Inventory (MBI) scored for both frequency and intensity of response: (a) Emotional Exhaustion, (b) Depersonalization, and (c) Personal Accomplishment. It was concluded that for the most part the subscales of the DOSC demonstrated promising concurrent validity relative to the criterion measures afforded by the MBI. Abstract: Nursing Home Care Units in Veterans Administration Medical Centers across the country were surveyed to determine methods of management of urinary incontinence (UI) in the nursing home (NH) setting. Information was obtained from 90 of the VA NHs on demographic aspects of the NH population, prevalence and severity of urinary and fecal incontinence, common problems encountered, and specific strategies and techniques. Written guidelines for bladder training and catheter care from many of the NHs were analyzed. Abstract: Factor analyses and Procrustes rotations of the responses of 1194 female and 877 male university students in seven countries to a 69-item Likert-type questionnaire revealed cross-cultural commonalities and differences for romantic jealousy and romantic envy. However, when mean ratings of the questionnaire items were categorized as indicating agreement or disagreement with the items, the evidence for cross-cultural invariance of jealousy and envy was less impressive. The stronger cross-cultural invariance found with factor-analytic data was interpreted as evidence that the issues of concern in jealousy and envy situations are similar across nations. The weaker invariance found with the ratings of the items was taken as evidence that events arousing those issues of concern differ across nations.

Abstract: Five patients with smoke inhalation from house fires presented to the hospital in a comatose state. Carboxyhemoglobin levels were elevated in all five patients, mean=32% +/- 6. Arterial blood gases revealed the following means: pH 7.16 +/- 0.06; PCO2 35 mm HG +/- 10.5; HCO3 12.6 mEq/L +/- 0.07; base excess -16 mEq/L +/- 1.58; PO2 353 mm Hg +/- 149; O2 saturation 66% +/- 5.5. The patients were presumed to have both cyanide and carbon monoxide intoxication and were treated with the cyanide antidote kit and hyperbaric oxygen (HBO). Four of five patients awoke within 15 minutes of reaching maximum pressure and remained neurologically intact thereafter. The fifth patient died one week later. Cyanide blood levels drawn prior to treatment revealed a mean of 1.62 microgram/mL +/- 1.44. The highest cyanide level was 3.9 microgram/mL (the death) and the lowest 0.35 microgram/mL. We conclude that smoke inhalation can result in acute cyanide poisoning and that hyperbaric oxygen is a useful adjunct in the treatment of smoke inhalation.

Abstract: Iliotibial band syndrome was diagnosed in 48 of 1030 runners treated for lower extremity musculoskeletal complaints. Most athletes had been running 20 to 40 miles a week for one year or longer and had significantly changed their distance, speed, terrain, surface, and/or shoes before the onset of symptoms. Symptoms often persisted for 2 to 6 months. All runners were treated conservatively with rest, stretching, reduced distance, anti-inflammatory medications, local cortisone injections and/or orthoses.

Abstract: Hydrogen sulfide inhalation injury can be life threatening. The toxic gas is produced, sometimes unexpectedly, from a wide variety of sources. Because its mechanism of toxicity is similar to that of cyanide, hydrogen sulfide poisoning is commonly treated with the nitrite component of the cyanide antidote kit. In this case report, hyperbaric oxygen was successfully used to treat hydrogen sulfide intoxication. Further evaluation of hyperbaric oxygen therapy as adjunctive treatment of hydrogen sulfide poisoning is recommended.

Abstract: The dendritic architecture of the neurons of the visual pretectal nuclei in the rat was studied with the Golgi-Cox method. The olivary pretectal nucleus (PO) is characterized by distinctive neurons with a gnarled, tufted, richly branched dendritic arbor forming a dense neuropil within the nucleus. The distinct dendritic morphology of the olivary pretectal neurons enables this nucleus to be identified at all levels of the pretectum in Golgi-impregnated preparations. Rostromedially, the PO is surrounded by peripheral neurons whose dendrites wrap around the surface of the PO. The nucleus of the optic tract (NTO) contains three types of cells: (1) superficial horizontal cells whose dendrites extend out transversely; (2) large multipolar neurons whose dendrites spread out predominantly in a transverse plane, and (3) small to medium multipolar neurons with varying dendritic architecture. The posterior pretectal nucleus (PP) is composed predominantly of (1) multipolar cells with horizontally and vertically oriented dendrites extending out transverse to the optic axons; (2) piriform cells with dendrites extending dorsally toward the brachium; and (3) small multipolar neurons. The presence of superficial horizontal and large multipolar neurons in the NTO distinguishes the NTO from the PP in Golgi preparations. The horizontally oriented dendrites of many of the multipolar neurons in the PP give this nucleus an appearance distinct from that of the NTO. The differences in dendritic morphology between the visual pretectal nuclei in the rat permit identification of these nuclei at all levels within the pretectum. The boundaries of these nuclei, as determined in the Golgi-Cox preparations, correlate quite well with the boundaries defined by studying retinal projections (Scalia and Arango, '79).

Abstract: Computerized tomography (CT) scans of the head were done on 93 children with acute lymphocytic leukemia in continuous complete remission who had been randomly assigned to three different methods. of central nervous system (CNS) prophylaxis. Twenty-nine children had received six doses of intrathecal methotrexate, 30 had received six doses of intrathecal methotrexate plus 2400 rad of cranial irradiation, and 34 had received six doses of intrathecal methotrexate plus three courses of intermediate-dose intravenous methotrexate. The overall incidence of abnormal scans was 35%, of which 91% were felt to represent minimal abnormalities. CT scan abnormalities were noted in 30% of the children receiving intrathecal methotrexate only, in 40% of those receiving intrathecal methotrexate plus cranial irradiation, and in 35% of those receiving intrathecal methotrexate plus intermediate dose methotrexate. These differences were not statistically significant. None of the three methods of CNS prophylaxis resulted in significant CT scan abnormalities. However, the few moderately or markedly abnormal scans evaluated were restricted to patients who received intrathecal methotrexate plus cranial irradiation. The clinical significance of CT scan abnormalities in leukemic children receiving these treatments remains unclear.

Abstract: For an electronically motivated door lock there is a key carrying magnetic coding for actuating an electronically triggered release mechanism to unlock a door. Insertion of the key in the keyway initially closes a switch to energize the electronic circuit. The electronic circuit is programmed to motivate a motor driven mechanism in successive stages to move a blocking finger into and out of engagement with a notch in a rotatable disc on a cylinder plug which operates the latch bolt. The motor rotates preferably in one direction but by use of a rotating drive pin shiftable from end to end of a transverse slot, the blocking finger is moved alternately into and out of engagement with the notch in response to the successive energizations of the motor.

Abstract: Support for the weak version of the facial feedback hypothesis has been obtained repeatedly with the dynamic pose paradigm. In contrast, support for the strong version, which is tested with the static pose paradigm, has been inconsistent. We attributed this to methodological limitations in some studies rather than to problems intrinsic to the static pose paradigm. Utilizing a methodology that improved upon the hypothesized limitations, we obtained the facial feedback effect for the static poses of joy and anger with 142 male undergraduate students. The effect was uniform across levels of stimulus intensity and stimulus affect. Facial feedback did not vary with skills in nonverbal affective communication, self-monitoring, and the ability to form mental images.
