TL;DR: The ARIA guidelines for the management of allergic rhinitis and asthma are similar in both the 1999 ARIA workshop report and the 2008 Update as discussed by the authors, but the GRADE approach is not yet available.
Abstract: Allergic rhinitis is a symptomatic disorder of the nose induced after allergen exposure by an IgE-mediated inflammation of the membranes lining the nose. It is a global health problem that causes major illness and disability worldwide. Over 600 million patients from all countries, all ethnic groups and of all ages suffer from allergic rhinitis. It affects social life, sleep, school and work and its economic impact is substantial. Risk factors for allergic rhinitis are well identified. Indoor and outdoor allergens as well as occupational agents cause rhinitis and other allergic diseases. The role of indoor and outdoor pollution is probably very important, but has yet to be fully understood both for the occurrence of the disease and its manifestations. In 1999, during the Allergic Rhinitis and its Impact on Asthma (ARIA) WHO workshop, the expert panel proposed a new classification for allergic rhinitis which was subdivided into 'intermittent' or 'persistent' disease. This classification is now validated. The diagnosis of allergic rhinitis is often quite easy, but in some cases it may cause problems and many patients are still under-diagnosed, often because they do not perceive the symptoms of rhinitis as a disease impairing their social life, school and work. The management of allergic rhinitis is well established and the ARIA expert panel based its recommendations on evidence using an extensive review of the literature available up to December 1999. The statements of evidence for the development of these guidelines followed WHO rules and were based on those of Shekelle et al. A large number of papers have been published since 2000 and are extensively reviewed in the 2008 Update using the same evidence-based system. Recommendations for the management of allergic rhinitis are similar in both the ARIA workshop report and the 2008 Update. In the future, the GRADE approach will be used, but is not yet available. Another important aspect of the ARIA guidelines was to consider co-morbidities. Both allergic rhinitis and asthma are systemic inflammatory conditions and often co-exist in the same patients. In the 2008 Update, these links have been confirmed. The ARIA document is not intended to be a standard-of-care document for individual countries. It is provided as a basis for physicians, health care professionals and organizations involved in the treatment of allergic rhinitis and asthma in various countries to facilitate the development of relevant local standard-of-care documents for patients.

TL;DR: There is a need to increase knowledge of adolescent females about the risks of breast cancer and benefits of early detection and health care professionals can develop effective breast health care programs and help young women to acquire good health habits.
Abstract: Young breast cancer patients have a lower rate of survival than old breast cancer patients due to being diagnosed at advanced stages. Breast self-examination makes women more "breast aware", which in turn may lead to an earlier diagnosis of breast cancer. The purpose of this study was to investigate knowledge and practice of breast self-examination and to determine knowledge of risk factors for breast cancer among high school students. This is a descriptive and cross-sectional study. It was conducted in a high school in Manisa, Turkey. The study sample included 718 female high school students. A socio-demographic characteristics data form, knowledge of breast self examination and risk factors for breast cancer form and breast self examination practice form were used to collect data. The female high school students had insufficient knowledge about breast self-examination and a low percentage of students reported that they had performed breast self examination monthly. The most common reason for not doing breast self- examination was "not knowing how to perform breast self-examination" (98.5%). Most of the students had little knowledge of the risk factors for breast cancer. The most widely known risk factor by the students was personal history of breast cancer (68.7%). There was a significant relation between breast self-examination practice and age, school grade, knowledge about breast cancer and knowledge about breast self- examination. There is a need to increase knowledge of adolescent females about the risks of breast cancer and benefits of early detection. In fact, health care professionals can develop effective breast health care programs and help young women to acquire good health habits.

TL;DR: Findings are discussed concerning the role of protease-activated receptor-2 in enteric disease which may account for the presence of abdominal pain and diffuse symptoms in Blastocystis infection, even in the absence of fever and endoscopic findings.
Abstract: Blastocystis is a prevalent enteric protozoan that infects a variety of vertebrates. Infection with Blastocystis in humans has been associated with abdominal pain, diarrhea, constipation, fatigue, skin rash, and other symptoms. Researchers using different methods and examining different patient groups have reported asymptomatic infection, acute symptomatic infection, and chronic symptomatic infection. The variation in accounts has lead to disagreements concerning the role of Blastocystis in human disease, and the importance of treating it. A better understanding of the number of species of Blastocystis that can infect humans, along with realization of the limitations of the existing clinical laboratory diagnostic techniques may account for much of the disagreement. The possibility that disagreement was caused by the emergence of particular pathogenic variants of Blastocystis is discussed, along with the potential role of Blastocystis infection in irritable bowel syndrome (IBS). Findings are discussed concerning the role of protease-activated receptor-2 in enteric disease which may account for the presence of abdominal pain and diffuse symptoms in Blastocystis infection, even in the absence of fever and endoscopic findings. The availability of better diagnostic techniques and treatments for Blastocystis infection may be of value in understanding chronic gastrointestinal illness of unknown etiology.

TL;DR: In this paper, the factors that shape individual happiness and life satisfaction among Turkish people are determined using a representative survey of sociocultural and political change, World Values Survey (WVS).
Abstract: This research aims to investigate life satisfaction and happiness in Turkey. It extends the previous researches on subjective well-being (SWB) for Turkey by considering both happiness and life satisfaction. The previous researches for Turkey are local studies, and their findings cannot be generalized to the population of Turkish society. Given these facts, the factors that shape individual happiness and life satisfaction amongst Turkish people are determined using a representative survey of sociocultural and political change, World Values Survey (WVS). Some results obtained in this study are similar to typical findings such as a negative age effect, positive influences of income and health status, and a negative effect of unemployment. Contrary to the expectations, middle education has a negative direct effect on life satisfaction among females, and the upper education level is insignificant in the life satisfaction model. By comparing the correlates of happiness and life satisfaction across different years, this research aims to provide insights for policy makers and professionals to improve the perceived lives of Turkish people.

TL;DR: The present study suggests that subtype 2 is a non-pathogenic genotype of B. hominis, one of the most common eukaryotic organisms in the intestinal tract of humans, while its pathogenic potential is still controversial.
Abstract: Blastocystis hominis is one of the most common eukaryotic organisms in the intestinal tract of humans, while its pathogenic potential is still controversial. A total of 286 stool samples obtained from adult and pediatric patients with or without gastrointestinal symptoms in two hospitals in Manisa, Turkey, were cultured to detect B. hominis infection. Forty-one and 51 isolates were obtained from the adults and children, respectively, and these isolates were subjected to subtyping by polymerase chain reaction (PCR) with the known sequence-tagged site primers. The correlation between the genotype and the symptoms was evaluated. PCR subtyping indicated that subtype 3 was the most common genotype in both symptomatic and asymptomatic groups, and the second common genotype was subtypes 1 and 2 in symptomatic and asymptomatic groups, respectively. A significant correlation between subtype 2 and the asymptomatic groups was found among both in pediatric and adult patients (chi(2) (cal) = 4.38, df = 1, p = 0.044). However, there were no significant differences between the other genotypes and the symptomatic or asymptomatic groups, as well as both the age and sex of the patients. The present study suggests that subtype 2 is a non-pathogenic genotype of B. hominis.

TL;DR: In this paper, the authors show that the inferred heterogeneities within the mantle source regions are likely to have originated from continuous processes of formation and destruction of enriched mantle domains by long-term plate recycling, convective mixing and melt extraction.
Abstract: The Late Triassic to Late Cretaceous age mafic lavas from the Neotethyan suture zone ophiolites in western Turkey exhibit a wide diversity of geochemical signatures, indicating derivation from extremely heterogeneous mantle sources. The rocks as a whole can be divided into three broad subdivisions based on their bulk-rock geochemical characteristics: (1) mid-ocean ridge basalts (MORB) that range in composition from light rare earth element (LREE)-depleted varieties (N-MORB; (La/Sm)N 1); (2) the ocean island basalt (OIB)-type alkaline volcanic rocks with significant enrichment in LILE, HFSE and L-MREE, and a slight depletion in HREE, relative to normal mid-ocean ridge basalts (N-MORB); and (3) the supra-subduction zone (SSZ)-type tholeiites originated from arc mantle sources that are characterized by selective enrichments in fluid-soluble large ion lithophile elements (LILE) and LREE relative to the high field strength elements (HFSE). The formation of MORB tholeiites with variable enrichments and depletions in incompatible trace elements is probably related to the processes of crust generation along an oceanic spreading system, and the observed MORB–OIB associations can be modelled by heterogeneous source contribution and mixing of melts from chemically discrete sources from sub-lithospheric reservoirs. Evaluation of trace element systematics shows that the inferred heterogeneities within the mantle source regions are likely to have originated from continuous processes of formation and destruction of enriched mantle domains by long-term plate recycling, convective mixing and melt extraction. The origin of SSZ-type tholeiites with back-arc basin affinities, on the other hand, can be attributed to the later intra-oceanic subduction and plate convergence which led to the generation of supra-subduction-type oceanic crust as a consequence of imparting a certain extent of subduction component into the mantle melting region. Mixing of melts from a multiply depleted mantle source, which subsequently received variable re-enrichment with a subduction component, is suggested to explain the generation of supra-subduction-type oceanic crust. The geodynamic setting in which much of the SSZ-type ophiolitic extrusive rocks from western Turkey were generated can be described as an arc-basin system that is characterized by an oceanic lithosphere generation most probably associated with melting of mantle material along a supra-subduction-type spreading centre.

TL;DR: Results show a wide spread in the chemical specification of these mineral waters, with differences in chemical composition observed in the regions being due to the geological environment and the majority of bottled mineral waters exceeding the pH limit of Turkish drinking water standards.
Abstract: Fifteen bottled mineral waters purchased at random all over Turkey were analyzed for their chemical composition by OPTIMA-2000 ICP-AES Perkin Elmer techniques. Results show a wide spread in the chemical specification of these mineral waters, with differences in chemical composition observed in the regions being due to the geological environment and the majority of bottled mineral waters exceeding the pH limit of Turkish drinking water standards. When the concentrations of elements are evaluated, it can readily be seen that generally there are three types of mineral water in Turkey. The concentrations of Al, B, Ba, Cd, Cu, Cr, Fe, Mn Pb and Zn in mineral water were compared with the limits established by the Turkish Standard for Natural Mineral Waters (Turkish Official Gazette 2004); water standards prepared by World Health Organization (2006) and the United States of America Environmental Protection Agency (US EPA) drinking water standards (1993). Such a comparison shows that, except for Ba and Mn, the concentrations of the other heavy metals are lower than the limit of the US.EPA in Turkey. Some parameters examined were found to comprise strong correlations pair-wise.

TL;DR: Zinc was found to be the most abundant metal in water and tissues of Chondrostoma nasus and Barbus capito pectoralis and in some tissues, the concentrations of some of these metals exceeded acceptable levels for human consumption.
Abstract: The genotoxic and histopathological effects of water pollution were investigated on two fish species caught from the Buyuk Menderes River and from its tributary, the Cine Stream. The Buyuk Menderes basin is an important agricultural area in Turkey. The levels of copper, zinc, cadmium, cobalt, and lead were measured at the surface of the water and in gills, liver, and muscle tissue of Chondrostoma nasus and Barbus capito pectoralis. In some tissues, the concentrations of some of these metals exceeded acceptable levels for human consumption. Zinc was found to be the most abundant metal in water and tissues. Maximal metal accumulation was observed in the liver. To detect the genotoxic potential of contaminants, the formation of micronucleus in erythrocytes was used as indicator of chromosomal damage. The frequency of micronucleus formation did not show significant differences between locations and controls in B. capito pectoralis caught from three locations and C. nasus from two locations. The histological changes included significant decreases of the mean lengths of primary and secondary lamellae. In gills epithelia, we observed cellular proliferation that developed Because of secondary lamellae fusion, ballooning degenerations, or club deformation of secondary lamellae and cystic structures in secondary lamellae. In the liver, the changes included swollen and ruptured parenchymal cells, loss of cord structure, vacuoles filled with cellular debris, focal necrosis, and a significant increase in Kupffer cells.

TL;DR: The weight-gain side effect of atypical antipsychotic medications can be related with the orexigenic effect of elevated serum ghrelin rather than leptin deficit, and quetiapine is the only one which does not elevate the gh Relin level.
Abstract: Background Weight gain is a major side effect of antipsychotic treatment. Some atypical antipsychotic agents have profound effects on weight. Body weight is regulated by a complex system, including both peripheral and central factors. Two of the hormones that seem to play an important role in the regulation of food intake, energy metabolism, and body weight are leptin and ghrelin. Leptin is a mediator of long-term regulation of energy balance, suppressing food intake and thereby inducing weight loss. Ghrelin on the other hand is a fast-acting hormone, seemingly playing a role in meal initiation. In this present study it is aimed to compare the effects of five different atypical antipsychotic medications on leptin and ghrelin. Method 112 patients who were treated either with clozapine ( n = 20), olanzapine ( n = 28), risperidone ( n = 22), quetiapine ( n = 20) or amisulpride ( n = 22) as monotherapy for at least one year and age, gender, and body mass index (BMI) matched control group ( n = 23) were assessed cross-sectionally. Ghrelin and leptin levels were measured with enzyme-immunoassay. Results When fasting serum leptin levels were compared between groups, control group had the highest mean value (9.2 ± 6.7) and amisulpride group had the lowest mean value (3.7 ± 2.1) but still there was no statistically significant difference between six groups ( F = 1993, p = 0.084). In the comparison of the mean values of fasting serum ghrelin levels there was a statistically significant difference between groups ( F = 11,473, p = 0.00). In post-hoc analysis it was seen that the control group had the lowest ghrelin level (194.5 ± 86.8). Quetiapine treated group (378.1 ± 260.4) had similar fasting serum ghrelin levels to control group. All the other antipsychotic treatment groups had significantly higher levels of fasting serum ghrelin compared to control group, highest in amisulpride treated group (597.0 ± 150.0). Conclusion The weight-gain side effect of atypical antipsychotics can be related with the orexigenic effect of elevated serum ghrelin rather than leptin deficit. Among the five widely used atypical antipsychotics quetiapine is the only one which does not elevate the ghrelin level.

TL;DR: Significant differences according to bodily pain and role emotional subscales of SF-36, and the BDI scores, show that the mothers were negatively affected by having a child with monosymptomatic nocturnal enuresis.
Abstract: The aim of this study was to evaluate the impact of enuresis nocturna on quality of life of the mothers. Mothers who have a child with monosymptomatic nocturnal enuresis (n=28) and mothers who have a child without any health problems (n=38) were enrolled in the study. Groups were in balance for background variables (child's age, gender, and number of siblings; mother's age, marital status, highest year of education completed, and occupation; presence of health insurance; and type of residence). Short-Form Health Survey (SF-36) Questionnaire, the Beck Depression Inventory (BDI), and Spielberg's State-Trait Anxiety Inventory (STAI) were applied to all mothers. The mothers of children with enuresis had significantly lower quality-of-life scores in the SF-36 for the bodily pain (p=0.015) and role emotional (p=0.014) subscales. We observed significant difference between groups according to BDI; mean score was higher in mothers who have a child with enuresis nocturna (p=0.017). There was no significant difference between groups according to the STAI. Significant differences according to bodily pain and role emotional subscales of SF-36, and the BDI scores, show that the mothers were negatively affected by having a child with monosymptomatic nocturnal enuresis.

TL;DR: In this paper, the equations of motion of a third grade fluid between micro-parallel plates are derived and made dimensionless, and approximate analytical solutions are obtained by perturbation techniques.
Abstract: Electro-osmotic flow of a third grade fluid between micro-parallel plates is considered. The equations of motion are derived and made dimensionless. Approximate analytical solutions are obtained by perturbation techniques. Constant viscosity and temperature dependent viscosity (Reynolds model) cases are treated separately. Numerical solutions of the equations are also obtained. Influences of non-Newtonian parameter, Joule heating effect, viscosity index and electro-kinetic effect on the velocity and temperature profiles are shown. Approximate and numerical solutions are contrasted.
