Abstract: A case of dermatomycosis caused by Arthrinium phaeosperrnum is reported. It is an opportunistic pathogenic fungus found for the first time in China.Multiple subcuta- neous nodules and atrophic alopecia developed on head of the patient.Hair follicles and se- baeous glands were severely destroyed in biopsy with skin lesion.Spores were brown or drab- colored and septate hyphae were found in granuloma especially in the subcutaneous fatty layer. Spherical spores were repeatedly detected from the dandriffs outside the skin lesion.It grew well at 37℃ in Sabouraud medium.Colonies in early stage are light yellow in color then turned gradually into orange.Hyphae with septa (1—2μm) were seen in culture under micros- cope.Conidia varies in size and are spherical,elliptical,and oval in shape.The fungus was tested by biochemistry assay.The suspensions prepared from A.phaeospermum were inoculated intradermally in animal toxicity experiment.Erythemous masses,small nodules,abacess,and alopecia are similar to those seen in patient's symptoms and signs developed 2 weeks after in- oculation.The pathological changes of animal skin lesion in biopsy are also similar to those found in the patient.Twelve anti-fungus drugs and 15 traditional medicinal herbs were tested for the sensitivity of A.phaeospermum.

Abstract: The incidence of chronic pancreatitis in China is unknown. Endoscopic retrograde cholangiopancreatography (ERCP), surgery, and autopsy demonstrated that chronic pancreatitis in China is mainly secondary to cholelithiasis and other diseases of the bile ducts. Stones in the common bile duct, both intra- and extrahepatic, are extremely frequent in China. Such patients may have scores, even hundreds, of stones filling the common bile duct and its radicles. Biliary tract disease constitutes the etiology of chronic pancreatitis in 40-50% of our patients. The prolongation of necrosis, abscess, or pseudocyst after acute pancreatitis may also lead to chronic inflammation of the pancreas, as may a diverticulum at the second part of the duodenum. The pancreatitis observed in China is apt to be of the chronic relapsing type. Pancreatic ductal stones and ascaris lumbricoides may sometimes be associated with chronic inflammation of the pancreas; however, the cause of chronic pancreatitis is obscure in a large segment of the patients. Beside the ordinary clinical manifestation of chronic pancreatitis, such as abdominal pain, fever, jaundice, and steatorrhea, regional (splenic) portal hypertension may be observed. ERCP has been useful in diagnosis, and the major changes found in the pancreatic duct are discussed. Since the symptoms and signs of chronic pancreatitis in China are usually mild or moderate, the patient with intractable pain is uncommon, most being treated with medication. Most surgical procedures utilized to treat chronic pancreatitis are related to the biliary system, such as cholecystectomy with internal or external drainage of the choledochus. Internal drainage of a pancreatic pseudocyst, partial pancreatectomy, and pancreaticojejunostomy are also performed, as indicated, but are less frequent.

Abstract: According to literature, it is difficult to make an early correct diagnosis and treatment of the intracapsular condylar vertical fracture The author recently suggested a new method to diagnose and treat this kind of fracture with an optimal result The diagnostic and treatment procedures are: 1 To take a condylo-pterygo-maxillo-oblique tomography which can clearly demonstrate the position of the bone fragment 2 To remove the bone fragment by submandibular incision with an oblique osteotomy of the posterior part of the ramus Therefore the secondary injury to the disk and the superior cavity could be avoided that is essential for TMJ reconstruction 3 To reconstruct the TMJ after condyloplasty under the centric relation Eight cases were successfully treated by this method In this paper the details of the operation are described and discussed

Abstract: Multiple organ failure (MOF) in the elderly is a new syndrome evolved from multiple organ chronic diseases on the basis of multiple organ dysfunction in the aged. Its characteristics are clinically different from those of MOF due to serious trauma. 122 cases of MOF were analysed retrospectively and their clinical features discussed. MOF with a long course is the natural presentation in many of the elderly before death. Its main precipitating factors are pulmonary infection, metastatic carcinoma, cardiac attack, etc. The sequence of a failure in organs is heart, lung, kidney, liver, etc. The mortality is similar to that of MOF due to trauma. However, those suffering from 4-organ failure can still survive, and instead, the renal failure can be mostly fatal. More attention should be paid to the prevention of MOF in the elderly so as to shorten its developing course.

Abstract: In this survey, 60 patients with pulmonary infection (mainly nosocimial pneumonia) were divided into two groups randomly. 30 patients treated by injection of Scutelleria compound and 30 patients by intravenous piperacillin sodium as control. Before treatment there was no significant difference of clinical data between two groups. The total efficacy was evaluated after treatment for one week. 40%, 36.7% were marked effective in corresponding. 33.3%, 40% were effective and 26.7%, 23% were ineffective respectively. Total effective rates were 73.3% and 76.7% in corresponding group. Temperature was average decreased for 7.43 +/- 3.02 and 7.63 +/- 5.79 days, symptoms disappeared or relieved for 11.67 +/- 6.75 days and 11.53 +/- 7.30 days, leucocyte decreased to normal on 9.03 +/- 6.53 days and 10.13 +/- 6.23 days, roentgenographical shadows disappeared or became smaller for 16.1 +/- 7.02 days and 16.1 +/- 9.88 days in corresponding group. There were no side-effect of the functions of liver, kidney and medulla ossium in two groups. In piperacillin sodium group, fungal infection were found in 4 of 30 patients, but there was no any case suffered from fungal infection in injection of Scutelleria compound group. It was suggested that injection on Scutelleria compound is a hopeful preparation and should be studied further.

Abstract: The authors developed an indirect immunofluorescence technique for the detection of Anti-salivary duct antibody (ASDA) and screened 34 patients with rheumatoid arthritis, 15 patients with Sjogren's syndrome-rheumatoid arthritis and 15 patients with primary Sjogren's syndrome, 63 cases with other connective tissue diseases, 9 cases with other diseases and 40 normal controls. The incidence of ASDA in patients with Sjogren's syndrome rheumatoid arthritis (66.67%) or rheumatoid arthritis (32.35%) was significantly higher than that in normal controls (P less than 0.001). In patients with primary Sjogren's syndrome and other CTDs, non-CTDs, no ASDA was found. However, in patients with Sjogren's syndrome-rheumatoid arthritis or rheumatoid arthritis alone, ASDA was not correlated with age, sex, disease duration or serological findings. The result suggests that the detection of serum ASDA might be useful in the differentiation of Sjogren's syndrome with rheumatoid arthritis from primary Sjogren's syndrome with arthralgia and/or arthritis.

Abstract: An adult case of metachromatic leukodystrophy, proved by characteristic findings of the brain and superficial sural nerve biopsies, but with absence of deficiency of arylsulphatase A activity in the leucocytes, was reported. The long course of thirty years, the absence of deficiency of arylsulphatase A activity, the discrepancy between the normal conduction velocity of the peripheral nerves and the typical pathological findings of the superficial sural nerve under the light and electron microscopes and the significance of the diffuse hypodense areas and high intensity signals of the cerebral white matter on CT and MRI respectively, were discussed.

Abstract: This paper reports the successful experimental studies of a laser detonating a mini-explosive head to break stones in vitro and in vivo. This technique has been used clinically with gastric bezoars since January 1988. All 31 patients were completely cured without complications. The relationship between gastric bezoars and ulcers and the treatment technique are also discussed in this paper. Diospyrobezoar is one of the common bezoars in the persimmon producing regions. This disease has been reported in many countries including our own, but until now there has been no effective treatment for large gastric bezoars except by surgery. To develop a better treatment for this problem the laser ignited mini-explosion technique to break large bezoars in vitro and in vivo was begun in October 1986 and first successfully applied clinically in January 1988.

Abstract: Using CO2 laser and YAG laser to seal experimental created the rupture of spleen on six dogs? the results were excellent. The incisions were made with scalpel, it’s length was 30–50mm and depth was 4–10mm. CO2 laser: power output 3W, spot size 0.3mm, energy density 1800–16200 J/sqcm, YAG laser: power at the tip of the fiber 5W, energy density 4095–14333J/sqcm. After sealing, the animals were killed at 0, 1, 3, 12 and 28 days respectively. The macroscopic and microscopic findings showed that the healing of lesions were excellent. Our primery experiment shows that CO2 laser and YAG laser may be used to seal the ruputer of spleen. The demage of CO2 laser was smaller then that of YAG laser, but YAG laser was more useful for blood field and more effective for the larger ruputer of spleen.
