Abstract: The development of noninvasive devices to manage hemodynamics in patients with shock is directly prompted by the results of recent studies in the intensive care unit (ICU), and during the perioperative period, which demonstrated the inability of an invasive approach, based on right heart catheterization, to improve prognosis [1]. Some authors have suggested that these results were largely due to inaccurate use of right heart catheterization, without clear goals or protocol [2], However, previous studies have demonstrated that optimization of cardiac output and mixed venous oxygen saturation (SvO2) with clear endpoints also fails to improve prognosis [3]. So, the lack of efficacy is inherent in the device. In 1985, Eugene Robin suggested that using right heart catheterization in patients with shock led physicians to give fluids plus diuretics whatever the wedge pressure [4]. In 2003, Francois Jardin claimed that we were going to move from the “age of oil lamps” to the “age of elec-tricity” [5]. In fact, we are going to change our practices in the management of shock, from an invasive and quantitative approach of hemodynamics to a non-invasive one, more functional and especially qualitative, mainly thanks to the use of echocardiography. This leads us to think about the meaning of hemodynamic monitoring.

Abstract: Surgical resection is the only method for treating colorectal liver metastases, because it can ensure long-term survival and cure in some patients. Only approximately 20 % of colorectal cancer patients with liver metastases can undergo liver resection at the time of diagnosis. Modern chemotherapy regimens are received by other patients, which could achieve high response rates but are insufficient for cure. Recurrences are observed in the majority of patients who undergo liver resection despite developed surgical technique and improved surgical skills. Many patients with resectable metastases undergo neoadjuvant chemotherapy before liver resection. Its benefit in terms of long-term disease control has been recently demonstrated, but neoadjuvant chemotherapy has also potential disadvantages, in particular chemotherapy-associated liver injury.

Abstract: En France, avec près de 6000 cas annuels, l’adénocarcinome gastrique est la 3 incidence plus élevée parmi les cancers digestifs. Dans 85 % des cas, il est diagnostiqué à un stade avancé avec un faible taux de survie de 25 % à cinq ans [1]. Chez les patients présentant une inflammation chronique de la muqueuse gastrique, le plus souvent secondaire à des infections actives ou passées à H. pylori, les lésions précancéreuses peuvent se produire dans un quart des cas [2]. Ces lésions sont le plus souvent de l’atrophie glandulaire et moins fréquemment de la métaplasie intestinale dans l’épithélium glandulaire. Dans ces lésions peuvent se développer une dysplasie ainsi qu’un cancer de l’estomac qui se produit chez 1 % des patients qui ont déjà été infectés par H. pylori. Dans 90 % des cas, une inflammation chronique de la muqueuse gastrique des lésions précancéreuses, telles que la dysplasie de la muqueuse, précède le cancer de l’estomac de plusieurs années. Chez ces patients, le risque de cancer de l’estomac a été évalué à 5,2 % dans les deux ans suivant le diagnostic de dysplasie [3]. Une surveillance active suite à la détection de l’atrophie glandulaire associée à une métaplasie intestinale favorise la détection précoce des patients atteints de dysplasie, permet un traitement précoce. Après résection endoscopique de la dysplasie ou un cancer précoce, le taux de survie sans cancer à trois ans est proche de 95 % [4]. Cependant, des études ont montré que les lésions de métaplasie intestinale ou de dysplasie sont manquées chez 50 % des patients dans la pratique courante de la gastroentérologie [5,6]. Afin de prélever les tissus de l’estomac et de diagnostiquer la dysplasie, quatre procédures peuvent être utilisées : 1) la pratique de quatre à cinq biopsies prélevées au hasard dans l’antre gastrique et corpus ; 2) la pratique de biopsies orientées sur les lésions de métaplasie intestinale visible à la lumière NBI ; 3) les biopsies sur des lésions suspectes dysplasiques par un examen sous lumière NBI avec un endoscope muni d’une lentille grossissante ; 4) la pratique de biopsies étagées systématiques.
