Abstract: Administrative coding of medical diagnoses in intensive care unit (ICU) patients is mandatory in order to create databases for use in epidemiological and economic studies. We assessed the reliability of coding between different ICU physicians. One hundred medical records selected randomly from 29,393 cases collected between 1998 and 2004 in the French multicenter Outcomerea ICU database were studied. Each record was sent to two senior physicians from independent ICUs who recoded the diagnoses using the International Statistical Classification of Diseases and Related Health Problems: Tenth Revision (ICD-10) after being trained according to guidelines developed by two French national intensive care medicine societies: the French Society of Intensive Care Medicine (SRLF) and the French Society of Anesthesiology and Intensive Care Medicine (SFAR). These codes were then compared with the original codes, which had been selected by the physician treating the patient. A specific comparison was done for the diagnoses of septicemia and shock (codes derived from A41 and R57, respectively). The ICU physicians coded an average of 4.6 ± 3.0 (range 1 to 32) diagnoses per patient, with little agreement between the three coders. The primary diagnosis was matched by both external coders in 34% (95% confidence interval (CI) 25% to 43%) of cases, by only one in 35% (95% CI 26% to 44%) of cases, and by neither in 31% (95% CI 22% to 40%) of cases. Only 18% (95% CI 16% to 20%) of all codes were selected by all three coders. Similar results were obtained for the diagnoses of septicemia and/or shock. In a multicenter database designed primarily for epidemiological and cohort studies in ICU patients, the coding of medical diagnoses varied between different observers. This could limit the interpretation and validity of research and epidemiological programs using diagnoses as inclusion criteria.

Abstract: Mathematical modelling of infectious diseases has gradually become part of public health decision-making in recent years. However, the developing status of modelling in epidemiology and its relationship with other relevant scientific approaches have never been assessed quantitatively. Herein, using antibiotic resistance as a case study, 60 published models were analysed. Their interactions with other scientific fields are reported and their citation impact evaluated, as well as temporal trends. The yearly number of antibiotic resistance modelling publications increased significantly between 1990 and 2006. This rise cannot be explained by the surge of interest in resistance phenomena alone. Moreover, modelling articles are, on average, among the most frequently cited third of articles from the journal in which they were published. The results of this analysis, which might be applicable to other emerging public health problems, demonstrate the growing interest in mathematical modelling approaches to evaluate antibiotic resistance.

Abstract: We have determined the acoustic and microwave frequencies of a misaligned spherical resonator maintained near the temperature of the triple point of water and filled with helium with carefully characterized molar mass g mol−1, with a relative standard uncertainty . From these data and traceable thermometry we estimate the speed of sound in our sample of helium at K and zero pressure to be m2 s−2 and correspondingly deduce the value J mol−1 K−1 for the molar gas constant. We estimate the value J K−1 for the Boltzmann constant using the currently accepted value of the Avogadro constant NA. These estimates of R and k, with a relative standard uncertainty of 1.06 × 10−6, are 1.47 parts in 106 above the values recommended by CODATA in 2010.

Abstract: In these experiments, we compared reading processes in arabic and in french. We observed that texts are read at the same speed in the two languages and that oculomotor activity is related to the morphological structure of words. Arabic text reading processes seem impaired when introducing vowels. We conclude that lexical access of non concatenative languages like arabic or hebrew may be very different from access in concatenative ones like french or english.
