Abstract: This paper discusses a qualitative and quantitative study of the circumstances of 20 Pakistani and Bangladeshi families with one or more severely disabled children living in Birmingham, England. Parents and other adult carers were interviewed using a combination of structured questionnaires and a semi-structured interview schedule focusing on the families' material circumstances, their use of formal services, informal care arrangements, and aspects of the parents' social and psychological well-being. The study suggests that previous national surveys of disabled families may have under-estimated the extent of material disadvantage, while it confirms that health and social care professionals should not assume that Pakistani and Bangladeshi parents have recourse to high levels of extended family support. Abstract: This paper develops a new fibre tracking algorithm to efficiently locate fibre centrelines (skeletons), from X-ray Computed Tomography (X-ray CT) images of carbon fibre reinforced polymer (CFRP), which are then used to generate micro-scale finite element models. Three-dimensional images with 330 nm voxel resolution of multidirectional [+45/90/−45/0] CFRP specimens were obtained by fast synchrotron X-ray CT scanning. Conventional image processing techniques, such as a combination of filters, delineation of plies, binarisation of images, and fibre identification by local maxima and ultimate eroding points, were tried first but found insufficient to produce continuous fibre centrelines for segmentation, especially in regions with highly congested fibres. The new algorithm uses a global overlapping stack filtering step followed by a local fibre tracking step. Both steps are based on the Bayesian inference theory. The new algorithm is found capable of efficiently define fibre centrelines for the generation of micro-scale finite element models with high fidelity.

Abstract: Osteoarthritis (OA) is the fastest growing cause of disability worldwide. The aim of this study was to understand the impact of OA on individuals and to explore current treatment strategies. An online UK-wide survey of people with self-reported OA was conducted, composed of 52 questions exploring the impact of OA, diagnosis and treatment, the role of health professionals and self-management. Four thousand forty-three people were invited with 2,001 respondents (49 % response, 56 % women; mean age 65 years). Fifty-two percent reported that OA had a large impact on their lives. Fifteen percent of respondents had taken early retirement on average 7.8 years earlier than planned. In consultations with general practitioners, only half reported a discussion on pain; fewer reported discussing their fears (21 %) or management goals (15 %). Nearly half (48 %) reported not seeking medical help until pain was frequently unbearable. Oral analgesics (62 %), topical therapies (47 %), physiotherapy (38 %) and steroid injections (28 %) were commonly used. The majority (71 %) reported varying degrees of persistent pain despite taking all prescribed medication. Although 64 % knew that increasing exercise was important, only 36 % acted on this knowledge; 87 % who increased exercise found it beneficial. Over half had future concerns related to mobility (60 %), maintaining independence (52 %) and coping with everyday activities (51 %). OA had significant individual economic impact especially on employment. Current treatment strategies still leave most people in pain with significant fears for the future. There is considerable opportunity to improve the holistic nature of OA consultations especially in provision of information and promotion of self-management strategies.

Abstract: The claw geometry of birds can be used to predict their mode of life. Previous studies, however, have not considered how bird size might affect these predictions. Thus, in the present study, the geometric scaling relationships of bird claws are examined for a variety of extant birds with different modes of life: predatory, climbing, perching or ground-dwelling. Measurements of hind-limb claw radius (i.e. claw size) and claw angle (i.e. claw ‘hookedness’) of the third digit claw were made on 120 species of bird ranging from 0.0057 kg to 44 kg in body mass. Claw radius was found to be proportional to (body mass)0.34 across all species. Claw angle was found to increase with body mass for predatory and climbing birds (i.e. bigger birds have relatively more hooked claws), and decrease with body mass for ground-dwelling birds (i.e. bigger birds have relatively less hooked or flatter claws). No significant relationship was found between claw angle and body mass for perching birds. Mode of life could not be predicted with any certainty using measurements of either claw radius or claw angle, suggesting difficulty in assigning fossil species such as Archaeopteryx to a specific locomotor category. As claw design should enable the claw to withstand the forces placed upon it, further work is needed to establish the stresses experienced by the claws of different types of bird.
