TL;DR: Of the muscle pumping sets examined, the calf pump is the most powerful, therefore perforator incompetence at this level is of the greatest importance in the condition of chronic venous insufficiency.
Abstract: A system of pumping units has been demonstrated in the calf, femoral and abdominal regions. This pumping action is produced by the direct action of muscles compressing the deep veins. The co-ordinated action of the leg muscles in the act of walking produces a synchronised pumping action which is of fundamental importance for the return of blood from the lower extremities to the heart. Valvular incompetence involving the perforating vein produces a definite embarrassment to the individual pump involved, and this embarrassment is of great clinical significance in chronic venous insufficiency. Of the muscle pumping sets examined, the calf pump is the most powerful, therefore perforator incompetence at this level is of the greatest importance in the condition of chronic venous insufficiency. A definite pumping action has also been demonstrated in the abdomen, which also affects the pressures in the leg pumps.
