Abstract: Self-management is of increasing importance in chronic obstructive pulmonary disease (COPD) management. However, there is confusion over what processes are involved, how the value of self-management should be determined, and about the research priorities. To gain more insight into and agreement about the content of programmes, outcomes, and future directions of COPD self-management, a group of interested researchers and physicians, all of whom had previously published on this subject and who had previously collaborated on other projects, convened a workshop. This article summarises their initial findings. Self-management programmes aim at structural behaviour change to sustain treatment effects after programmes have been completed. The programmes should include techniques aimed at behavioural change, be tailored individually, take the patient's perspective into account, and may vary with the course of the patient's disease and co-morbidities. Assessment should include process variables. This report is a step towards greater conformity in the field of self-management. To enhance clarity regarding effectiveness, future studies should clearly describe their intervention, be properly designed and powered, and include outcomes that focus more on the acquisition and practice of new skills. In this way more evidence and a better comprehension on self-management programmes will be obtained, and more specific formulation of guidelines on self-management made possible.

Abstract: Aim: To compare self-reported school-day sleep duration in 10- to 15-year-old South Australians between 1985 and 2004. Methods: Data were collected from 10- to 15-year-old participants in the 1985 Australian Schools Health and Fitness Survey (n = 390) and the 2004 South Australian Physical Activity Survey (n = 510). Identical self-report questionnaires were administered in both surveys, providing data on school-day bed-time, wake-time and sleep duration; as well as age, gender and socioeconomic status (SES). Analysis of co-variance (ANCOVA), controlling for age and SES, was used to compare all sleep variables between surveys. Results: Declines in sleep duration were found for both girls (28 min) and boys (33 min) between surveys. The reduction was more pronounced in ‘lower SES’ boys (44 min) than ‘higher SES’ boys (23 min). Boys reported later bed-time than girls in the 2004 survey, while no gender differences were apparent in the 1985 survey. Conclusion: Our findings suggest reductions in school-day sleep duration have occurred in Australian children and adolescents over the last 20 years, due largely to later bed-times. The physiological significance of these declines and mediating influences, such as SES, are yet to be explicated.

Abstract: Eight smooth muscle preparations from the stomach, small intestine and large intestine of the guinea-pig were used to compare apamin's actions in reducing the effectiveness of transmission from enteric inhibitory nerves and in reducing responses to inhibitory agonists alpha, beta-methylene ATP, VIP and isoprenaline. The effects of apamin on inhibitory reflexes in the ileum and colon were also evaluated. Apamin had little or no effect on responses to VIP and isoprenaline in any region, but consistently and substantially reduced responses to alpha, beta-methylene ATP. Responses to stimulation of enteric inhibitory neurons were substantially reduced by apamin in the antrum circular muscle, ileum longitudinal and circular muscle, taenia coli and distal colon longitudinal muscle, but it was ineffective in the fundus circular muscle, proximal colon longitudinal muscle and distal colon circular muscle. It caused a small reduction of the relaxation of the ileal circular muscle caused reflexly by distension, but did not modify the similar descending inhibitory reflex in the circular muscle of the colon. It is concluded that apamin can be used to distinguish two types of non-noradrenergic transmission from enteric inhibitory nerves to gastrointestinal muscle. Furthermore, neither VIP nor ATP can be the sole transmitter chemical released from enteric inhibitory neurons throughout the gastrointestinal tract.
