Abstract: Newly emerged molecular markers in gliomas provide prognostic values beyond the capabilities of histologic classification. BRAF mutation, especially BRAF V600E, is common in a subset of gliomas and may represent a potential prognostic marker. The aim of our study is to investigate the potential use of BRAF mutations on prognosis of glioma patients. Four electronic databases were searched for potential articles, including PubMed, Scopus, ISI Web of Science, and Virtual Health Library (VHL). Data of hazard ratio (HR) for overall survival (OS) and progression-free survival (PFS) were directly obtained from original papers or indirectly estimated from Kaplan Meier curve (KMC). A random effect model weighted by inverse variance method was used to calculate the pooled HR. From 705 articles, we finally included 11 articles with 1308 glioma patients for the final analysis. The overall estimates showed that BRAF V600E was associated with an improved overall survival (OS) in glioma patients (HR = 0.60; 95% CI = 0.44–0.80). Results for progression-free survival (PFS), however, were not statistically significant (HR = 1.39; 95% CI = 0.82–2.34). In subgroup analyses, BRAF V600E showed its effect in improving survival in pediatric and young adult gliomas (under 35 years) but did not have prognostic value in old adult. Additionally, BRAF V600E was only associated with a favorable prognosis in lower grade glioma. Our meta-analysis provides evidence that BRAF mutation has a favorable prognostic impact in gliomas and its prognostic value might be dependent on patient age and tumor grade. This mutation can be used as a prognostic factor in glioma but additional studies are required to clarify its prognostic value taking into account other confounding factors.

Abstract: Objective: Several retrospective studies with small cohorts reported neutrophil-to-lymphocyte ratio (NLR) as a prognostic marker in upper tract urothelial carcinoma (UTUC) following radical nephroureterectomy (RNU). We aimed at validating the predictive and prognostic role of NLR in a large multi-institutional cohort. Methods : Preoperative NLR was assessed in a multi-institutional cohort of 2477 patients with UTUC treated with RNU. Altered NLR was defined by a ratio >2.7. Logistic regression analyses were performed to assess the association between NLR and lymph node metastasis, muscle-invasive and non-organ-confined disease. The association of altered NLR with recurrence-free survival (RFS) and cancer-specific survival (CSS) was evaluated using Cox proportional hazards regression models. Results: Altered NLR was observed in 1428 (62.8 %) patients and associated with more advanced pathological tumor stage, lymph node metastasis, lymphovascular invasion, tumor necrosis and sessile tumor architecture. In a preoperative model that included age, gender, tumor location and architecture, NLR was an independent predictive factor for the presence of lymph node metastasis, muscle-invasive and non-organ-confined disease (p < 0.001). Within a median follow-up of 40 months (IQR 20–76 months), 548 (24.1 %) patients experienced disease recurrence and 453 patients (19.9 %) died from their cancer. Compared to patients with normal NLR, those with altered NLR had worse RFS (0.003) and CSS (p = 0.002). In multivariable analyses that adjusted for the effects of standard clinicopathologic features, altered NLR did not retain an independent value. In the subgroup of patients treated with lymphadenectomy in addition to RNU, NLR was independently associated with CSS (p = 0.03). Conclusion:In UTUC, preoperative NLR is associated with adverse clinicopathologic features and independently predicts features of biologically and clinically aggressive UTUC such as lymph node metastasis, muscle-invasive or non-organ-confined status. NLR may help better risk stratify patients with regard to lymphadenectomy and conservative therapy.

Abstract: We utilized polymerase chain reaction (PCR) to demonstrate that Angiostrongylus cantonensis was responsible for 67.3% of 55 cases of eosinophilic meningitis from a cohort of 1,690 adult patients with CNS infection at a tertiary hospital in southern Vietnam. Longer duration of illness, depressed consciousness, and peripheral blood eosinophilia were associated with PCR positivity.

Abstract: Purpose To determine modifiable maternal risk factors for adverse pregnancy, postpartum maternal and child health outcomes in Vietnam. Participants This prospective cohort study included pregnant women seeking prenatal care at six hospitals in three large cities in Vietnam. After enrolment, eligible participants who gave their consent to participate in the study were interviewed at 24–28 weeks9 gestation. Glucose testing was conducted and blood pressure was measured during this period. Each participant will be assessed prospectively during their postnatal visits at delivery, 1, 3, 6, 12, 18 and 24 months, and will be followed up for 5 years. Findings to date Of 2248 eligible pregnant women, 2030 were recruited (participation rate 90.3%) between August 2015 and July 2016. All participants completed the baseline assessment. Their mean (SD) age was 27.6 (5.3) years. The mean pre-pregnancy body mass index (BMI) was 20.2 (SD 2.6) kg/m 2 , with nearly two-thirds of participants having a normal pre-pregnancy BMI (18.5 to 2 ) and one-quarter being underweight (pre-pregnancy BMI 2 ). Overweight or obese mothers (pre-pregnancy BMI ≥23.0 kg/m 2 ) accounted for 12.8%. No pregnant women reported smoking during their pregnancy while 13.4% of them had continued drinking. 22.8% of participants had hyperglycaemia. Their mean systolic blood pressure was 105.6 (SD 8.2) mm Hg, and diastolic blood pressure was 67.4 (SD 7.5) mm Hg. Future plans The relationships of maternal lifestyle and nutritional status with the health outcomes of pregnancy, postpartum maternity and infants will be analysed. Meanwhile, participants will be closely tracked to minimise loss to follow-up.

Abstract: The Human respiratory syncytial virus (RSV) is one of the most important viral pathogens, causing epidemics of acute respiratory infection (ARI), especially bronchiolitis and pneumonia, in children worldwide. To investigate the RSV burden in Laos, we conducted a one-year study in children <5 years old admitted to Mahosot Hospital, Vientiane Capital, to describe clinical and epidemiological characteristics and predictive factors for severity of RSV-associated ARI. Pooled nasal and throat swabs were tested using multiplex real-time PCR for 33 respiratory pathogens (FTD® kit). A total of 383 patients were included, 277 (72.3%) of whom presented with pneumonia. 377 (98.4%) patients were positive for at least one microorganism, of which RSV was the most common virus (41.0%), with a peak observed between June and September, corresponding to the rainy season. Most RSV inpatients had pneumonia (84.1%), of whom 35% had severe pneumonia. Children <3-months old were a high-risk group for severe pneumonia, independently of RSV infection. Our study suggests that RSV infection is frequent in Laos and commonly associated with pneumonia in hospitalized young children. Further investigations are required to provide a better overall view of the Lao nationwide epidemiology and public health burden of RSV infection over time.

Abstract: Objective Our first objective was to report our experience on robot-assisted vesico-vaginal fistula (VVF) repair after radical surgery for gynecologic malignancies without omental flap interposition using a da Vinci robotic system. The second objective was to critically review the literature in order to analyze surgical solutions used to avoid repair failure. Materials and methods Five patients with VVF diagnosed after previous open surgery for gynecologic malignancies referred to our tertiary institutions were selected. After an adequate oncologic follow-up, based on the fistula diameter and conservative management failure, robotic surgery repair was proposed. A bivalve 2-layer suturing technique was carried out without tissue interpositions; omentum was not available secondary to previous surgery including hysterectomy, ovaryectomy, and omentectomy and citoreductive peritoneomectomy. A systematic review of the literature was performed in December 2016 using the PubMed database with the following keywords: robotic, robot-assisted, vesico-vaginal, fistula repair. Results Median age was 62 years (range 55-71) bearing long-lasting VVF were referred to our divisions. Median fistula diameter was 5 mm (range 3-8 mm). Fistula site was the trigone and identified during cystoscopy near the mid-line, left, and right urether meatus, respectively. The median overall and console operatory time were 250 and 120 min, respectively. Blood loss was insignificant (median 40 mL) and the median length of stay was 7 days without any complication. Ten papers were found fulfilling the mentioned criteria, from which 6 were case reports, single or multiple, accounting for the overall 41 robotic-approach-operated patients. Conclusion The quality of the dissection and suture associated with efficient urine drainage are in our opinion the key elements of the success of our technique, which can be performed even without omentum or other tissue flap or graft interposition.

Abstract: We previously reported a significant reduction in the prevalence of human immunodeficiency virus type 1 (HIV) from 2007 to 2012 in people who inject drugs (PWID; 35.9% to 18.5%, p < 0.001) and female sex workers (FSW; 23.1% to 9.8%, p < 0.05), but not in blood donors (BD) or pregnant women, in Haiphong, Vietnam. Our aim in the present study was to assess trends in the prevalence of infection with hepatitis B and C viruses (HBV and HCV, respectively). We also investigated the coinfection rates of HBV and HCV with HIV in the same groups. Between 2007 and 2012, HBV prevalence was significantly decreased in BD (18.1% vs. 9.0%, p = 0.007) and slightly decreased in FSW (11.0% vs. 3.9%, p = 0.21), but not in PWID (10.7% vs. 11.1%, p = 0.84). HCV prevalence was significantly decreased in PWID (62.1% in 2007 vs. 42.7% in 2008, p < 0.0001), but it had rebounded to 58.4% in 2012 (2008 vs. 2012, p < 0.0001). HCV prevalence also increased in FSW: 28.6% in 2007 and 2009 vs. 35.3% in 2012; however, this difference was not significant (2007 vs. 2012, p = 0.41). Rates of coinfection with HBV and HCV among HIV-infected PWID and FSW did not change significantly during the study period. Our findings suggest that the current harm reduction programs designed to prevent HIV transmission in PWID and FSW may be insufficient to prevent the transmission of hepatitis viruses, particularly HCV, in Haiphong, Vietnam. New approaches, such as the introduction of catch-up HBV vaccination to vulnerable adult populations and the introduction of HCV treatment as prevention, should be considered to reduce morbidity and mortality due to HIV and hepatitis virus coinfection in Vietnam.

Abstract: The aim of this study was to present an epidemiological update regarding the classical prognostic parameters of gastric cancer (GC) in 3 countries from Eastern Europe and to suggest a modification of the pTNM staging system. In 333 consecutive cases which were diagnosed between 2003 and 2012 in 3 departments of pathology from Romania, Hungary, and Poland, the following parameters were analyzed: age and gender of patients, tumor localization, macroscopic and microscopic aspects including the degree of discohesivity, depth of tumor infiltration, and pTNM stage. From all of the studied parameters, the following proved to have independent prognostic value, indicating a lower survival rate: presence of distant metastases (p=0.001), lymph node positivity (p=0.0009), depth of tumor infiltration (p=0.04), age over 50 (p=0.02), proximally located tumors (p=0.03), and ulceroinfiltrative or diffusely infiltrative macroscopic aspect (p=0.0002). The pT2N1-3 staged cases showed a worse prognosis compared with the pT3N0 ones (p=0.02). Regardless of depth of invasion, the lymph node status remains the strongest indicator of the survival rate in GC. The pTN staging system should be adapted and a Dukes-MAC-like staging system should include the following groups: stage A1—T1N0, stage A2—T1N1-3, stage B1—T2N0, stage B2—T2N1-3, stage C1—T3N0, stage C2—T3N1-3, and stage D—T4N0-3. The grade of discohesivity/budding is not a prognostic factor in GC.

Abstract: Ti6Al4V alloy is widely used for biomedical implants, and the modification of its surface, at nanoscale level, is a method to enhance its osseointegration. We modified the Ti6Al4V surface, with an initial micro rough topography, by using electrochemical anodization in 1M H3PO4 + 0.5 wt% HF electrolyte, and anodization potential of 20 V. By using short anodization duration, 5 min, or 15 min, the oxide layer presents a nanoporous morphology, and do not cover the entire surface. At an anodization time of 30 min, our results demonstrate the synthesis of continuous, highly ordered nanotubular TiO2 layer (diameter of nanotubes: 25-90 nm, thickness of the layer: 350-450 nm), superimposed over a micro rough topography, the oxide layer exhibiting a good adherence to the substrate, coherence and no brittleness. Anodization time of 2 hours provides the results very similar with those obtained at 30 min in terms of nanotubular layer topography, but some drawbacks appear in terms of surface coverage and continuity of the layer.

Abstract: To present the morphological changes of classic primary versus rapidly progressive and secondary hip osteoarthritis (HO) and to examine the expression of two pluripotent mesenchymal stem cell-like markers in the synovial membrane. A prospective observational study was conducted in 57 consecutive cases of radiologically confirmed HO in which total hip arthroplasty was performed. Based on the radiological and clinicopathological features, the cases were divided into three categories: classic primary HO (group A; n=16), rapidly destructive HO (group B; n=24), and HO secondary to avascular osteonecrosis of the femoral head (group C; n=17). Immunostains were performed using the markers CD44 and CD105. The cases from group A were mainly characterized by a marked perivascular inflammatory infiltrate and simple synovial hyperplasia. In group B, the papillary type of synovial hyperplasia was found and presence of chondromatosis, ossification, and ectopic follicles with germinal centers in the subsynovial layer was characteristic, whereas marked calcification and/or ossification were seen in group C. Focal expression of the CD105 and CD44 was noted in the hyperplastic synovial cells and subsynovial layer in cases from group A, whereas synovial cells from group B were diffusely positive for both CD44 and CD105. In secondary HO, CD44 marked the inflammatory cells. Mobilization of the CD44/CD105 positive synovial cells seems to play a role in the genesis of HO. The number of the pluripotent mesenchymal stem cell-like cells derived from the hyperplastic synovial cells might be related to the severity of possible immune-mediated rapidly destructive HO.

Abstract: In this study, we developed a simple and convenient one-step method for activation of graphite using high basic red mud slurry to produce a mixture of red mud/graphene (REEG) under mild reaction conditions. The wet red mud was mixed up with graphene by direct electrochemical exfoliation of graphite in its solution. The REEG obtained was characterized and applied for removal of Cadmium (Cd(II)) from aqueous solutions. The effects of pH, contact time, and initial concentrations on the removal of Cd(II) were investigated. A removal efficiency of 98.95% at pH 6 was obtained after 60 min of adsorption, via the dispersion of 0.1 g of REEG in Cd(II) stock solutions (50 mL, 1 ppm). The maximum adsorption capacity (qmax) was calculated to be 12.598 mg/g. Results showed that REEG is effective for removal of Cd(II) from aqueous environment, which could also act as an effective adsorbent for the removal of other heavy metals from polluted water.

Abstract: Recently, there has been increasing trend in the use of laser therapy at points of acupuncture as an alternative method to metal needles. Many researchers conducted the number of studies on laser acupuncture. Most of them used many kinds of laser with different wavelengths, or power in order to show the impacts on healing pains. Besides, many clinical doctors applied laser therapy for a variety of treatment purposes. However, it still lacks evidence to prove the actual mechanism, and the effects on brain in detail. In this study, the authors aim to investigate how laser acupuncture at acupoints effects on human brain by measuring the electroencephalogram (EEG) signal. To carry out the research, the authors initially are going to use acupuncture needles at acupoints on twenty participants. Then, the laser therapy will be replaced in order to compare its effect to the previous one on human brain. During the experiments, Alice 5 machine is used for collecting and analyzing the EEG signal. Finally, the authors expect that when the acupoints are reached by a laser light, the response of brain will be recognized with producing the same EEG signals with the acupuncture needles.

Abstract: Access control is one of the most efficient ways to restrict resource access violations. However, in software development process, programs may not be fully complied with access policies represented in specifications. In this paper, we present an approach to verify the access policies compliance between a SecureUML model and its software program. We extract access control rules specified in SecureUML model, analyze source code of its program and propose an algorithm to check the compliance between two paradigms. Our approach can help programmers to detect some resource access violations and to improve the quality of software systems.

Abstract: Internal hernias are defined as the protrusion of abdominal organs, especially the small intestine, into various intraabdominal or retroperitoneal cavities, recesses or fossae through natural or acquired orifices (foramen). Many types of internal hernias have been described, but according to Ghahremani [1] these can be grouped as follows: paraduodenal hernias (Treitz), hernias through the foramen of Winslow, transmesenteric (Petersen) hernias, pericecal (Rieux) hernias, intersigmoid hernias and paravesical hernias. Although rare, internal hernias are encumbered by increased morbidity and mortality [2], primarily due to the difficulty of early diagnosis, before the onset of intestinal occlusion by incarceration and strangulation followed then by intestinal necrosis and peritonitis. Petersen type internal hernias are more common nowadays following the development of the laparoscopic approach, especially in bariatric surgery, due to the inappropriate closure of mesentery defects, but in this case, the diagnosis is somewhat facilitated by a higher degree of suspicion [3]. Early diagnosis of an internal hernia is quite difficult, especially in the congenital type, but it is essential in saving the patient’s life. A CT scan may be of genuine help if the investigator has knowledge of the imaging features of internal hernias and their mechanism [4, 5]. Herein, we present a case of an internal complicated hernia produced in a space between two vertebrae. A 69-year-old male patient, without any prior abdominal surgery, had suffered multiple traumas by falling from a horse-drawn cart. He was hospitalized in the intensive care unit of another hospital with the diagnosis of polytrauma with head injury, orbital bone fracture, right thoracic trauma with rib fractures, and spine trauma with fractures of spinous processes and olisthesis of vertebrae L3 and L4. CT investigation highlighted bone fractures and also revealed the presence of air in the retroperitoneal and epidural spaces, raising the suspicion of posttraumatic duodenal injury (Fig. 1a, b). Because of the unfavorable evolution with the emergence of the clinical picture of peritonitis, after 4 days, the patient was transferred to the surgery department in our hospital. Clinical examination revealed signs of generalized peritonitis. The patient underwent emergency surgery by median laparotomy. The intraoperative surprise was that of an internal hernia, the first jejunal loop being incarcerated with necrosis and perforation rupture into a cavity between the corpus of vertebrae L3 and L4. (Fig. 1c) The affected intestinal segment was resected, followed by a side-to-side anastomosis. Orthopedic consultation recommended only brace treatment and the postoperative evolution was uneventful, the patient being discharged after 3 weeks of hospitalization. The patient recovered well but 8 months later he was operated for an incisional hernia. We have conducted a thorough search of the literature to find any publications regarding this type of hernia but we failed to find any. It is unclear whether a chronic gap between L3 and L4 with herniation of the jejunum was present before, or the herniation occurred as a result of the acute traumatic event.
