Abstract: espanolEn los marcos del proyecto de evaluacion y diagnostico del patrimonio arqueologico y sociocultural de Cuba, desarrollado por un colectivo del Departamento de Arqueologia del Instituto Cubano de Antropologia (ICAN), se efectuo una expedicion de conjunto con miembros del Grupo Cubano de Investigaciones del Arte Rupestre (GCIAR) para evaluar los impactos medioambientales que estan deteriorando al dibujo rupestre de esta provincia. En este sentido de manera aleatoria de las 37 estaciones que posee la region se seleccionaron 17, lo que representa el 45,9%), para conocer el estado de conservacion de las mismas, mediante la aplicacion del metodo de evaluacion y diagnostico del patrimonio arqueologico que aplica nuestra institucion. La referida metodologia permite clasificar las acciones en naturales y antropicas para la mejor busqueda de soluciones que permitan eliminarlas o la ejecucion de acciones paliativas para frenar, en alguna medida, su deterioro. Para ello en el trabajo se definen, enumeran y caracterizan las acciones y los impactos que mas perjudican el estado de conservacion de este importante patrimonio EnglishIn the marks of the research project ‘Evaluation and diagnosis of the archaeological and sociocultural patrimony of Cuba’, developed by a group of the Department of Archaeology of the Cuban Institute of Anthropology (ICAN), an expedition wasmade joined to members of the Cuban Rock Art Research Group (GCIAR) to evaluate the environmental impacts that are affecting the rock drawings of Pinar del Rio province. Seventeen stations of 37 in that area were randomly selected to apply the evaluation and diagnosis methodology of environmental impacts that is applied by ICAN. The applied method allows classifying the natural and anthropic actions for the best search of solutions to eliminate the damage, or the practice of palliative actions to stop, in some way, its deterioration. So, the actions and impacts that affect in a deeper way the state of conservation of this Cuban important heritage are defined, enumerated and characterized in this paper.

Abstract: RESUME Les indications de l’assistance circulatoire mecanique dans le traitement de l’insuffisance cardiaque chronique sont de trois types : (a) l’attente de transplantation chez des patients en insuffisance cardiaque severe ; (b)l’attente de recuperation des patients presentant une situation de detresse en relation avec une pathologie aigue ; (c)l’assistance definitive du fait de la difficulte a obtenir un nombre suffisant de greffons pour repondre aux besoins de la population. Les criteres classiques, eligibilite, correspondent a une insuffisance cardiaque severe, resistante aux therapies disponibles. Ils sont consideres comme trop tardifs pour beaucoup et, actuellement on insiste sur la necessite d’envisager une indication chez les patients qui deviennent dependants des inotropes positifs ou chez lesquels, afin de maintenir une situation hemodynamique precaire, il est necessaire d’augmenter ses doses d’inotropes. Suivant les situations, le choix se portera sur des ventricules pneumatiques para-corporels, des ventricules pneumatiques intra-corporelles ou des ventricules electromecaniques. Si il existe des signes de defaillance ventriculaire droite, une assistance bi-ventriculaire est necessaire. En son absence, une assistance univentriculaire gauche est proposee. Les progres realises dans la technologie des systemes d’assistance mecanique ont permis de reduire significativement la mortalite en attente de transplantation. Il ne faut toutefois pas sous-estimer les complications a type de thrombose, saignement, accidents vasculaires cerebraux, infection. Le cout des dispositifs impose egalement un respect strict des contreindications.

Abstract: In a recent issue of Intensive Care Medicine , Ouweneel et al . provided a meta-analysis on extracorporeal life support (ECLS) during cardiac arrest (CA) and cardiogenic shock (1). Cardiogenic shock analysis compared ECLS vs. Impella, TandemHeart, or intra-aortic balloon pump (IABP). As these devices have various support levels, different specifications, and therefore different clinical indications, results issued from this analysis are clinically questionable. In addition, based on the recent IABP Shock II trial (2), current European guidelines on cardiogenic shock no longer support routine IABP therapy use, except for mechanical complications (class IIaC). As reported by the authors, caution is required in interpreting this part of the meta-analysis and our interest was therefore focused on the evaluation of ECLS during CA.
