Abstract: White adipose tissue is composed of mature adipocytes, precursors (preadipocytes), endothelial cells, macrophages, and other immune cells The phenotype, amount, and biology of each adipose tissue component are profoundly altered in human obesity Low-grade inflammation both at the local and systemic levels characterizes obesity and appears to have a key role in mediating the consequence of increased adipose tissue mass on metabolic and vascular co-morbidities Moreover, a new concept of accumulation of extracellular matrix (ECM) components in adipose tissue has been recently proposed Integrity of the ECM is essential for the proper development of AT in a physiological context In pathologic conditions, more knowledge is nevertheless necessary to understand the contribution of ECM modifications and the consequence of fibrous depots Observations in morbidly obese subjects suggest that the amount of fibrosis might hamper fat mass loss induced by surgery

Abstract: Patients suffering from interstitial lung disease (ILD) share a number of similarities with chronic obstructive pulmonary disease (COPD) patients. The natural evolution in both patient groups is a slow deterioration of lung capacity punctuated by exacerbations, and available treatments are mostly aimed at sustaining lung capacity and preventing exacerbations. Additionally, the last treatment option is lung transplantation (LTx), which is usually performed, in chronic, stable patients. However, management discrepancies are noted between the two patient groups regarding intensive care unit (ICU) admission policy.

Abstract: Resume Introduction L’objet de cet article est de contribuer a mieux definir la transition des patients, porteurs de pathologies endocriniennes chroniques des services de pediatrie vers les services d’adultes. Il s’agit de preciser les moyens et les competences qu’elle requiert chez l’adolescent et les equipes soignantes, afin de proposer quelques modalites pouvant contribuer a garantir une meilleure qualite de la prise en charge de ces jeunes patients. Methodes Ce programme s’appuie sur les mouvements psychiques propres a l’adolescence. Des seances de groupe sont proposees lors d’une premiere journee puis a distance des entretiens individuels. Une analyse thematique des seances et les reponses a des questionnaires ont permis l’elaboration de diagnostic educatif individuel et d’evaluer l’autonomie des jeunes adolescents. Resultats Cinquante jeunes patients ayant des pathologies rares sur 214 adresses par les services pediatriques ont participe au programme d’education therapeutique (ETP) – transition entre le 1er janvier 2011 et le 30 septembre 2014. Un an apres cette journee ETP, 48 patients sont venus aux consultations et on observe chez eux une evolution dans leur autonomie. Conclusion Il reste des progres a realiser quant a l’organisation de ce programme d’ETP et en particulier sur les modalites d’inclusion des jeunes patients dans ce programme transition.
