Abstract: Andersen's syndrome is a clinically distinct form of potassium-sensitive periodic paralysis associated with cardiac dysrhythmias. The subtle nature of the cardiac and dysmorphic features may delay the recognition of this syndrome and its potentially lethal cardiac dysrhythmias. The genetic defect in Andersen's syndrome is not genetically linked to other forms of potassium-sensitive periodic paralysis and is probably distinct from the long QT syndrome locus.

Abstract: We assessed the incidence of hip fracture and ecological correlates in residents of 14 communities in six countries of Southern Europe. Hip fracture cases were recorded prospectively in defined catchment areas over a 1-year interval. A retrospective questionnaire was used to assess ecological differences between communities. During a 1-year period of observation a total of 3629 men and women over the age of 50 years were identified with hip fracture from a catchment of 3 million. In all communities the fracture rate increased exponentially with age. There were large and significant differences between centres in the doubling time for hip fracture risk with age and in crude and age-standardized rates. Greater than 4-fold and 13-fold differences in age-standardized risk were found amongst men and women respectively. The lowest rates were observed from Turkey and the highest from Seville, Crete and Porto. Fractures were significantly more frequent among women than men with the exception of three rural Turkish centres. Indeed, in rural Turkey the normal female/male ratio was reserved. Variations in incidence between regions were greater than the differences within centres between sexes, and there was a close and significant correlation between incidence rates for men and those for women in the regions studied. Excess female morbidity increased progressively from the age of 50 years but attained a plateau after the age of 80 years, suggesting a finite duration of the effect of the menopause. The retrospective questionnaire completed by 80% of cases suggested that differences in incidence between the communities studied could not be explained by differences in gonadal status in women. In both men and women cross-cultural associations were found with factors related to age or socioeconomic prosperity, the majority of which disappeared after adjustment for age. We conclude that there are marked and sizeable differences in the incidence rates of hip fracture throughout Southern Europe. The reasons for these differences are not known but affect both men and women, and are likely to be related to lifestyle or genetic factors rather than to differences in endocrine status.

Abstract: OBJECTIVE To test the efficacy of systemic interferon alpha 2b (IFN alpha 2b) treatment in the mucocutaneous and joint symptoms of Behcet's syndrome. METHODS The 48-week open, self-controlled trial was conducted in 3 phases. After the pretreatment phase of 16 weeks, 20 patients (8 men, 12 women; mean age 37 +/- 7 years SD) were treated with IFN-alpha 2b at a dose of 5 million units 3 times a week for 6 weeks followed by 5 million units once a week for 10 weeks. The subsequent 16 weeks not taking the medication was the posttreatment phase. RESULTS Treatment with IFN-alpha 2b significantly reduced the mean number of arthritis attacks (F = 3.48; p < 0.05), their mean duration (F = 3.77; p < 0.05), and the mean erythrocyte sedimentation rate (F = 6.66; p < 0.001). The mean number of mucocutaneous lesions also showed a decrease, but this was not statistically significant. Except for the duration of arthritis, the mean number and frequency of all symptoms tended to return to pretreatment levels in the posttreatment phase. CONCLUSION This pilot study shows that systemic IFN may be effective in the treatment of arthritis of Behcet's syndrome.

Abstract: PURPOSE: To describe findings at radiologic evaluation of pulmonary arterial involvement in Behcet disease. MATERIALS AND METHODS: Between 1978 and 1993, 31 pulmonary arterial aneurysms were diagnosed in 15 consecutive male patients, aged 21-56 years (mean, 32 years 5 months), with known Behcet disease. The patients were assessed at chest radiography (n = 15), computed tomography (n = 14), angiography (n = 10), and magnetic resonance (MR) imaging (n = 2). RESULTS: Three patients had single and 12 had multiple pulmonary aneurysms. In six patients, aneurysms were associated with pulmonary artery occlusion. Follow-up chest radiography revealed five subjects with progression leading to fatal rupture of aneurysms and two subjects with regression of the aneurysms and cessation of their symptoms. Abstract: A 3½ year old boy is presented with postaxial hexodactily in all extremities, a persisting large left vena cava superior (VCS) draining into the left part of a single atrium and a mild dextroversio cordis. All extra fingers and the lateral parts of the fused metatarsals were amputated. At age 10, the cardiovascular anomalies were surgically corrected successfully by separating the atrium with a pericardial patch and by draining the left VCS to the right atrium with a graft. The patient seems to represent a new syndrome, since the combination of postaxial hexodactily with single atrium had previously been observed by the author in another patient in 1970.

Abstract: 115 traumatic extradural haematoma cases who were treated surgically at Cerrahpasa Medical Faculty Neurosurgery Department between 1987 and 1992 are evaluated. When factors affecting the outcome were examined, a strong correlation was found between the result and Glasgow coma scale (GCS) (p < 0.00001). The existence of a fracture, the interval between onset of haematoma symptoms and intervention and the existence of an intracerebral haematoma together with contusion accompanying intradural haematoma, affect the outcome in a negative direction. There was no statistical correlation between the outcome and the age of patient, localization of the haematoma and aetiology.

Abstract: Forty patients with typical obstetric history of Sheehan's syndrome were reviewed retrospectively. Together with baseline laboratory values, insulin hypoglycemia test was evaluated in 15 patients, thyrotropin-releasing hormone (TRH) in 27 and luteinizing hormone-releasing hormone (LH-RH) in 7 patients. Baseline hormone values suggested secondary hypothyroidism, hypogonadotropic hypogonadism and hypocortisolemia. According to the results of the anterior pituitary stimulation tests, one patient (6.6%) showed normal cortisol response and one patient (6.6%) showed normal growth hormone response to hypoglycemia. Nine patients (33.3%) who were clinically and biochemically hypothyroid demonstrated adequate TSH response to TRH. None of the patients showed normal prolactin response to TRH. Four out of seven amenorrheic patients (57.1%) had adequate follicle-stimulating hormone and/or LH responses to LH-RH. It has been concluded that isolated anterior pituitary hormone deficiencies may occur in patients with Sheehan's syndrome. Prolactin response to TRH seems the most sensitive screening test for detecting Sheehan's syndrome in patients with typical obstetric history.

Abstract: Inflammatory periodontal diseases are related to dental plaque formation. Increase in the perfusion of the inflamed tissue results in increased oxygen supply. Although oxygen has healing effects, it is bound to be a mediator of peroxidation in biological membranes. Chemotherapeutic agents such as chlorhexidine, listerine, sanguinarine, and cetylpridinium chloride and oral antibiotics such as tetracycline HC1 and doxycyline were tested for their antioxidative activities. While doxycyline has the highest antioxidant activity in lower volumes (0.1 ml), sanguinarine, listerine and a pace after them, tetracycline HC1, had similar effects in higher volumes (0.3 and 0.4 ml). The results showed that in addition to their antiseptic or antimicrobial effects, these preparations have an antioxidative activity against spontaneous oxidation.

Abstract: The apical seal produced by low-temperature injection of gutta-percha, with or without root canal sealer, in the presence or absence of the smear layer was evaluated quantitatively using an electrochemical technique. The root canals of 44 teeth were prepared with the smear layer intact following a final flush of NaOCl (group 1), while the root canals of a further 44 teeth were prepared and the smear layer removed with a final flush with EDTA followed by NaOCl (group 2). Subgroups were obturated using Ultrafil System alone or accompanied by Calciobiotic Root Canal Sealer. Microleakage data was expressed as a percentage of apical leakage evident in each group. Statistical analysis of the results revealed that the groups obturated with Ultrafil alone showed no significant differences in leakage compared with the groups where sealer was used, either when the smear layer had been left intact or removed. (P > 0.05). However, comparison of the combined two groups with smear layer present versus the combined two groups with smear layer removed showed a highly significant difference (P < 0.01), with the incidence of leakage reduced in the absence of the smear layer.

Abstract: Previous studies have shown little agreement between the test methods used to assess the bond strength and the mode of failure of resilient liners. This study evaluated the bond strength characteristics of resilient liners by means of 180-degree peeling and butt tensile strength testing. Seventy-two specimens were divided into peel bond and tensile bond specimen groups and were then subdivided into four test groups to evaluate each resilient liner. Tests were conducted with an Instron universal testing machine at a cross-head speed of 2 mm/minute for the tensile specimens and 5 mm/min for the peel specimens. Tensile bond strength and peel bond strength varied significantly among resilient liners except between Novus and Palasiv-62 liners in tensile testing. The mode of failure of Molloplast-B and Novus liners was significantly different between the tensile bond and peel bond test methods. It was concluded that bond strength characteristics can vary according to the test method used.

Complexation of the tetrathiamacrocycles of 7 and 8 with PdII or AgI to form pentanuclear products was accomplished from their suspensions.

Abstract: The new vic-dioxime ligand 2 (LH4) carrying a 14-membered N2O2 macrocycle has been prepared. Mononuclear complex (LH3)2Ni, trinuclear complexes (LH)2Cu3, (LH)2Co3, (LH)2-Pd3, and (LH)2Co3(py)3Cl3, dinuclear complex (LH3)2-(UO2)2(OH)2, and pentanuclear complex [L2Cu5(phen)2]-(NO3)2 have been synthesized from LH4. The electrochemical properties of tri- and pentanuclear complexes were investigated by cyclic voltammetry. For (LH)2Cu3 two reduction waves appeared at 0.215 and -0.870 V. [L2Cu5(phen)2](NO3)2 exhibited four reduction waves at 0.200, -0.025, -0.670, and -1.170 V.

Abstract: Glutathione levels were found to be decreased while lipid peroxide levels were increased in total liver homogenates 6 h following paracetamol treatment (500 mg kg-1 i.p.). Furthermore, it has been determined that cytosolic glutathione S-transferase (GST) activity was decreased and glutathione peroxidase (GSH-Px) activity remained unchanged. On the other hand, a decrease in liver microsomal lipid peroxide levels and an increase in GST and GSH-Px activity has been observed. We concluded that decreased lipid peroxide levels in microsomes could be a consequence of increased GSH-Px and GST enzyme activities. In this way, these glutathione-related defence enzyme systems may play an important role in protecting microsomes from lipid peroxidation.

Abstract: There are conflicting reports about the natural history and prognosis of bladder tumours in patients under 40 years of age. A review of 156 patients younger than 40 treated at our Department between 1960 and 1991 with transitional cell carcinoma of the bladder revealed that 89.1% had superficial (Ta/T1) disease and the remaining 10.9% presented with invasive disease. Slightly more than half of the patients with superficial disease had multiple tumours. The follow-up of 97 patients over a period of 12-372 months revealed that there was a recurrence rate of 10.3% and 38.4% for Ta and T1 tumours, respectively (p < 0.01). Further analysis comparing patients under 30 to those between 30 and 40 years revealed recurrence rates of 7.5% and 22%, respectively (p < 0.05). Progression rates for Ta and T1 tumours are 3.5% and 19.3% (p < 0.05). In the invasive disease group 8 patients were lost for follow-up, 2 died of the disease and the remaining 7 are alive, with a mean follow-up of 3.6 years. We conclude that while transitional cell carcinoma of the bladder in patients under 30 behave less aggressively, the behaviour of the disease in patients 30 to 40 years old is similar to the older age group and should be monitored closely, especially when risk factors for recurrence and progression are present.

Abstract: Crude extracts of Ruta chalepensis and R. chalepensis var. latifolia showed antifertility activity in female rats. In order to establish the active constituents which may be responsible for this effect, a group of selected coumarins and alkaloids were tested in vivo. Although chalepensin, bergaptol, chalepin and xanthatoxin showed antifertility activity, 70% of the test animals developed cystic and atretic follicles in their ovaries and glomerulocapsular adhesion and segmental fusion was observed in the kidneys. No harmful effect was observed in the brain. Alkaloids in this study showed no antifertility activity.
