Abstract: In this paper, we proposed to design a new model of mechanical ventilator based on the Ambu bag automation for the patient who is unable to take breath normally. Here we have automated an Ambu bag for air supply whose inlet is connected with an oxygen cylinder and environmental air and outlet is connected to lung patient. The project device includes a robotic operator which can operate an Ambu bag continuously by compressing and decompressing it. The robotic operator is a Computer-Aided Manufacturing (CAM) arm that is controlled by a single microcontroller for operating on the Ambu bag from outside. It has a great advantage of using a single adult Ambu bag to deliver necessary air to all aged lung patients by setting different controlling modes with respect to age with reducing the necessity of pediatric Ambu bag and infant Ambu bag. By considering all of the physiological parameters, we have added three modes namely Adult mode, Pediatric mode, and Child mode. Each mode is included by different respiratory rate and tidal volume to be friendly with their corresponding subject. The proposed device can detect the air pressure and temperature from the Ambu bag outlet to make feedback for preventing the lung harm of the patient and display the parameters using an LCD. All medical data can be transferred via a communication protocol to an Android or iOS phone for telemedicine purposes in real-time. The overall system is portable, small in size (45cm×25cm×35cm), low weighted, time-efficient, and cost-effective. There is no need for training or the study of an operator about the proposed system to handle the device for the benefit of automation of the device.

Abstract: Enormous studies have been conducted worldwide regarding CagA + status of H. pylori and patients habits in gastric carcinoma. But no study has been carried out in our country yet. Thus, this study has been designed to see the association between CagA + H. pylori strain and patients habits with gastric carcinoma. For this purpose, a total number of 80 (eighty) patients were selected. Of the 80 (eighty) patients 40 (forty) were selected as cases (malignant) and the remainder 40 (forty) were selected as controls (non-malignant). H. pylori was detected by applying noninvasive ( H. pylori IgG serology and CagA IgG serology) and invasive (Histology and rapid urease test) technique. Of them Histology was done by Modified Giemsa stain, CagA IgG was detected by ELISA method. In this study, we see that among the 40 cases, 35 (thirty five) possess the CagA + H. pylori strain. And among the 40 controls, 33 (thirty three) bear the CagA + H. pylori strain. This study also discloses that among cases, who were habituated with betel leaf and betel-nut ranking the highest number of gastric carcinoma. There are sufficient papers in favor of it which argues that CagA positivity as well as H. pylori positivity is not the sole causative agent of gastric carcinoma. Abstract: Meigs syndrome is defined as the co-existence of benign ovarian tumor with ascites and hydrothorax that resolves after resection of tumor. The ovarian tumor in Meigs syndrome is fibroma. On the contrary, Pseudo- Meigs syndrome is characterized by the co-existence of hydrothorax, ascites and ovarian tumor either benign or malignant other than ovarian fibroma. The case herein concerns a 70 year old postmenopausal woman presented with the complaints of progressive shortness of breath on exertion and progressive swelling of lower abdomen. Her clinical and radiological examination revealed a massive right sided pleural effusion, mild ascites and a large heterogeneous pelvic mass with solid and cystic components, suggestive of left ovarian tumor. The patient underwent an exploratory laparotomy with excision of tumor, right ovary and whole of the uterus. The tumor was diagnosed histologically as an ovarian mucinous ystadenoma. Abstract: Background Parasomnias are undesirable events occurring in the sleep-wake transition period. Several predisposing factors are reported to induce parasomnia in preschool children. Objective To es timate the magnitude of parasomnia in school children and to evaluate its relationship with possible predisposing factors . Methods Five hundred children aged 5- 16 years from a boys' school and a girls' school in Khulna City, Ban gladesh, were randomly selected for the study conducted fromJuly to December 2011. The survey was don e in two steps: self-administered questionnaire and clinical interviews of affected students and their parents. Apart from demographic features, questionnaires included details of perinatal and personal fac tors as well as familial and socioeconomic factors . The diagnoses of variants of parasomnias was based on the criteria for category-based classification by the American Academy of Sleep Medicine. Results Seven hundred thir teen filled questionnaires revealed parasomnia in 187 (26.2%) children. Most parasomnias were accompanied by other sleep disorders, in which 23 (12.3%) having primary dyssomnias including 18 (9.3%) obstructive sleep apnea, and 10 (5.3%) parasomnias with hypersomnias . Nightmares (7. 4%) were highest among the parasomnias fo llowed by nocturnal enuresis (4.1 %) and sleep terrors (3 .4%). More girls experienced parasomnias than boys (107 /360 vs . 80/353, respectively; P= 0.039) . Perinatal factors such as problems during pregnancy (17 .1 %) or eventful delivery (25. 7%), and socioeconomic factors such as familial disharmony (11.8%) and low socioeconomiclevel (3 1.6%) had positive associations with parasomnia. Conclusion One-quarter of school children experience parasomnia. We found perinatal factors particularly problem during pregnancy, and socioeconomic factors particularly familial disharmony have significant influences on this condition.
