Abstract: Background: One of the common medical emergencies is Snake-bite. Many patients die every year due to snakebite. However, the demographic feature, clinical manifestations, mortality and morbidity from snake-bite in our country has not been well studied. This study analyses demographic feature, clinical manifestations and outcomes of the snake bite poisoning in patients admitted to a medical college hospital. Aims & Objectives: To determine the demographic features, clinical manifestations and outcomes of the snake bite poisoning. Materials and Methods: This is an observational study done in the department of Medicine,Khulna Medical College Hospital from 1st July 2017 to 30th June 2018. Patients aged 13 years and above admitted with the suspicion of snake bite during the period were selected. Results: Out of 114 admitted snakebite patient most of the patients 50 (43.85%) are of younger age groups (13-40 years). Most bites occurred in rural area 108 (94.73%) and 74 (64.91%%) occurred between the months of July to September. Most cases 70 (61.40%) are non-venomous snake bites; krait was the main culprit for all venomous 44 (38.59%) cases showing neurotoxic features. Foot was the most common (42.10%) site of bite. ptosis (38.59%) and broken neck sign (17.54%) were the two most common clinical features. All venomous cases received 10-20 vials of anti-venom. 31.81% (14) of venomous patients developed respiratory failure requiring mechanical ventilation; of them 6 (13%) died. Conclusions: Snakebite is one of common emergency that endangers life in the study area. Treatment by Ohzas is very common and this causes Delay in hospitalization resulting in respiratory failure and increases mortality.

Abstract: A 27 years old women para -1 had a complaint of dullache pain in right iliac fossa for 5 days with irregular spotting. Her previous menstrual cycle was normal but in last one month she had an irregular spotting. On examination she was normotensive. Hb% was 65%. there was no tenderness during per abdominal examination but tenderness during per vaginal examination on right side. Transvaginal ultrasonography (TUVS) detected a degenerative sub serous fibroid of 3.5 x 3 cm on the right fundoanterior side, both adnexae were normal, no fluid was present in cul-de-sac. Her pregnancy test was positive but signs and symptoms of pregnancy were absent. Operative plan was intervention by laparoscope. An unruptured interstitial ectopic pregnancy was found at the anterolateral wall of the fundus. Laparoscopic excision was done and the uterine wall sutured with 2/0 polyprolylin. Abstract: Splenic cyst is an uncommon problem in surgical practice. It is usually diagnosed in scanning procedure. In some cases it produces symptoms like abdominal mass, abdominal pain, distention, constipation either due to haemorrhage, rupture or big size. In our case, patient presents with abdominal pain, distention, vomiting, constipation and respiratory embarrassment. Routine investigation was done and diagnosis was confirmed after laparotomy, Splenectomy was done. Post operative recovery was good. Bang Med J (Khulna) 2016; 49 : 34-36

Abstract: Intracerebral hemorrhage (ICH) constitutes 10% to 15% of all strokes. Within 30 days reported mortality is 35-52% and only 20% is functionally independent in 6 months. Despite several existing outcome prediction models for ICH, modified Rankin scale is found to be best predictor of outcome in early and long term period. To find out 30-day mortality in ICH and predict outcome based on modified Rankin score. In this study, 48 patients presenting with acute ICH presenting to a tertiary hospital in Khulna were enrolled. The 30-day mortality and disability were recorded, and ICH score along with modified Rankin score at presentation were calculated. In this study, the 30-day mortality rate was 27.1%; regression analysis showed the correlation between the scores (as measured by modified Rankin scale) for patient disability, intraventricular hemorrhage, the Glasgow Coma score, and volume of hematoma (>30 ml vs <30 ml) were significantly correlated with corresponding ICH scores. The ICH scale is a simple clinical grading scale which can predict mortality as well as disability in haemorrhagic stroke within 30 days that can be helpful to physicians in prioritization of their patient management and forecasting about prognosis.
