Abstract: Aims Provocation tests of coronary artery spasm are useful for the diagnosis of vasospastic angina (VSA). However, these tests are thought to have a potential risk of arrhythmic complications, including ventricular tachycardia (VT), ventricular fibrillation (VF), and brady-arrhythmias. We aimed to elucidate the safety and the clinical implications of the spasm provocation tests in the nationwide multicentre registry study by the Japanese Coronary Spasm Association.
Methods and results A total of 1244 VSA patients (M/F, 938/306; median 66 years) who underwent the spasm provocation tests were enrolled from 47 institutes. The primary endpoint was defined as major adverse cardiac events (MACEs). The provocation tests were performed with either acetylcholine (ACh, 57%) or ergonovine (40%). During the provocation tests, VT/VF and brady-arrhythmias developed at a rate of 3.2 and 2.7%, respectively. Overall incidence of arrhythmic complications was 6.8%, a comparable incidence of those during spontaneous angina attack (7.0%). Multivariable logistic regression analysis demonstrated that diffuse right coronary artery spasm ( P < 0.01) and the use of ACh ( P < 0.05) had a significant correlation with provocation-related VT/VF. During the median follow-up of 32 months, 69 patients (5.5%) reached the primary endpoint. The multivariable Cox proportional hazard model revealed that mixed (focal plus diffuse) type multivessel spasm had an important association with MACEs (adjusted hazard ratio, 2.84; 95% confidence interval, 1.34–6.03; P < 0.01), whereas provocation-related arrhythmias did not.
Conclusion The spasm provocation tests have an acceptable level of safety and the evaluation of spasm type may provide useful information for the risk prediction of VSA patients.

Abstract: Induction of haem oxygenase-1 (HO-1) as well as nitric oxide (NO) biosynthesis during tumour growth was investigated in an experimental solid tumour model (AH136B hepatoma) in rats. An immunohistochemical study showed that the inducible isoform of NO synthase (iNOS) was localized in monocyte-derived macrophages, which infiltrated interstitial spaces of solid tumour, but not in the tumour cells. Excessive production of NO in the tumour tissue was unequivocally verified by electron spin resonance spectroscopy. Tumour growth was moderately suppressed by treatment with either Nω-nitro-L-arginine methyl ester (L-NAME) or S-methylisothiourea sulphate (SMT). In contrast, HO-1 was found only in tumour cells, not in macrophages, by in situ hybridization for HO-1 mRNA. HO-1 expression in AH136B cells in culture was strongly enhanced by an NO (NO+) donor S-nitroso-N-acetyl penicillamine. HO-1 mRNA expression in the solid tumour in vivo decreased significantly after treatment with low doses of NOS inhibitors such as L-NAME and SMT (6–20 mg kg−1). However, the level of HO-1 mRNA in the solid tumour treated with higher doses of NOS inhibitor was similar to that of the solid tumour without NOS inhibitor treatment. Strong induction of HO-1 was also observed in solid tumours after occlusion or embolization of the tumour-feeding artery, indicating that ischaemic stress which may involve oxidative stress triggers HO-1 induction in the solid tumour. Lastly, it is of great importance that an HO inhibitor, zinc protoporphyrin IX injected intra-arterially to the solid tumour suppressed the tumour growth to a great extent. In conclusion, HO-1 expression in the solid tumour may confer resistance of tumour cells to hypoxic stress as well as to NO-mediated cytotoxicity. © 1999 Cancer Research Campaign

Abstract: SUMMARY Observations of Late Pleistocene and Holocene sea-level change relative to the crust exhibit very considerable variations across NW Europe in consequence of the response of the Earth’s crust to the deglaciation of Fennoscandia and of the water added to the oceans from the melting of all Late Pleistocene ice sheets. Inversion of sea-level observations from a site near the centre of the Fennoscandian ice sheet and from three sites located beyond the margin of the ice sheet at the time of maximum glaciation yield a range of plausible models for the Earth’s response and for the ice models. Further constraints on this range of models is placed by a comparison of observed sea-levels with predicted values at other sites near the former ice sheet margins. The resulting mantle parameters are: upper mantle viscosity (3-5) X 10’’ Pa s; lower mantle viscosity (2-7) X loz1 Pa s; lithospheric thickness 100150 km. These values represent effective parameters that describe the response of the Earth to surface loading of short to intermediate wavelengths on a time-scale of 104yr. The lower mantle viscosity is poorly constrained but the marked increase from upper to lower mantle is a characteristic of all plausible solutions. The inversion places a constraint on the total volume of ice in the Fennoscandian ice sheet such that the equivalent sea-level rise from this contribution is about 13-14 m. A less well-determined constraint of about 10 m equivalent sea-level rise is suggested for the Barents-Kara ice sheet. The inversion also indicates that a small amount of melt-water, from ice sheets far away from Europe, continued to be added into the oceans during Late Holocene time so as to raise the equivalent sea-level by about 3 m during the past 6000 yr, consistent with similar inversions of data from sites in the Australian and Pacific regions.
