TL;DR: Dependency levels in residential care have risen substantially, particularly in the private sector, even beyond levels expected from the greater numbers of elderly people, with the impending move to community care.
Abstract: OBJECTIVE--To determine the changes between 1979 and 1990 in demography and dependency levels in elderly people in residential care. DESIGN--Censuses of those aged 65 years and over in any type of residential care at midnight on 11 December 1979 and 27 November 1990. SETTING--Leicestershire District Health Authority (population 865,133, 1991 census), coterminous with county and social services boundaries. MAIN OUTCOME MEASURES--Age, sex, length of stay, and dependency levels (measured by activities of daily living). RESULTS--In 1990 (1979), 6079 (4678) elderly people were enumerated in 241 (133) establishments, a 30% increase in the numbers of elderly people in residential care and an 82% increase in the number of establishments between 1979 and 1990. Dependency levels rose between 1979 and 1990 in all but the geriatric sector, the greatest increases being found in private residential homes where the largest percentage increase in the number of residents had occurred. CONCLUSIONS--Dependency levels in residential care have risen substantially, particularly in the private sector, even beyond levels expected from the greater numbers of elderly people. With the impending move to community care, dependency levels are likely to rise further, and more appropriate staff training and medical input to homes will become necessary.

TL;DR: The data show a significant difference in the number of residual pellets in the small bowel of the treated group (n=40), when compared with the control group (P < 0.0001), and the inefficiency of gastric-emptying procedures is highlighted.
Abstract: A prospective, randomized, single-blind study was carried out to determine whether gastric content is forced into the small bowel when gastric-emptying procedures are employed in self-poisoned patients. They were asked to swallow barium-impregnated polythene pellets, immediately prior to either gastric lavage or ipecacuanha-induced emesis. A second group of patients, who did not require treatment, were used as controls. Sixty patients were recruited to the study. The data show a significant difference in the number of residual pellets in the small bowel of the treated group (n = 40), when compared with the control group (P less than 0.0001). There was no statistical difference in the number of pellets in the small bowel when the treated groups were compared with each other. In addition, the inefficiency of gastric-emptying procedures is highlighted; 58.5% of the total number of pellets ingested were retained in the gastrointestinal tract of the ipecacuanha-treated group, while 51.8% of total pellets ingested were retained in the gastric lavage-treated group.

TL;DR: Lymphatic endothelium was always negative; this proved to be a useful feature in differentiating between blood vessel and lymphatic invasion by breast carcinoma and Ulex europeus I‐peroxidase could certainly have a role in routine histopathology in this area.
Abstract: Peroxidase-labelled Ulex europeus I lectin has been applied to a series of routinely fixed and processed vascular lesions to assess its value in the identification of endothelium A group of breast carcinomas previously examined for binding of the lectin to the tumour have been re-assessed for evaluation of the use of Ulex europeus I in the determination of vascular invasion There was consistent binding to normal vascular endothelium but this could be variable, there being stronger reactivity in small vessels of some tissues All benign lesions stained showed clear definition of vascular channels but angiosarcomas were much less consistent in their staining reaction and the peroxidase-labelled lectin was considered to be a less satisfactory marker Lymphatic endothelium was always negative; this proved to be a useful feature in differentiating between blood vessel and lymphatic invasion by breast carcinoma Ulex europeus I-peroxidase could certainly have a role in routine histopathology in this area

TL;DR: Type I diabetes incidence rates for South Asian children in Leicestershire were very similar to those for children who were in the white/other ethnic group, in contrast to very low rates reported from Asia.
Abstract: Aims/hypothesis:
Estimates of incidence of Type I (insulin-dependent) diabetes mellitus in childhood populations vary around the world. This study aimed to estimate and compare the incidence of Type I diabetes in Leicestershire of children of South Asian and White or Other ethnic backgrounds.
