Among the multiple participating factors, some are particularly important in determining the outcome of tumor growth. Abstract: A modified shoulder abduction manual muscle test was incor porated in this study to demonstrate strength changes following tactile stimulation of the skin. Resistance was applied to the distal radioulnar joint and the stimulus (scratching) was applied inferior to the clavicle on the clavicular head of the pectoralis major muscle after maximum contraction. An electromechani cal device quantified the isotonic (eccentric) measurements. A standard dynamometer system (Cybex II) was used to measure isometric strength. The nondominant side was used as the "control." Two populations, a normal (random) and a strong (athletic) group, were studied. Twenty-three persons (52% women, 48% men; mean age, 27 years; mean height, 67 inches (170 cm); and mean weight, 147 Ib (66.7 kg)) were in the "normal" group and 17 persons (100% men; mean age, 25 years; mean height, 74 inches (188 cm); and mean weight, 215 Ib (97.5 kg)) were in the "strong" group. Abstract: The recently recognized mediastinal lymphoma of childhood is associated with early blood involvement and relentless, rapid progression. Component neoplastic cells characteristically exhibit diffuse growth patterns, convoluted nuclei, and T-cell markers. Occasionally, as in the present lymphoma, the tumors are composed of cellular nodules resembling the histologic pattern of nodular lymphocytic lymphoma. However, the nodules are derived from germinal centers and consequently do not represent the histologic pattern commonly associated with a more favorable prognosis.

Abstract: Dr. Nina Pearlmutter: This was the first admission to Lenox Hill Hospital for a 37-year-old woman who claimed to be in good health until 3 weeks prior to admission. When lifting a suitcase she felt pain in her back which radiated down the leg. Later, she developed malaise, weakness, and swelling of the left leg below the knee, and then noted small hard swellings on her upper back, and in the abdomen and neck. She felt feverish and had mild left upper quadrant pain, with a bloating sensation, but no nausea, vomiting, constipation, diarrhea, hematochezia or melena. Physical examination revealed a well-developed, thin, white woman in no acute distress. There were palpable lymph nodes in the left anterior cervical, right posterior cervical, axillary, and left supraclavicular regions. All nodes were firm, rubbery, nontender and movable. The abdomen was slightly distended and tympanitic to percussion. The bowel sounds were active. There was slight left upper quadrant tenderness without rebound. The liver was palpable, one finger breadth below the right costal margin. There was a subcutaneous nodule measuring 1.5 cm in the epigastrium and inguinal adenopathy bilaterally. Last year he was seen for a &dquo;medical&dquo; evaluation and the folder states that &dquo;nothing abnormal&dquo; was found. By observation there does not seem to be anything wrong with the soft palate or any other part of the articulation system. We do not have easy access to any fancy instrumentation although we might be able to send a tape (cassette) to one of the state university clinics if that would help. Help.
