Abstract: Experience with laser angioplasty in 16 coronary arteries in 9 cadaver hearts is presented. Coronary obstructions were due to experimentally created thrombi as well as to naturally occurring calcified plaques. Successful laser angioplasty was achieved in 14 of 15 arteries. One artery was sacrificed to determine factors necessary for deliberate perforation of the arterial wall. This procedure required more than 30 seconds of laser energy at 3.0 W with the catheter tip almost perpendicular to the wall. Penetration of the arterial wall occurred only in the second left anterior descending artery which was plaque-occluded because of operator inexperience.

Abstract: The first transluminal laser angioplasty in animals is reported herein. An Argon laser and a specialized coronary arterial catheter with a fiberoptic wave guide were used. Immediate histologic changes consisted of a moderate degree of intimal necrosis and some loss of elastic tissue. At 5 days the intima was repaired, but focal elastic tissue loss persisted.

Abstract: To identify the unknown liver cell type initially invaded by sporozoites of mammalian malaria, young rats were inoculated intravenously with large numbers of Plasmodium berghei sporozoites obtained from infected Anopheles stephensi mosquitoes. Fine structural studies of liver specimens obtained from the rats within 2 min after inoculation demonstrated the presence of morphologically unaltered sporozoites in the cytoplasm of hepatocytes. Many sporozoites were also observed undergoing cytolysis within the lysophagosomes of Kupffer cells, as well as other phagocytic cells. These observations strongly suggest direct infection of the hepatocyte by the sporozoite.

Abstract: The purpose of this study was to compare the manual assessment of muscle strength with a small, handheld (by the examiner) force-measuring device developed by the Institute of Sports Medicine and Athletic Trauma (ISMAT). One hundred twenty-eight patients presented with a known lower extremity orthopaedic pathology. All patients were clinically evaluated for hip abductor and hip flexor weakness in standard positions using the "break test" technique. All 128 patients were then evaluated with the ISMAT Manual Muscle Tester, a small, hand-held device which recorded the peak force (kg) required to break a muscle contraction. Three bilateral measures of hip abduction and hip flexion were recorded, averaged, and compared to the subjective clinical evaluation using a chisquare analysis. Bilateral values which were within 5% of each other were not considered significant and therefore not included in the calculations. The average hip abduction and hip flexion scores measured by the ISMAT tester were consistent with the examiner's perception of muscle weakness (P less than 0.001). The results demonstrate consistent detection of muscle weakness by the ISMAT Manual Muscle tester over a broad range of testing conditions.

Abstract: A portable manual muscle tester has an external actuator, a case capable of being held in one hand and a digital display indicating peak force applied to the actuator. Strain gages connected in a bridge circuit are located on a flexure contacted by the actuator. The flexure is a cantilever beam shaped to fit the small case and to prevent reflected or mounting stresses being registered in the force display. The circuit from the strain gage bridge to the display includes an instrumentation amplifier followed by a peak freeze circuit that includes operational amplifiers and a peak freeze capacitor. The peak freeze capacitor provides a voltage that is read by a digital volt meter. A further operational amplifier connected as an integrator feeds back from the peak freeze circuit to an input of the instrumentation amplifier to reset the meter to zero after the displayed force reading has been noted.

Because of the wide scope of this important topic, the present contribution will focus on a single aspect of this situation: The use of dreams spontaneously reported by supervisees to their supervisors as a means of conceptualizing and dealing with supervisory crisis situations.

Abstract: Summary Definitions and illustrations are provided for endometrial hyperplasias, including slight, cystic, adenomatous and polypoid types; for endometrial dysplasia; and for endometrial carcinoma in situ and invasive carcinomas. Three grades of adenocarcinoma and the diagnostic criteria of adenoacanthoma, adenosquamous carcinoma and several special types of endometrial carcinoma are described. The importance of high quality histologic preparations, a cool professional analysis of the histopathology and a conservative attitude toward the diagnosis of endometrial carcinoma are emphasized.
