Abstract: A prospective clinical and radionuclide study was done to ascertain the effect of a lateral release on patella vascularity in total knee arthroplasty. Although the basic surgical procedure was the same in all 36 knees, postoperative technetium bone scans revealed a higher incidence (9 of 16, 56.4%) of vascular compromise in knees with a lateral release than in those without a lateral release (3 of 20, 15%). Although only one clinical complication (fractured patella not requiring secondary surgery) occurred, the lateral release itself appears to have a causal relationship with patella viability. It does not seem to be the sole determinant, however, because the development of a "cold" patella in three knees that did not have a lateral release implicates other factors, such as thermal necrosis and anatomic variation of the blood supply.

Abstract: A child had a pulmonary plasma cell granuloma/fibrous histiocytoma that recurred after lobectomy in 1952 and required pneumonectomy in 1953, at which time it extended into the mediastinum at the pulmonary hilus. A metastatic nodule was removed from within the diaphragmatic musculature. The long-term favorable outcome suggests a good prognosis after adequate surgical therapy. The possible relationship of fibrous histiocytoma/plasma cell granuloma to modified measles is discussed.

Abstract: Non-Hodgkin's lymphomas with particular clinicopathologic features occur with a high incidence in homosexual men affected by AIDS. More frequently than in the general population, these lymphomas have an extranodal location including the gastrointestinal tract. We have recently observed four cases of AIDS-associated lymphomas whose primary location is in the lower rectum and anus. The patients were 27 to 44 years of age, had greatly depressed helper-suppressor T-cell ratios, and antibodies to human immunodeficiency virus (HIV). All four presented with anorectal symptoms and originally had surgery for anorectal fistulas or tumor masses. When staged, three patients had no other organ involvement, the fourth only had lymphoma in an axillary lymph node, and all four had large tumor masses confined within the rectum. In terms of histology, the lymphomas were of undifferentiated or of large cell type and of B-cell phenotype. Lymphomas with primary or major location in the rectum are a rare occurrence in the general population, and until recently, are rare even among the lymphomas associated with the immune deficiency syndrome. At the Lenox Hill Hospital in New York City, no rectal lymphoma among the 58 cases of AIDS-related lymphoma diagnosed during the past 4 years was recorded until 10 months ago. The recognition of the new feature in presentation, a neoplasm associated with AIDS, is obviously important for its early diagnosis and treatment. Its unusual occurrence at the suspected portal of entry of the HIV infection is of interest because it may provide new clues to the association between AIDS and neoplasia.

Abstract: Fifty patients whose surgical approach violated the arterial geniculate circulation to the patella were evaluated to determine the subsequent effect on patella viability. Comparative postoperative radionuclide scans in 100 knees, 50 control and 50 surgical, revealed an avascular insult in only 7 knees (14%). It appears that the intratendinous circulation described by Bjorkstrom and Goldie protects the patella from an avascular insult in the presence of geniculate arterial disruption.

Abstract: Fluid excess can cause hyporeninemia and hypoaldosteronemia in hemodialysis patients. In six patients on CAPD, plasma renin activity (PRA) and plasma aldosterone (PA) were elevated to levels of : PRA 10.2 +/- 2.9 nl (1-5 ng/ml/h), and PA: 47.4 +/- 16.2 (normal 5-20 ng/dl). In 4 patients, data posthemodialysis and later during CAPD revealed that PRA increased from 0.9 +/- 0.3 to 14.1 +/- 4.6 and PA increased from 3.4 +/- 0.3 to 67.4 +/- 24.9 on CAPD (p less than 0.05). Mean arterial pressure was lower on CAPD and serum glucose was higher. No significant difference was seen in weight, hematocrit, BUN, or potassium, however. Plasma volume was not decreased in five CAPD patients: 3619 +/- 358 ml (predicted 3083 +/- 201 ml). Elevated catecholamine levels were seen in CAPD patients: norepinephrine 868.0 +/- 104.1 (normal 358.4 +/- 41.5 pg/ml), epinephrine 386.3 +/- 49.2 (normal 58.3 +/- 10.6 pg/ml). Plasma vasopressin levels were elevated to the range usually seen with hyperosmolality. In eight patients who lost or gained weight on CAPD, levels of PRA and PA changed as expected, but catecholamine levels did not correlate with weight changes. The data suggest that in CAPD patients, PRA and PA may be elevated in association with augmented sympathetic stimulation and elevated vasopressin levels. Serial observations demonstrated that PRA and PA can respond appropriately to changes in body weight, while catecholamine and vasopressin levels seem to be influenced by other factors.

Abstract: Rehabilitation programs designed to restore leg, ankle, and foot function following injury frequently ignore the proximal muscles. During athletics, these knee, hip, and trunk muscles derive much of their functional power from the foot and ankle. They also serve to integrate distal segment motions into a total movement pattern such as jumping, running, or kicking. The linkage system, which is a theoretical concept, describes the normal biomechanical and physiological interactions between proximal and distal musculoskeletal structures. Immobilization or injury of distal segments interrupts the normal generation, summation, and transmission of muscular forces across joints. Adequate measures must be taken to properly assess proximal structures for weakness and tightness and to prescribe specific exercises to prevent the migration of the effects of injury away from the involved segment.

Abstract: An increased incidence of lymphoid neoplasias is associated with the states of immune deficiency both congenital and acquired. Thirty-five cases of lymphoma in males at high risk for AIDS were diagnosed in one community hospital in New York City within the past 2 years. The mean age of these patients was 39.6 years; 34 were homosexual, and one was an intravenous drug abuser. There were four Hodgkin and 31 non-Hodgkin lymphomas of various histologic types but almost all of high-grade categories. The proportion of extranodal lymphomas, the involvement of the gastrointestinal tract, central nervous system, bone marrow, and myocardium were significantly higher than in the lymphomas of the general population. The phenotypes were B-cell and non-B-non-T-cell types without any T-cell lymphomas. All patients had reversed helper-suppressor T-cell ratios, and all those tested had circulating HTLV-III and antilymphocyte antibodies. Nine patients have had previous lymph node biopsies showing the lesions of AIDS-related lymphadenopathies that were often directly associated with lymphoma. A variety of severe opportunistic infections and Kaposi sarcoma affected these patients. All lymphomas associated with immune deficiency were highly aggressive, involved multiple organs, and responded poorly to treatment resulting in early deaths.

Abstract: Pseudomosaicism is of particular concern in prenatal diagnosis when it involves mosaicism for a cytogenetic abnormality associated with a clinical syndrome which is compatible with postnatal life, such as trisomies for chromosomes 13, 18, and 21. The lack of data regarding the outcome of pregnancies involving these specific kinds of pseudomosaicism makes genetic counselling difficult. Three cases of prenatal diagnosis of pseudomosaicism for trisomy 13, each of which had a normal outcome, will be presented (Tables 1 and 2). The three main areas for consideration are: (1) the genetic counselling issues, (2) the additional prenatal diagnostic options available to evaluate the status of the fetus in an attempt to identify some of the clinical features of trisomy 13, and (3) the outcome of the pregnancies.

Abstract: Prostatic acid phosphatase (PAP) and prostate specific antigen (PSA) are markers for tumors of prostatic origin. The immunoperoxidase staining for these substances is described for both benign prostates and prostatic carcinomas, including several less frequent subtypes. A list is presented of several non prostatic tissues, which can occassionally cross-react with antibodies to PAP and PSA. PSA is found to be the antigen of choice with PAP a valuable adjunct in differential diagnosis of difficult bladder neck lesions and of metastatic tumors. The usefulness of these markers in diagnosis of fine needle aspiration cytology is also examined.

La colectomie prophylactique, fondee seulement sur la duree de la maladie, n’est pas une attitude acceptable puisqu’un sous-groupe de patients a risque plus eleve peut etre determine par l’etude histologique des biopsies obtenues par voie coloscopique. L’indication de la colectomie est basee sur la mise en evidence d’un cancer colique ou d’une dysplasie confirmee, lesion connue comme etant un precurseur du carcinome.
