TL;DR: A case of a vertically based submental island flap for maxillary reconstruction that allowed for debulking and recontouring of prior pectoralis flap and correction of submental ptosis is described.

TL;DR: A case of open irreducible dislocation of the lesser interphalangeal joints complicated by peripheral neuropathy and treated with resectional arthroplasty is presented.
Abstract: We report on the rare case of multiple open interphalangeal dislocations. This uncommon pathologic condition results from dorsiflexion and axial loading of the digit. In the present patient, closed reduction was not possible. We review the literature on this pathologic condition and present a case of open irreducible dislocation of the lesser interphalangeal joints complicated by peripheral neuropathy and treated with resectional arthroplasty.

TL;DR: Evidence is limited with mainly case reports and case series reported in the literature, NIV modalities have similar indications and contraindications in the pregnant patient as the general population, and three electronic databases were searched.
Abstract: Acute respiratory failure in pregnancy is considered a common indication for ICU admissions in the obstetrical population. NIV (i.e., NIPPV and HFNC), although controversial, have been shown to be safe and effective in the treatment of acute respiratory failure and can help prevent invasive mechanical ventilation during pregnancy in a variety of scenarios. However, evidence is limited with mainly case reports and case series reported in the literature, NIV modalities have similar indications and contraindications in the pregnant patient as the general population.

TL;DR: Older age, age over 70 with BMI of less than 25, osteoporosis, and Parkinson disease were associated with increased risk for early postoperative PPFx following DA approach THA.
Abstract: Background: Although total hip arthroplasty (THA) performed through the direct anterior (DA) approach is frequently marketed as superior to other approaches, there are concerns about increased risks of intraoperative and early postoperative femoral fracture. Purpose: We sought to assess patient-specific and radiographic risk factors for intraoperative and early postoperative (90-day) periprosthetic femoral fracture (PPFx) following DA approach THA. Methods: We retrospectively reviewed 1107 consecutive, primary, non-cemented DA THAs, performed between April 2009 and January 2015, for intraoperative and early postoperative PPFx. Patients lost to follow-up before 90 days (63), cemented or hybrid THA (52), or early femoral failure for another indication (3) were excluded, yielding 989 hips for analysis. Demographic variables and patient comorbidities were analyzed as risk factors for PPFx. Continuous variables were initially compared with 1-way analysis of variance (ANOVA) and categorical variables with chi-square test. A demographic matched-paired radiographic analysis was performed for femoral stem canal fill and compared using univariate logistic regression. Results: The incidence of perioperative PPFx was 2.02%, including 10 intraoperative and 10 early postoperative fractures. Sustaining a postoperative PPFx was associated with being 70 years old or older with a body mass index (BMI) of less than 25, or with having either osteoporosis or Parkinson disease. Radiographs demonstrated that intraoperative PPFx was associated with stems that filled greater proximally rather than distally. Conclusion: Our cohort study found older age, age over 70 with BMI of less than 25, osteoporosis, and Parkinson disease were associated with increased risk for early postoperative PPFx following DA approach THA. Intraoperative fractures may occur with disproportionate proximal femoral canal fill. Further study can evaluate whether cemented femoral components may mitigate risk in these patient populations.

TL;DR: CALGB 39802 established the feasibility of a standardized approach to video-assisted thoracoscopic surgery lobectomy while preserving core principles of individual ligation technique for the lobectomy with standard node sampling or dissection.
Abstract: Evarts Graham performed the first successful pneumonectomy for lung cancer on April 5, 1933, establishing pneumonectomy as the operative treatment of choice for the management of lung cancer at that time. In 1942, Kent and Blades from the Chest Service of the Barnes hospital published their clinical experience of the technique of individual ligation for 27 patients undergoing pulmonary lobectomy, which had previously been read at the 24th Annual Meeting of the American Association for Thoracic Surgery in Toronto. Core principles of preoperative bronchoscopy and operative ligation of the artery followed by ligation of the vein and then bronchus were established. Several decades later, in 1962, Shimkin and colleagues compared case series of pneumonectomy and lobectomy performed by Dr Overholt and Dr Ochsne and demonstrated that “survival after lobectomy was equivalent to pneumonectomy with fewer complications.” CALGB 39802 established the feasibility of a standardized approach to video-assisted thoracoscopic surgery lobectomy while preserving core principles of individual ligation technique for the lobectomy with standard node sampling or dissection. Cerfolio and colleagues performed a multi-institutional retrospective review of a consecutive series of 1339 patients from 4 institutions, which demonstrated “an impressive stage-
