TL;DR: The severity of visual impairment significantly correlated with the severity of axonal loss in affected as well as the fellow eye, and RNFL thickness is an indicator of the progression ofVisual impairment in demyelinating diseases and OCT can help distinguish the etiology.
Abstract: Purpose: This purpose of this study was to find the association between severity of visual impairment and retinal nerve fiber layer (RNFL) thickness loss in different demyelinating diseases using optical coherence tomography (OCT) and, simultaneously, assess the fellow eye for subclinical RNFL thickness loss. Methods: This cross-sectional, observational study included 60 eyes of 30 patients above the age of 20 years with diagnosed cases of multiple sclerosis (MS), neuromyelitis optica (NMO), and clinically isolated syndrome (CIS) who had history of (h/o) optic neuritis (ON) attack were included. Participants included in the study group underwent best-corrected visual acuity (BCVA) measurement, color perception, swinging flashlight test, slit-lamp examination, and dilated fundus examination (DFE). Abstract: Correction of rigid clubfoot in an older child by gradual differential distraction using the Ilizarov's device or Joshi's external stabilization system (JESS) is a time-consuming procedure. We simultaneously combined the lateral column shortening by cuboid wedge resection along with the differential distraction by application of JESS fixator for the treatment of severe, rigid, neglected, or relapsed congenital talipes equinovarus (CTEV) foot deformity in 30 feet in 18 children with mean age of 5.3 years and mean follow-up of 27.6 months. The mean pretreatment Pirani score was 5.3 which improved to mean Pirani score after treatment of 1.4. Excellent results were obtained in 22 (71.33%) feet, good results in four (13.33%) feet, and poor results in four (13.33%) feet. The average period for distraction in our series was 6.3 weeks, and total average period of fixator in place was 11.2 weeks. Abstract: Background: Psoriasis is an autoimmune disorder associated with alteration of different metabolism. The present study was aimed to assess the lipid metabolism and its correlation with severity of disease and associated cardiovascular risk factors in psoriasis. Methods: Study comprises total of 60 cases of psoriasis attended the dermatology clinics at Maharaja Yashwant Rao hospital, Indore, Madhya Pradesh, India and 30 age, gender matched healthy controls. Subjects were enrolled in the study as per the inclusion criteria. Severity of the disease was assessed by PASI score. Fasting blood samples were collected and evaluated for Lipid profile and risk ratio was calculated. Results: The results indicated that serum total cholesterol, triglycerides, LDL-C,VLDL-C were significantly increased in moderate to severe cases in comparison to control and level of HDL-C significantly decreased in moderate psoriasis and highly significant decreased was observed in severe cases when compared to control. Serum triglyceride (TG), total cholesterol, low density lipoprotein showed a significant positive correlation with severity of psoriasis. Study concludes that lipid derangement correlate with the severity of disease and also acts as a good prognostic sign. Conclusions: Present study concludes that psoriatic patients should be evaluated and followed up for the risk of dyslipidemia and cardiovascular morbidity.

Abstract: Objectives
We aim to delineate the differences in demographic characteristics and hospitalization outcomes including the severity of illness, hospitalization length of stay (LOS) and cost, utilization of deep brain stimulation (DBS), and disposition in Parkinson’s disease (PD) inpatients with psychiatric comorbidities versus without psychiatric comorbidities.
Methods
We conducted a cross-sectional study using the Nationwide Inpatient Sample (NIS), included 56,844 PD inpatients (age ≥40 years), and subdivided them by inpatients into those without psychiatric comorbidities (N = 38,629) and with psychiatric comorbidities (N = 18,471). We compared the distributions of demographic characteristics and hospitalization outcomes (severity of illness, utilization ofDBS, and disposition) by performing Pearson’s chi-square test, and we measured the differences in continuous variables (i.e., age, LOS, and cost) by using the independent samples t-test.
Results
A significantly higher proportion of PD inpatients with psychiatric comorbidities were female (44.4%) and white (83%) and had a moderate loss of functioning (48.8%) compared to those without psychiatric comorbidities. PD inpatients with psychiatric comorbidities had an increased mean LOS (4.7 days vs. 3.7 days, P <0.001) but a lower mean cost ($37,445 vs. $ 41,957, P <0.001). Also, there was a significantly lower utilization of DBS in PD inpatients with psychiatric comorbidities (19.2% vs. 26.9%, P <0.001) compared to those without psychiatric comorbidities, and an adverse disposition of transfer to a skilled nursing facility/intermediate care facility (47.1% vs. 39.6%, P <0.001) compared to PD inpatients without psychiatric comorbidities.
Conclusion
Although PD patients with psychiatric comorbidities had a moderate loss of functioning, there was significant underutilization of DBS. Meanwhile, psychiatric comorbidities among PD patients led to increased LOS and transfer to skilled facilities.
