Abstract: The epidemiology of malaria in "low-transmission" areas has been underestimated. Molecular detection methods have revealed higher prevalences of malaria than conventional microscopy or rapid diagnostic tests, but these typically evaluate finger-prick capillary blood samples (∼5 μl) and therefore cannot detect parasite densities of 20 parasites/ml) was developed and validated.

Abstract: Purpose: The purpose of this study was to compare the surface roughness of nanofill (Filtek Supreme XT dentin shade and transparent shade, Filtek Z350, and Estelite Sigma), nanohybrid (Tetric EvoCeram, Ceram X, and Premise), and microhybrid resin composites (Filtek Z250, Tetric Ceram, and Clearfil AP-X) with the materials after polishing or brushing. Materials and Methods: Forty specimens of each resin composite were polymerized for 40 seconds under a matrix strip in cylindrical molds. Each type of polymer was divided into four subgroups: unpolished, polished with abrasive disks (Sof-Lex), polished with silicone-impregnated polishing devices (Astropol), and brushed with a toothbrush (Oral B) and toothpaste (Colgate). The surface roughness of each specimen was determined using a contact stylus profilometer and by observation under a scanning electron microscope. Results: No significant differences in surface roughness among the materials were found on unpolished surfaces. For the nanofill resin composites, there were no significant differences in surface roughness between the two polishing methods or among the unpolished surfaces. After brushing, the surfaces of all materials, except those made from Filtek Z350 and Filtek Supreme XT (dentin), had greater roughness than unpolished surfaces and surfaces polished with either abrasive disks or silicone devices. The scanning electron microscope studies revealed that the surface irregularities of the materials corresponded to the results obtained using the surface roughness tester. CLINICAL SIGNIFICANCE The use of nanofill resin composites made with nanoclusters demonstrated the smoothest surfaces after polishing and brushing. (J Esthet Restor Dent 19:265–275, 2007)

Abstract: We constructed chimeric dengue type 2/type 1 (DEN-2/DEN-1) viruses containing the nonstructural genes of DEN-2 16681 virus or its vaccine derivative, strain PDK-53, and the structural genes (encoding capsid protein, premembrane protein, and envelope glycoprotein) of DEN-1 16007 virus or its vaccine derivative, strain PDK-13. We previously reported that attenuation markers of DEN-2 PDK-53 virus were encoded by genetic loci located outside the structural gene region of the PDK-53 virus genome. Chimeric viruses containing the nonstructural genes of DEN-2 PDK-53 virus and the structural genes of the parental DEN-1 16007 virus retained the attenuation markers of small plaque size and temperature sensitivity in LLC-MK(2) cells, less efficient replication in C6/36 cells, and attenuation for mice. These chimeric viruses elicited higher mouse neutralizing antibody titers against DEN-1 virus than did the candidate DEN-1 PDK-13 vaccine virus or chimeric DEN-2/DEN-1 viruses containing the structural genes of the PDK-13 virus. Mutations in the envelope protein of DEN-1 PDK-13 virus affected in vitro phenotype and immunogenicity in mice. The current PDK-13 vaccine is the least efficient of the four Mahidol candidate DEN virus vaccines in human trials. The chimeric DEN-2/DEN-1 virus might be a potential DEN-1 virus vaccine candidate. This study indicated that the infectious clones derived from the candidate DEN-2 PDK-53 vaccine are promising attenuated vectors for development of chimeric flavivirus vaccines.

Abstract: Summary Background The Thai phase 3 HIV vaccine trial RV 144 showed modest efficacy of a vaccine against HIV acquisition. Baseline variables of age, sex, marital status, and risk did not modify vaccine efficacy. We did a post-hoc analysis of the trial's data to investigate behavioural risk and efficacy every 6 months after vaccination. Methods RV 144 was a randomised, multicentre, double-blind, placebo-controlled efficacy trial testing the combination of the HIV vaccines ALVAC-HIV (vCP1521) and AIDSVAX B/E to prevent HIV infection or reduce setpoint viral load. Male and female volunteers aged 18–30 years were recruited from the community. In this post-hoc analysis of the modified intention-to-treat population (16 395 participants), HIV risk behaviour was assessed with a self-administered questionnaire at the time of initial vaccination in the trial and every 6 months thereafter for 3 years. We classified participants' behaviour as low, medium, or high risk. Both the acquisition endpoint and the early viral-load endpoint were examined for interactions with risk status over time and temporal effects after vaccination. Multiple proportional hazards regression models with treatment and time-varying risk covariates were analysed. Findings Risk of acquisition of HIV was low in each risk group, but 9187 (58·2%) participants reported higher-risk behaviour at least once during the study. Participants classified as high or increasing risk at least once during follow-up were compared with those who maintained low-risk or medium-risk behaviour as a time-varying covariate, and the interaction of risk status and acquisition efficacy was significant (p=0·01), with greater benefit in low-risk individuals. Vaccine efficacy seemed to peak early—cumulative vaccine efficacy was estimated to be 60·5% (95% CI 22–80) through the 12 months after initial vaccination—and declined quickly. Vaccination did not seem to affect viral load in either early or late infections. Interpretation Future HIV vaccine trials should recognise potential interactions between challenge intensity and risk heterogeneity in both population and treatment effects. The regimen tested in the RV 144 phase 3 trial might benefit from extended immunisation schedules.
