Abstract: Purpose To evaluate the repeatability and reproducibility of a newer swept-source optical biometer and to compare it with a standard partial coherence interferometry (PCI) biometer. Setting Siriraj Hospital, Mahidol University, Bangkok, Thailand. Design Prospective comparative study. Methods One hundred eyes from 100 cataract patients were enrolled in this study. Each patient was measured with 2 optical biometers, a newer swept-source optical biometer (IOLMaster 700) and a standard partial coherence interferometry biometer (IOLMaster 500) by 2 independent operators. The keratometry, axial length (AL), anterior chamber depth, white-to-white corneal diameter, and intraocular lens (IOL) power, calculated by the SRK/T and the Haigis formulas for each device, were recorded. Intraoperator repeatability and interoperator reproducibility of both devices were analyzed using intraclass correlation coefficients (ICCs). Agreement of ocular biometry and IOL power between the 2 devices was evaluated using the Bland-Altman method. Results The repeatability and reproducibility of the swept-source and standard biometers were high for all ocular biometry parameters (ICC, 0.93-1.00). The agreement between the 2 biometers was also high (ICC, 0.92-1.00). The IOL powers obtained from both devices were not distinct. Because of the density of the cataracts, the AL in 5 eyes could be measured only by the swept-source biometer. Conclusions Repeatability and reproducibility of a swept-source optical biometer was excellent and agreement with a standard biometer was very high. Better lens penetration ability and AL measurements were obtained with the swept-source biometer than with the standard biometer. Abstract: The biochemical characteristics of 213 isolates of Burkholderia pseudomallei from patients with melioidosis and 140 isolates from the soil in central and northeastern Thailand were compared. Whereas the biochemical profiles of all the clinical isolates were similar, all soil isolates from the central area and 25% of isolates from northeastern Thailand comprised a different phenotype. This was characterised by the ability to assimilate L-arabinose (100%), adonitol (100%), 5-keto-gluconate (90%) and D-xylose (84%), but failure to assimilate dulcitol (0%), erythritol (0%) and trehalose (10%). Compared with clinical isolates, these organisms had similar antibiotic susceptibility profiles and were also recognised by a specific polyclonal antibody against B. pseudomallei. As melioidosis is rare in central Thailand, but common in the northeast, this raises the possibility that this biochemical phenotype may be less virulent, or may even represent a different species.

Abstract: Fuzzy logic approach is preferable to fix the drawbacks for reprioritization of the Risk Priority Number (RPN).Fuzzy logic could reduce the drawback of occurred Traditional FMEA in evaluation and prioritization of failures.The application of using Fuzzy FMEA in the emergency department can be adopted suitably. All of members were able to assess dependently without any bias from the team members.Fuzzy FMEA can be applied for the first time to improve decision making process in an emergency department of a public hospital. Hospitals are one of the important service industries of health care for patients. The emergency department is the heart of every hospital, because the errors or failures occurring in it will significantly affect the safety of patients and the goodwill of the hospital. Therefore, emergency departments should be monitored carefully. This study proposed the application of Fuzzy failure mode and effects analysis (FMEA) for prioritization and assessment of failures that likely occur in the working process of an emergency department. All individuals were assessed independently without the interference of team members. In addition, this method could reduce the limitations of traditional FMEA. The prioritization of risks could also help the emergency department to choose corrective actions wisely. In conclusion, the Fuzzy FMEA method was found to be suitably adopted in the emergency department. Finally, this method helped to increase the level of confidence on hospitals.

Abstract: Minoxidil was first introduced as an antihypertensive medication and the discovery of its common adverse event, hypertrichosis, led to the development of a topical formulation for promoting hair growth. To date, topical minoxidil is the mainstay treatment for androgenetic alopecia and is used as an off-label treatment for other hair loss conditions. Despite its widespread application, the exact mechanism of action of minoxidil is still not fully understood. In this article, we aim to review and update current information on the pharmacology, mechanism of action, clinical efficacy, and adverse events of topical minoxidil.
